The incidence of mycobacterium avium complex pulmonary disease (mACPD) has been globally increasing. The present study aims to provide a comprehensive analysis of clinical characteristics and treatment response among patients in the Amazon region of Brazil and conduct a retrospective cohort study at a prominent referral hospital in the State of Par, Brazil, from 2012 to 2020. The m. avium group represented 58.9% of cases, followed by m. intracellulare (35.7%), m. colombiense (3.6%), and m. chimaera (1.8%). The majority of patients (73.2%) were female and with an average age of 63 years. Primary clinical manifestations included persistent cough and sputum production. The Charlson comorbidity index (CCI) averaged 2.47, with 36.3% of patients having a CCI score equal to higher than 3. Notably, over 70% of patients had received treatment for pulmonary tuberculosis before initiating treatment for mAC infection. The fibrocavitary radiologic form was the most prevalent (55.4%), frequently exhibiting a bilateral distribution (53.6%). Antimicrobial susceptibility revealed a significant prevalence of mAC resistance to drugs in the therapeutic regimen. Despite this, most of the patients experienced clinical improvement (50%). In conclusion, this study highlights a higher prevalence of mAC infections among middle-aged women, with a history of pulmonary tuberculosis treatment and presenting the fibrocavitary radiological form was predominant.

Effect of silymarin on biochemical indicators in patients with liver disease: systematic review with meta-analysis

To evaluate the effect of silymarin on the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and gamma glutamyl transpeptidase (γGT) in patients with liver diseases. mETHODSA systematic review with meta-analysis of ramdomized and controlled clinical trials was performed, evaluating the effects of sylimarin in patients with hepatic diseases, published by January 31, 2016. Clinical trials were sought on the basis of The Cochrane Central Register of Controlled Trials in the Cochrane Library, Pubmed/medline, Scopus, Web of Science, Lilacs and Clinical Trials. The trials with adult and elderly patients of both sexes, with Liver Diseases who took oral silymarin supplementation, as extract or isolated, as well as Silymarin combined with other nutrients, were included. The trials should provide information about the intervention, such as dosages and detailing of the product used, besides the mean and standard deviation of serum levels of ALT, AST and γGT of the baseline and at the end of the intervention. RESULTSAn amount of 10904 publications were identified. From those, only 17 were included in the systematic review and 6 in the meta-analysis, according to the used selection criteria. In this meta-analysis, the results indicated a reduction of 0.26 IU/mL (95%CI: -0.46-0.07, P = 0.007) at the level of ALT and 0.53 IU/mL (95%CI: -0.74-0.32, P = 0.000) at the serum levels of AST after using the silymarin, both, statistically significant, but with no clinical relevance. There was no significant change in the γGT levels. Subgroup analyzes were also performed for the biochemical markers in relation to the type of intervention, whether silymarin isolated or associated with other nutrients and the time of intervention (whether ≥ 6 mo or mo). Objective: To analyze the severe cases of COVID-19 in Brazil in 2020 and compare those vaccinated and unvaccinated against influenza in invasive ventilation, admission in Intensive Care Unit (ICU) and deaths. method: Cross-sectional study with public data from the OpendataSUS platform, regarding confirmed severe cases for COVID-19 in Brazil in the year 2020. data were analyzed by SPSS, from the chi-square test of independence and binary logistic regression. Results: The population was 472,688 cases and 177,640 deaths, with a lethality of 37.58% in severe cases. The test of independence was highly significant in vaccinated survivors (most twofold odds ratio for invasive ventilation, ICU admission, and death in unvaccinated cases. Conclusion: We recommend mass influenza vaccination as an adjuvant in combating the COVID-19 pandemic in Brazil.

Analysis of genetic relationship among Arbutus unedo L. genotypes using RAPD and SSR markers

The strawberry tree （Arbutus unedo L.） is an underutilized, drought tolerant, fire resistant species with a south western distribution in Europe, and with ecological and putative socio-economical impact in Portugal and mediterranean countries. Our aim was to develop an appropriate set of molecular markers to enable genetic diversity to be assessed and to fingerprint Arbutus unedo genotypes for breeding and conservation purposes in Portugal. Twenty-seven trees from a broad geographic range were screened with 20 random amplified polymorphic DNA （RAPD primers） and 11 microsatellite markers （SSR）. The RAPDs generated 124 bands, 57.3% of which were polymorphic, with an expected heterozygosity of 27%. We cross-amplified 11 SSR primers developed for Vaccinium spp., and 5 were found to be polymorphic in A. unedo, with 75% of expected heterozygosity, a number of alleles of 11.6, a null allele frequency of 7.6% and a polymorphic information content of 71%. Although the SSRs were more polymorphic and informative than the RAPDs, both markers displayed high genetic variability with the gathered data. No geographic pattern was observed in the genetic variation distribution based on both marker systems, and the lack of correlation between genetic and geographical matrices was confirmed by mantel tests. Likely, no correlation was found between pairwise SSR and RAPD band-sharing matrices. These results and their implications on A. unedo breeding and conservation programs are discussed.

The growth and production modeling of individual trees of Eucalyptus urophylla plantations

Individual tree models(ITms)are classified as growth and production models for projecting current and future forest *** are more complex than other growth and production models,show a higher level of detail and,consequently,produce a better modeling ***,the accuracy and efficiency of ITms have not been properly assessed to *** this study,we estimated the growth in height,diameter,and individual tree volume of a Eucalyptus urophylla plantation by applying an *** used a continuing forest inventory dataset in which 1554 individual trees within 29 permanent plots were measured in the field over a 6-year period(24 to 72 months).Each individual tree volume was estimated for future tree *** achieve this,we adjusted the model to predict the height and diameter growth,and the probability of mortality as a function of the competition *** ITm accuracy was assessed based on the analysis of variance results and,subsequently,the multiple mean comparison test at the 5%significance *** tree volumes predicted by the ITm for the forest stand aged 72 months,beginning at ages 24,36,48,and 60 months,were compared to the field measured tree volume acquired from the 72-month forest inventory that was used as the reference *** and observed tree volumes were similar when the estimation was based on the 48-month forest *** results might help to reduce financial costs of forest inventory because the ITm produces accurate future predictions of forest stand *** estimated ITm for Eucalyptus plantations using measurement intervals up to 2 years is recommended because it significantly reduced the projected volume discrepancy compared to the field measurements.

Functional and Structural Arterial Vessel Features of Female Patients with Stable Dermatomyositis and Antisynthetase Syndrome

Introduction: Dermatomyositis (Dm) and antisynthetase syndrome (ASS) show a high frequency of metabolic syndrome, which can be preceded by endothelial dysfunction and arterial stiffness. To date, only one study has evaluated these vessel parameters in Dm, and no study of ASS exists. Therefore, the aim of the study was to assess the structural and functional arterial of arterial vessels in Dm and ASS. methods: This cross-sectional study enrolled 21 adult female patients (14 Dm and 7 ASS) who were age-, gender- and ethnicity-matched to 12 healthy individuals. Patients using lipid lowering agents or prednisone at doses ≥ 0.25 mg/kg/day, and patients with uncontrolled systemic arterial hypertension, diabetes mellitus, cardiac insufficiency, and disease activity were excluded. Arterial stiffness was evaluated using carotid-femoral pulse wave velocity (PWV), and endothelial function was evaluated using dependent flow-mediated dilatation (FmD) of the brachial artery. Results: The mean age of patients with Dm or ASS were 45.4 ± 8.6 and 44.0 ± 6.1 years, respectively (P = 1.000), and patients were predominantly of white ethnicity. Six Dm patients and three ASS patients had systemic arterial hypertension, whereas 9 Dm patients and six ASS patients had dyslipidemia. Endothelial baseline diameter, hyperemia diameter and FmD values were similar among the three groups (P > 0.05). moreover, the median FmD values were also similar between the patients with Dm and patients with ASS [8.3% (4.5% - 10.9%) vs. 6.0% (−1.8% - 8.2%);P = 0.585]. The PWV values were comparable among the three groups (P = 0.253). In addition, no difference was observed between patients with Dm and patients with ASS (7.4 ± 0.8 m/s vs. 7.4 ± 0.9 m/s;P = 1.000). Conclusions: Despite the high prevalence of dyslipidemia and systemic arterial hypertension, our female patients with stable Dm and ASS had FmD and PWV values comparable to those of the control group.

Topographical Analysis of Frontal Lobe Executive Functions in Patients Following a Stroke

Objective: This study aimed to carry out a topographical analysis of frontal lobe executive function components in post-stroke patients. methods: A descriptive, cross-sectional study. The assessment using the WCST (Wisconsin Card Sorting Test) included sixteen patients with a clinical diagnosis of CVA and frontal lobe impairment. Results from the two groups were compared (patients with right frontal lobe impairment and patients with left frontal lobe impairment). Statistical analysis was performed using the program “Statistical Package for Social Sciences” (SPSS) Version 20.0. For comparison between groups was used mann-Whitney test and considered the alpha level of 0.05 on the statistically significant results. Results: There was no statistically significant difference between the groups, thus showing that executive functions are symmetrically distributed across the frontal lobes. Conclusion: Executive functions are symmetrically organized across the two frontal lobes, since the comparison between the groups did not demonstrate a statistically significant difference for any of the variables of WSCT.

Oxidative Stress Biomarkers Response to Exercise in Brazilian Junior Soccer Players

The purpose was to analyze biomarkers of oxidative stress and muscle damage in junior soccer players undergoing intermittent exercise to verify the biochemical changes. Ten trained, healthy male soccer players (age 18.3 ± 0.7 years, body mass 74.3 ± 7.4 kg, height 175.5 ± 6.7 cm, body mass index 24.14 ± 1.15 kg/m2;mean ± SD) from the junior cate-gory of an elite Brazilian football association participated in this study. They accomplished a running test (Loughbo-rough Intermittent Shuttle Test) which simulates common soccer activity patterns. Blood samples were collected before, during and immediately after the exercise for glucose, lactate, creatinine, urea, ascorbic acid, total plasma antioxidant potential, lipid hydroperoxides, malondialdehyde and creatine kinase concentrations. During the exercise no changes were observed in biomarkers, but, immediately after there was a significant decrease in total plasma antioxidant potential (ranging from 650.37 ± 66.53 μmol.L-1 to 559.95 ± 91.38 μmol.L-1, p -1 to 8.35 ± 0.83 nmol.L-1 and from 272.01 ± 49.67 U.I.?L-1 to 304.65 ± 39.13 U.I.?L-1, p dative stress selected biomarkers in the early stage of recovery. Thus, taking into account that soccer players’ competitive careers initiate very precociously being under constant physical exhaustion, further research on junior soccer players’ physiology and health are important for more effective physical and nutritional programming.

Functional abdominal pain disorders(FAPDs) are an important and prevalent cause of functional gastrointestinal disorders among children, encompassing the diagnoses of functional dyspepsia, irritable bowel syndrome, abdominal migraine, and the one not previously present in Rome Ⅲ, functional abdominal pain not otherwise specified. In the absence of sufficiently effective and safe pharmacological treatments for this public problem, non-pharmacological therapies emerge as a viable means of treating these patients, avoiding not only possible side effects, but also unnecessary prescription, since many of the pharmacological treatments prescribed do not have good efficacy when compared to placebo. Thus, the present study provides a review of current and relevant evidence on non-pharmacological management of FAPDs, covering the most commonly indicated treatments, from cognitive behavioral therapy to meditation, acupuncture, yoga, massage, spinal manipulation, moxibustion, and physical activities. In addition, this article also analyzes the quality of publications in the area, assessing whether it is possible to state if non-pharmacological therapies are viable, safe, and sufficiently well-based for an appropriate and effective prescription of these treatments. Finally, it is possible to observe an increase not only in the number of publications on the non-pharmacological treatments for FAPDs in recent years, but also an increase in the quality of these publications. Finally, the sample selection of satisfactory age groups in these studies enables the formulation of specific guidelines for this age group, thus avoiding the need for adaptation of prescriptions initially made for adults, but for children use.
