Comparison on the Pharmacokinetics and Weight Reduction of Clobenzorex Slow Release and Immediate Release Formulations in Obese Patients

Clobenzorex is an anorexigenic drug that is widely used in mexicofor the treatment of obesity, since it helps to reduce body weight. This drug is available as immediate release capsules. To improve compliance to treatment, it was developed a new slow release formulation. In order to establish its usefulness, oral pharmacokinetics and weight reduction of slow release and immediate release formulations of clobenzorex in obese patients were compared. Sixty patients with a BmI higher than 27 kg/m2 were included in the study. Two groups of 30 patients were formed, one of them received 30 mg immediate release formulation b.i.d. and the other group received one 60 mg slow release formulation once a day, since under this scheme these formulations are prescribed. Blood samples were obtained at selected times during the first day and once weekly during 4 weeks. After the last dose, samples were obtained at selected times during 48 h. Plasma levels were determined by HPLC-mS/mS and pharmacokinetic parameters were obtained. Reduction in Cmax due to increased tmax, as well as, increased half-life were observed with the slow release formulation in comparison with immediate release formulation. Although lower plasma levels of clobenzorex were reached with the slow release formulation, reduction of body weight was similar with both products. Based on the results, it was concluded that slow release formulation of clobenzorex is an adequate formulation of clobenzorex, since pharmacokinetics and effects observed are compatible with a once a day administration.

The monitoring of urban slopes employing in-situ equipment seeks to avoid the loss of human lives midst emergency events like landslides. Cemaden (Brazilian National Centre for monitoring and Early Warnings of Natural Disasters) has a pilot network of soil moisture and rain gauge stations located in urban slopes to understand, prevent, and mitigate landslides. Due to the large volume of data produced by this system there was a need for tools for data retrieval and visualization. This paper describes the Solam (Environmental Data Observation System) software which generates automated graphs that enable researchers to analyze and comprise data from soil moisture sensors and rain gauge stations. Those results allow to research moisture variations in six different sensor levels (from 0.5 m to 3 m deep) and precipitation indexes (hourly and accumulated rainfall) for the established period. This analysis will provide information to set critical environmental thresholds on base recorded landslides.

Cytomegalovirus (CmV) and Pneumocystis jirovecii fungus are the main opportunistic microorganisms that affect transplanted individuals. Immunosuppressive drugs administered to prevent organ rejection leave the immune system vulnerable to these infections. The present report is about a kidney transplanted patient using immunosuppressants who was diagnosed with cytomegalovirus and pneumocystosis requiring admission to the intensive care unit (ICU). Female patient, 57 years old, a kidney transplanted three years ago, with comorbidities, such as systemic arterial hypertension, hypertriglyceridemia and type 2 diabetes mellitus. She was admitted to the hospital in January 2020 with a history of diarrhea, cough, malaise and weight loss of seven kg in a month. She made continuous use of the immunosuppressants tacrolimus® and mycophenolate sodium (mFS). After five days of hospitalization, she was transferred to the ICU due to refractory diarrhea, worsening renal function and respiratory pattern, requiring mechanical ventilation. Chest tomography showed changes that led to the diagnostic hypothesis of CmV pneumonia or Pneumocystis jirovecii. Treatment with Ganciclovir® and Bactrim® was started. The bronchial lavage polymerase chain reaction test confirmed the infectious condition for CmV and Pneumocystis jirovecii. Despite the drug therapy instituted, there was no improvement in the infectious condition. The patient started to present a general and progressive worsening of the clinical picture with loss of renal graft function, respiratory failure, metabolic acidosis, hemodynamic instability and severe distributive shock, evolving to death. In the present report, it was observed that after late kidney transplantation the fragility of the immune system caused by the use of immunosuppressants contributed to the development of a severe infection with CmV and Pneumocystis jirovecii. Beta-blocker therapy in elderly patients with renal dysfunction and heart failure

OBJECTIVE To assess the role of beta-blockers(BB)in patients with chronic kidney disease(CKD)aged≥75 *** AND RESULTS From January 2008 to July 2014,we included 390 consecutive patients≥75 years of age with ejection fraction≤35%and glomerular filtration rate(GFR)≤60 m L/min per 1.73 m^*** analyzed the relationship between treatment with BB and mortality or cardiovascular *** mean age of our population was 82.6±4.1 *** ejection fraction was 27.9%±6.5%.GFR was 60-45 m L/min per 1.73 m^2 in 50.3%of patients,45-30 m L/min per 1.73 m^2 in 37.4%,andm L/min per 1.73 m^2 in 12.3%.At the conclusion of follow-up,67.4%of patients were receiving *** median follow-up was28.04(IR:19.41-36.67)*** the study period,211 patients(54.1%)died and 257(65.9%)had a major cardiovascular event(death or hospitalization for heart failure).BB use was significantly associated with a reduced risk of death(HR=0.51,95%CI:0.35-0.74;Pm L/min per 1.73 m^2;HR=0.47,95%CI:0.26-0.86,Pm L/min per 1.73 m^2;HR=0.55,95%CI:0.26-1.06,P=0.007),and stages IV and V(GFRm L/min per 1.73 m~2;HR=0.29,95%CI:0.11-0.76;P=0.047).CONCLUSIONS The use of BB in elderly patients with HFr EF and renal impairment was associated with a better *** of BB should be encouraged when possible.

背景:分子流行病学在近期的结核病传播中的重要性得到了加强,但同时也发现了一些与传统流行病学的显著区别。目的:1.在城市中心区的结核病高流行区(163/100000)和有成簇危险的人群中用RFLP法确定成簇病例的百分比。2.比较传统接触者追踪法(CCT)与RFLP法的作用。分析:1997-1998年间巴塞罗纳市Ciutat Vella区收集的培养阳性的165例标本,利用IS6110和pTBN12(PGRS)序列进行RFLP分析:对251例公布的病例中的171例(68.1％)进行了接触者追踪调查。通过计算比值比(OR)、使用95％的可信区间(CI)评价它们之间的联系。多变量的分析采用逻辑回归。结果:通过RFLP分析,发现76例(46.0％)的分离株在IS6110和PGRS序列上有成簇现象,通过传统接触者追踪法(CCT),在858名接触者中,发现了30名新病人,57名病人存在关联。就RFLP和CCT调查结果来看,主要危险因素为静脉吸毒(IVDU)。44名独居者病例未包括在传统接触者追踪中,50％的病例用RFLP分析存在成簇。RFLP和CCT法的符合率为8/13(61.5％);5例不符合者为新近的非洲移民。在随后的RFLP分析中15/55例病例发现有流行病学关联。结论:成簇病例的百分率很高。CCT在发现新病例时很有用,但在追踪传播路径时存在缺陷。CCT的覆盖面需要在被忽视的个体及与RFLP结果的符合率方面提高。

Breast cancer is the number one cause of can- cer deaths among Hispanic women in the United States, and in mexico, it recently became the primary cause of cancer deaths. This malign- nancy represents a poorly understood and un- derstudied disease in Hispanic women. The ELLA Binational Breast Cancer Study was es- tablished in 2006 as a multi-center study to as- sess patterns of breast tumor markers, clinical characteristics, and their risk factors in women of mexican descent. We describe the design and implementation of the ELLA Study and provide a risk factor comparison between women in the U.S. and those in mexico based on a sample of 765 patients (364 in the U.S. and 401 in mexico). Compared to women in mexico, U.S. women had significantly (p mean live births) and breastfeeding rates (57.5% vs. 80.5%), higher use of oral contraceptives (60.7% vs. 50.1%) and hormone replacement therapy (23.3% vs. 7.6%), and higher family history of breast cancer (15.7% vs. 9.0%). Re- sults show that differences in breast cancer risk factor patterns exist between mexico and U.S. women. We provide lessons learned from the conduct of our study. Binational studies are an important step in understanding disease pat- terns and etiology for women in both countries.
