SImAP500: A novel risk score to identify recipients at higher risk of hepatocellular carcinoma recurrence following liver transplantation

BACKGROUND Recurrence of hepatocellular carcinoma(HCC)following liver transplantation(LT)has a devastating influence on recipients’survival;however,the risk of recur-rence is not routinely *** stratification is vital with a long LT waiting time,as that could influence the recurrence despite strict listing *** This study aims to identify predictors of recurrence and develop a novel risk pre-diction score to forecast HCC recurrence following *** A retrospective review of LT for HCC recipients at University Hospitals Bir-mingham between july 2011 and February *** and multivariate analyses were performed to identify recurrence predictors,based on which the novel SImAP500(satellite nodules,increase in size,microvascular invasion,AFP>500,poor differentiation)risk score was *** 234 LTs for HCC were performed with a median follow-up of 5.3 *** developed in 25 patients(10.7%).On univariate analyses,RETREAT score>3,α-fetoprotein(AFP)at listing 100-500 and>500,bridging,increased tumour size between imaging at the listing time and explant histology,increase in the size of viable tumour between listing and explant,presence of satellite nodules,micro-and macrovascular invasion on explant and poor differentiation of tumours were significantly associated with recurrence,based on which,the SImAP500 risk score is *** SImAP500 demonstrated an excellent predictive ability(c-index=0.803)and outper-formed the RETREAT score(c-index=0.73).SImAP500 is indicative of the time to disease *** SImAP500 risk score identifies the LT recipients at risk of HCC *** stratification allows patient-centric post-transplant surveillance *** validation of the score is recommended.

Comprehensive studies identify motor neuron spectrum disorders including amyotrophic lateral sclerosis(ALS)as globally rising fatal disorders with the highest prevalence in aging populations,influenced by ethnicity and ancestry(GBD 2016 motor Neuron Disease Colla borators,2018).While~10% of diagnoses involve a family history(fALS),most cases are considered sporadic(sALS).However,population-based studies suggest that even cases without a common index mutation impart heritability(Ryan et al.,2019),indicating a crucial role of rare and as yet unknown genetic denominators.

On the monotonicity of Q^(3) Spectral Element method for Laplacian

The monotonicity of discrete Laplacian, i.e., inverse positivity of stiffness matrix, implies discrete maximum principle, which is in general not true for high order accurate schemes on unstructured meshes. On the other hand,it is possible to construct high order accurate monotone schemes on structured meshes. All previously known high order accurate inverse positive schemes are or can be regarded as fourth order accurate finite difference schemes, which is either an m-matrix or a product of two m-matrices. For the Q3spectral element method for the two-dimensional Laplacian, we prove its stiffness matrix is a product of four m-matrices thus it is unconditionally monotone. Such a scheme can be regarded as a fifth order accurate finite difference scheme. Numerical tests suggest that the unconditional monotonicity of Q^(k) spectral element methods will be lost for k ≥ 9 in two dimensions, and for k ≥ 4 in three dimensions. In other words, for obtaining a high order monotone scheme, only Q^(2) and Q^(3) spectral element methods can be unconditionally monotone in three dimensions.

ENSO impacts on litter stocks and water holding capacity in secondary forests in eastern Amazonia

Among the impacts of climate change,there is the intensification of phenomena such as the El Niño South-ern Oscillation(ENSO)responsible for El Niño and La Niñ***,understanding their effects on the functional pro-cesses of forests is ***,this study evaluated the effects of ENSO on litter stock and water holding capac-ity(WHC)in a successional forest in eastern *** occurred in periods with the most rainfall in El Niño(2019)and least in La Niña(2021)*** permanent plots were used to sample *** effects were evident for WHC,higher during El Niñ***,this influence was not clear for litter,as only in the rainy season effects were *** was a positive correlation of WHC with precipitation and humidity,while litter stocks were negatively correlated with temperature and wind *** the subject of this study requires long-term assessments,preliminary results suggests that,depending on the intensity of ENSO,forest functional processes can be strongly impacted and *** conclusion reinforces warnings by the scientific community about the impacts of climate change on the maintenance of litter stocks,decomposition and,consequently,the biogeochemical cycle and essential ecosystem services for the maintenance of Amazonia *** need to develop long-term research to understand the effects of climatic change on litter stocks and water holding capacity is highlighted,especially in Amazonia.

Nephronophthisis-related ciliopathies(NPHP-RC)represent one of the most common causes of chronic kidney disease in the first three decades of life and are characterized by a broad genetic and clinical heterogeneity.1 To date,more than 90 genes have been identified that cause autosomalrecessive NPHP-RC if mutated,accounting for up to 60%of cases.1 Among these,homozygous deletions of NPHP1 are the most common *** genes localize to primary cilia,basal bodies,or the centrosome and lead to a primary ciliary disruption if mutated,thereby causing a broad phenotypical spectrum.1 Patients that suffer from NPHP-RC can either have an isolated renal phenotype,such as polyuria,polydipsia,decreased urinary concentration ability,and secondary enuresis due to loss of tubular function,or present with extrarenal symptoms including retinal degeneration,cerebellar vermis hypoplasia,or hepatic *** are often diagnosed in early adolescence,reaching end-stage renal disease before 25 years of age,but early onset and rapidly progressive forms of NPHP-RC also exist.1 Renal ultrasound indicates kidneys of normal or reduced renal length with increased echogenicity and corticomedullary cysts.
