Orthopaedic care provided by the 14th combat support hospital in support of humanitarian and disaster relief after hurricane maria in Puerto rico

On September 20,2017 Hurricane maria,a category 4 hurricane,made landfall on the eastern coast of Puerto *** was preceded by Hurricane Irma,a category 5 hurricane,which passed just off the coast 13 d *** destruction from both Hurricane Irma and maria precipitated a coordinated federal response which included the Federal Emergency management Agency(FEmA)and the United States *** United States Army dispatched the 14^th Combat Support Hospital(CSH)to Humacao,a city on the eastern side of the island where maria made *** mission of the 14^th CSH was to provide medical humanitarian aid and conduct disaster relief operations in support of the government of Puerto rico and *** the 14^th CSH deployment to Puerto rico,1157 patients were evaluated and ***-seven operative cases were performed to include 23 orthopaedic *** mean age of the orthopaedic patients treated was 45.7 years(range 13-76 years).The most common operation was irrigation and debridement of open contaminated and/or infected *** presented a mean 10.8 d from their initial injury(range 1-40 d).Fractures and infections were the most common diagnoses with the greatest delay in treatment from the initial date of *** deployment of the 14^th CSH to Puerto rico was unique in its use of air transport,language and local customs encountered,as well as deployment to a location outside the continental United *** factors coupled with the need for rapid deployment of the 14^th CSH provided valuable experience which will undoubtedly enable future success in similar endeavors.

饲粮中添加类胡萝卜素、浓缩苜蓿叶蛋白(APC)可提高牛奶中高纯度多不饱和脂肪酸的氧化稳定性。试验将6头泌乳期荷斯坦奶牛(平均产奶天数224±18 d)分配到变形的3×3拉丁方阵中(试验期为21 d,预试期14 d),饲粮中添加APC或VE,观察其对产奶量、乳成分及牛奶中高纯度多不饱和脂肪酸的影响。各组试验日粮物重比例不同,CTL组含豆粕9%;VE组含豆粕9%,并添加VE 300 U·kg^(-1);APC组含有APC 9%。奶牛皱胃每天注入亚麻籽油450 g·d^(-1)。试验结果表明,与试验期前相比,试验期牛奶中顺式-9、顺式^(-1)2 18:2脂肪含量从1.08±0.13%增长到3.9±0.40%(X±SD),同时顺式-9、顺式^(-1)2、顺式^(-1)5 18:3脂肪含量从0.40±0.04%增长到14.27±1.81%。APC组牛奶产量为14.7 kg·d^(-1),与CTL组13.4 kg·d^(-1)相比有增高趋势,远高于VE组的13.0 kg·d^(-1)。APC组牛奶中蛋白质含量为518 g·d^(-1),高于VE组的445 g·d^(-1),但与CTL组的483g·d^(-1)相比,无显著差异(P>0.05)。结果表明,奶牛饲料中添加APC,氮的利用效率更高(牛奶中含氮26.1%),而CTL组为23.0%,VE组为22.9%。APC组牛奶脂肪中叶黄素含量增加到252μg·g^(-1),而CTL组为204μg·g^(-1),VE组为190μg·g^(-1)。APC组牛奶脂肪中的VE含量为34.5μg·g^(-1),高于CTL组的19.0μg·g^(-1),但显著低于VE组的44.9μg·g^(-1)。APC组新鲜牛奶中的氧化还原电位为152 m V与VE组的144 m V相近,但低于CTL组的189 m V。各组间溶解氧含量之间差异显著(8.1±1.5 mg·L^(-1)),共轭二烯氢过氧化物无显著变化(2.7±0.5 mmol·L^(-1))。APC组挥发性脂质氧化产物浓度(如丙醛、己醛、顺-4-庚烯醛、1-辛烯-3-酮)与CTL组相比,有降低趋势,VE组除己醛降低40%外,其余结果与APC组相似。试验结果表明,泌乳奶牛饲粮中添加APC可提高日粮中蛋白质的含量;改善日粮中N的利用率,可作为采前技术来提高牛奶抗氧化水平来减少氧化。

Optical and magnetic properties of GaN epilayers implanted with ytterbium

We have studied the optical and magnetic properties of ytterbium implanted GaN epilayer grown on (0001) sapphire by metalorganic chemical vapor by deposition (mOCVD). Samples were implanted at room temperature with Yb ions at dose 4 1015 cm-2 and energy of 150 keV. The implanted samples were annealed at 1000 C in N2 at atmospheric pressure to recover implantation damages. The photoluminescence (PL), PL excitation (PLE), and PL kinetics have been studied with continuous and pulse photo-excitations in 360-1100 nm spectral range at different temperatures. The characteristic Yb3+ ion emission spectra were observed in the spectral range between 970-1050 nm. Theoretical fittings of the experimental PL temperature and PL kinetics data suggest that Yb3+ ions are involved in at least two major luminescence centers. The PLE spectra indicate that excitation of the Yb3+ ion occurs via electron-hole pair generation and complex processes. magnetization versus magnetic field curves shows an enhancement of magnetic order for Yb-implanted samples in 5 K to 300 K temperature range. The Yb-implanted GaN sample showing weak ferromagnetic behavior was compared with the ferromagnetic in situ doped GaYbN material.

Objective: We sought to evaluate the safety and efficacy of tacrolimus ointment in a large cohort of adult and pediatric patients with atopic dermatitis (AD). methods: A cohort of 3964 adult and 3959 pediatric patients with AD were enrolled in this open- label noncomparative study. Patients applied tacrolimus 0.03% or 0.1% ointment twice daily to the affected areas. Efficacy and safety assessments included percentage of body surface area affected and incidence of adverse events, respectively. Results: A total of 7923 patients were evaluated; 95.5% had severe or moderate AD at baseline. There was a 52% decrease from baseline in the mean percentage of body surface area affected at month 1 and a 91% decrease at month 18. Two of the most common adverse events, skin burning and pruritus, were generally mild, transient in nature, and decreased in prevalence as AD improved. Severity and frequency of other common adverse events were consistent with expected rates in the general population. Conclusion: Tacrolimus ointment monotherapy in almost 8000 pediatric and adult patients led to continuous improvement in AD and revealed no change in the safety profile reported in previous clinical trials.

Labeling of influenza viruses with synthetic fluorescent and biotin-labeled lipids

Direct labeling of virus particles is a powerful tool for the visualization of virus–cell interaction events. However, this technique involves the chemical modification of viral proteins that affects viral biological properties. Here we describe an alternative approach of influenza virus labeling that utilizes Function-Spacer-Lipid(FSL) constructs that can be gently inserted into the virus membrane. We assessed whether labeling with fluorescent(fluo-Ad-DOPE) or biotin-labeled(biot-CmG2-DOPE) probes has any deleterious effect on influenza virus hemagglutinin(HA) receptor specificity, neuraminidase(NA) activity, or replicative ability in vitro. Our data clearly show that neither construct significantly affected influenza virus infectivity or viral affinity to sialyl receptors. Neither construct influenced the NA activities of the influenza viruses tested, except the A/Puerto rico/8/34(H1N1) strain. Our data indicate that lipid labeling provides a powerful tool to analyze influenza virus infection in vitro.

Land Use and Land management during the Past Century Determine mangrove Dynamics in Northwestern Puerto rico： the Case of the maracayo mangrove

Political and economic decisions have determined throughout the history of Puerto rico land use for agriculture, livestock and urban sprawl. Knowing this, this study is imperative to understand how these changes caused by the various uses and management affected adjoining wetlands. It is hypothesized that these changes affected the hydrology of the area, resulting in increased salinity, providing the right niche for the development of current mangrove. The resources used were aerial photographs, oral history, a report done in 1979 by the Department of Natural and Environmental Resources of the Government of Puerto rico and analyses of salinity, pH and conductivity of soils in three different sites. The conclusion of this study was that the wetland underwent changes in ecosystem composition by ambitious elimination of sand dunes due to hydrological changes and marine effects. The oral history confirms the presence of springs in the past and present. Salinity intrusion was documented in those springs in the 1979 report due to land use change and elimination of sand dunes, changing the habitat, therefore allowing a mangrove community to be established. The continuity of ecophysiological and hydrogeological studies of the area will allow for a predictive understanding of how the mangrove wetland will continue developing.

Green coffee seeds from Colombia, Guatemala, India, Kenya, Papua New Guinea, Puerto rico and Vietnam were sampled for the presence of fungal endophytes. Stions of surface sterilized seeds were plated on yeast malt agar, and fungal growth was isolated for subsequent DNA extraction and sequencing. Several fungal genera were isolated, including Acremonium, Aspergillus, Eurotium, Fusarium, Gibberella, Penicillium, Pseudozyma and an undescribed clavicipitaceous species. The biological activities that these fungi might be playing in coffee seeds remain unknown, but in other plants some of the genera isolated have been reported to protect against plant pathogens.

Spinal cord injury（SCI） is a condition without a cure,affecting sensory and/or motor *** physical trauma to the spinal cord initiates a cascade of molecular and cellular events that generates a non-permissive environment for cell survival and axonal *** these complex set of events are damage of the blood-brain barrier,edema formation,inflammation,oxidative stress,demyelination,reactive gliosis and *** multiple events activated after SCI require a multi-active drug that could target most of these events and produce a permissive environment for cell survival,regeneration,vascular reorganization and synaptic ***,a selective estrogen receptor modulator,is an FDA approved drug with several neuroprotective properties that should be considered for the treatment of this devastating *** investigators using different animal models and injury parameters have demonstrated the beneficial effects of this drug to improve functional locomotor recovery after *** suggest that the mechanism of action of Tamoxifen administration is to modulate anti-oxidant,anti-inflammatory and anti-gliotic responses.A gap of knowledge exists regarding the sex differences in response to Tamoxifen and the therapeutic window available to administer this *** addition,the effects of Tamoxifen in axonal outgrowth or synapse formation needs to be *** review will address some of the mechanisms activated by Tamoxifen after SCI and the results recently published by investigators in the field.

Prevalence of refractive errors in children of Puerto rico

AIm:To determine the prevalence of refractive error in 5-to 17-year-old schoolchildren in Puerto ***:A quantitative descriptive study of 2867 children aged 5 to 17y from all seven educational regions of Puerto rico was conducted from 2016–*** error was determined via static and subjective *** with distance acuity≤20/40 or near visual acuity≤20/32 had a cycloplegic *** analysis included descriptive statistics,correlation coefficient,Kruskal-Wallis,Chi-square,and t test ***:Twenty percent of the children had a spherical equivalent refractive error≤-0.50 D,3.2%had a spherical equivalent≥+2.00 D,and 10.4%had astigmatism≥1 *** was a statistically(but non-clinically)significant myopic change in spherical equivalent refractive error with age(Pmyopia increased with age(Pmatism(P=0.51).males had a significantly higher hyperopic spherical equivalent than females(Pmales had a higher prevalence of myopia(Pmales,but there was no difference in the hyperopia(P=0.74)or astigmatism prevalence(P=0.87).CONCLUSION:The prevalence of a spherical equivalent equal to or less than-0.50 D(myopia,20.7%)is one of the highest among similar-aged children *** studies should explore the rate of myopia progression in children in Puerto *** children must be monitored to examine the need for treatment of myopia progression.

We present a new approach to model selection and Bayes factor determination,based on Laplaceexpansions(as in BIC),which we call Prior-based Bayes Information Criterion(PBIC).In thisapproach,the Laplace expansion is only done with the likelihood function,and then a suitableprior distribution is chosen to allow exact computation of the(approximate)marginal likelihoodarising from the Laplace approximation and the *** result is a closed-form expression similar to BIC,but now involves a term arising from the prior distribution(which BIC ignores)andalso incorporates the idea that different parameters can have different effective sample sizes(whereas BIC only allows one overall sample size n).We also consider a modification of PBIC whichis more favourable to complex models.
