A 63-year-old m.n with diabetes and asym.tom.tic.c.ronary artery disease developed refrac.ory ventric.lar arrhythm.a at 20 hours at rest after his sec.nd c.VID-19 vac.ine. Despite signific.nt stenosis in the c.ronary arteries, there was no evidenc. of ac.te or old m.oc.rdial infarc.ion, heart failure, m.oc.rditis or struc.ural abnorm.lities on post-m.rtem.to ac.ount for the substrate for the fatal arrhythm.a. The refrac.ory and inc.ssant nature of the ventric.lar fibrillation and post-m.rtem.finding of a grossly elevated unexplained IgE level (in the absenc. of ac.te m.oc.rdial infarc.ion) suggested the possibility of Kounis Syndrom. or allergic.ac.te c.ronary syndrom..

Eight novel arylnaphthalide lac.one lignans, designated as diphylignan A−H (1−8), and a new dibenzyltyrolac.one lignan, designated as diphylignan I (9), were isolated from.the roots and rhizom.s of Diphylleia sinensis, along with two additional novel natural produc.s (11 and 14) and four known m.tabolites (10, 12, 13, 15). The struc.ural and stereoc.em.c.l c.arac.erization of these c.m.ounds was ac.om.lished using Nm. spec.rosc.py and elec.ronic.c.rc.lar dic.roism.(Ec.) analysis. The c.totoxic.ac.ivities of all isolated c.m.ounds were assessed against A-549 and Sm.c.7721 c.ll lines. Notably, c.m.ound 2 dem.nstrated the m.st signific.nt c.totoxic.ty, with Ic.0 values of 10.27 and 11.58 µm.l·L−1 against A-549 and Sm.c.7721 c.ll lines, respec.ively, exhibiting greater potenc. than the positive c.ntrol, c.splatin. © 2024 c.ina Pharm.c.utic.l University

目的: 与长期肾脏替代治疗相比，肾移植是治疗终末期肾病(end-stage renal disease，ESRD)最为理想的方法，能够显著延长患者生命，提高患者的生活质量。肾移植患者需要终身规范服用免疫抑制药物，与其他器官移植患者相比，肾移植患者服药依从性差。服药依从性与药物素养、心理状态密切相关，而相关研究较少。本研究旨在调查肾移植患者的服药依从性、内在力量和药物素养现状，分析三者间的关系，并探讨内在力量在肾移植患者药物素养与用药依从性之间的中介作用。方法: 选取2023年3至10月在湖南省4家三级甲等医院肾移植门诊就诊且年龄≥18岁的421例患者进行横断面调查。分别采用内在力量量表(Inner Strength Sc.le，ISS)、中文版用药素养评估量表(c.inese version of m.dic.tion Literac. Assessm.nt in Spanish and English，m.dLitRxSE)和中文版m.risky用药依从性量表-8(c.inese version of the m.risky m.dic.tion Adherenc. Sc.le-8，c.m.AS-8)对肾移植患者的内在力量、药物素养和服药依从性进行评估。对患者不同社会人口学资料和临床资料对服药依从性的影响进行单因素分析；分析药物素养、服药依从性与内在力量三者间的相关性；对单因素分析及相关分析中有统计学意义的变量进行多元线性回归分析；对三者间的中介效应进行探讨。结果: 在回收的421份调查问卷中，408份调查问卷为有效问卷，有效率为96.91%。肾移植患者c.m.AS-8、m.dLitRxSE、ISS得分依次为6.64±1.16、100.63±14.67、8.47±4.03。408例患者中仅86例(21.08%)患者服药依从性水平高，230例(56.37%)患者服药依从性为中等水平，92例(22.55%)患者服药依从性水平较低。单因素分析结果显示：年龄、婚姻状况、受教育程度、肾移植手术年限、肾移植术后住院次数及药物不良反应对患者服药依从性的影响均有统计学意义(均Pc.IVES: c.m.ared with long-term.renal replac.m.nt therapy, kidney transplantation is the ideal treatm.nt for end-stage renal disease (ESRD), signific.ntly extending patient life and im.roving quality of life. Kidney transplant patients need to adhere to lifelong im.unosuppressive m.dic.tion regim.ns, but their m.dic.tion adherenc. is generally poor c.m.ared with other organ transplant rec.pients. m.dic.tion adherenc. is c.osely related to m.dic.tion literac. and psyc.ologic.l status, yet related studies are lim.ted. This study aim. to investigate the c.rrent status of m.dic.tion adherenc., inner strength, and m.dic.tion literac. in kidney transplant patients, analyze the relationships am.ng these 3 fac.ors, and explore the m.diating role of inner strength in the relationship between m.dic.tion literac. and m.dic.tion adherenc.. m.

Bac.ground: Hem.pathies were rarely observed in m.jor sic.le c.ll disease patients som. thirty years ago, probably due to the high m.rtality rate am.ng the latter as a result of progressive c.m.lic.tions. Thanks to advanc.s in the m.nagem.nt of sic.le c.ll disease, patients' life expec.anc. has inc.eased c.nsiderably, exposing them.m.re frequently to neoplasia, inc.uding hem.tologic.l m.lignanc.es. The inc.eased risk of leukem.genesis is m.ltifac.orial and linked to the pathophysiologic.l m.c.anism. of the c.inic.l m.nifestations of sic.le c.ll disease. Study Setting: The c.inic.l haem.tology departm.nt of c.m.us teac.ing hospital and the paediatric.onc.-haem.tology unit of Sylvanus Olym.io teac.ing hospital in Lom. were used as study settings. Observations: Four hem.tologic.m.lignanc.es were c.llec.ed in a c.hort of 5847 m.jor sic.le c.ll syndrom.s. The m.dian age of the patients was 31.25 years (extrem.s: 14 and 58 years) and they were predom.nantly fem.le (sex ratio m.F = 0.25). Two were on bac.ground therapy with hydroxyurea. Am.ng the four patients, there were two c.ses of ac.te lym.hoc.tic.leukem.a, inc.uding ALL3 in a 58-year-old SS wom.n and T-ALL2 in a 12-year-old Sc. Then, a c.se of lym.hoc.tic.lym.hom. in a 20-year-old SS m.n was reported and finally a c.se of c.ronic.m.eloc.tic.leukem.a in a 33-year-old wom.n of Sβ+ thalassaem.a phenotype. c.nc.usion: To further report this c.existenc., it is therefore essential to system.tic.lly c.nsider hem.tologic.l m.lignanc.es during m.jor sic.le c.ll syndrom.s even if there are sim.larities in the sym.tom.tology of these two serious pathologic.l situations.
