This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

A Study of Address Shortage in a Tree Based ZigBee Network for Mobile Health Applications

There are increasing demands for mobile health applications. This paper reports the development of a mobile health profile which dedicates to mobile applications. The mobile health profile is developed in association with the ZigBee Health Care profile and the IEEE 11073 standard which is normally applied to non-mobile applications. Since mobile sensors have to be carried by patients, the mobile health profile must facilitate mobility. In this investigation, a ZigBee fixed-mobile network (ZFMN) is defined and developed to supplement the ZigBee Health Care Profile for patient monitoring. The mobility study of ZigBee is performed using a random waypoint OPNET simulation model. In a ZFMN, the critical issue of address shortage is identified and discussed. It is analyzed that the problematic address shortage in a ZFMN may generate a huge amount of orphaned end devices and thus the packet drop percentage may potentially rise to 70%, rendering the network unable to function properly. Without introducing additional governing schemes, it is evaluated that the communication of the entire ZigBee network may paralyze. Further vigorous test are performed (by OPNET) on the communication capability of ZFMN when devices are randomly moving and sending data in 1s. It is vital to point out that under the adverse condition of address shortage, the performance of a ZFMN is still encouraging as long as the packet drop percentage has been kept below 3% before running out of address. The conclusion drawn in this analysis is that the packet drop percentage should be kept below 3% to provide a satisfactory QoS for an effective mobile health application using ZFMN such as patient monitoring. Such finding is also important for other future mobile application design of ZigBee. The address shortage issue is left as an open problem that needs attention for a resolution.

The widely adoption of Electric Vehicle (EV) has been identified as a major challenge for future development of smart grids. The ever increasing electric vehicle charging further increases the energy demand. This paper reports the development of an Advanced Metering Infrastructure (AMI) as an effective tool to reshape the load profile of EV charging by adopting appropriate demand side management strategy. This paper presents a total solution for EV charging service platform (EVAMI) based on power line and internet communication. It must be stressed that the development of Third Party Customer Service Platform in this investigation facilitates a single bill to be issued to EV owners. Hence, EV owners understand their energy usage and thus may perform energy saving activity efficiently. Experiment and evaluation of the proposed system show that the throughput achieved is about 5 Mbps at 10 ms end to end delay in Power line Communication. By introducing two dimensional dynamic pricing and charging schedule, the proposed EVAMI successfully reduces 36% peak consumption and increases the “off peak” consumption by 54%. Therefore the EVAMI does not only reduce the peak consumption but also relocates the energy demand effectively.

Comparing 11 early warning scores and three shock indices in early sepsis prediction in the emergency department

BACKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROC]0.75,95%CI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%CI 0.37-0.52)and 0.88(95%CI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

A flexible extra broadband metamaterial absorber(MMA)stacked with five layers working at 2 GHz–40 GHz is *** layer is composed of polyvinyl chloride(PVC),polyimide(PI),and a frequency selective surface(FSS),which is printed on PI using conductive *** investigate this absorber,both one-dimensional analogous circuit analysis and three-dimensional full-wave simulation based on a physical model are *** crucial electromagnetic properties,such as absorption,effective impedance,complex permittivity and permeability,electric current distribution and magnetic field distribution at resonant peak points,are studied in *** shows that the working frequency of this absorber covers entire S,C,X,Ku,K and ka bands with a minimum thickness of 0.098λ_(max)(λ_(max) is the maximum wavelength in the absorption band),and the fractional bandwidth(FBW)reaches 181.1%.Moreover,the reflection coefficient is less than-10 dB at 1.998 GHz–40.056 GHz at normal incidence,and the absorptivity of the plane wave is greater than 80%when the incident angle is smaller than 50°.Furthermore,the proposed absorber is experimentally validated,and the experimental results show good agreement with the simulation results,which demonstrates the potential applicability of this absorber at 2 GHz–40 GHz.

The precise building performance assessment of residential housings in subtropical regions is usually more difficult than that for the commercial premises due to the much more complicated behavior of the occupants with regard to the change in indoor *** conventional use of a fixed schedule for window opening,clothing insulation and cooling equipment operation cannot reflect the real situation when the occupants respond to the change in thermal comfort,thus affecting the appropriateness of the assessment *** rectify the situation,a new modeling strategy in which the modification of the various operation schedules was based on the calculated thermal comfort(TC),was developed in this *** this new TC-based strategy,the realistic building performances under different cooling provision scenarios applied to a high-rise residential building under the near extreme weather conditions were investigated and *** was found that sole provision of ventilation fans could not meet the zone thermal comfort by over 68%of the time,and air-conditioning was *** optimal use of ventilation fans for cooling could only help reduce the total cooling energy demand by less than 12%at best which could only be realistically evaluated by adopting the present *** studies were conducted which revealed that some design factors could offer opportunities for reducing the total cooling energy under the near extreme weather conditions.

Many cities have pledged to achieve carbon *** urban water industry can also contribute its share to a carbon-neutral *** a multi-city time-series analysis approach,this study aims to assess the progress and lessons learned from the greenhouse gas(GHG)emissions management of urban water systems in four global cities:Amsterdam,Melbourne,New York City,and *** cities are advanced in setting GHG emissions reduction targets and reporting GHG emissions in their water *** four cities have reduced the GHG emissions in their water industries,compared with those from more than a decade ago(i.e.,the latest three-year moving averages are 13%–32%lower),although the emissions have“rebounded”multiple times over the *** emissions reductions were mainly due to various engineering opportunities such as solar and mini-hydro power generation,biogas valorization,sludge digestion and incineration optimization,and aeration system *** cities have recognized the many challenges in reaching carbon-neutrality goals,which include fluctuating water demand and rainfall,more carbon-intensive flood-prevention and water-supply strategies,meeting new air and water quality standards,and revising GHG emissions accounting *** study has also shown that it is difficult for the water industry to achieve carbon neutrality on its own.A collaborative approach with other sectors is needed when aiming toward the city’s carbon-neutrality *** an approach involves expanding the usual system boundary of the water industry to externally tap into both engineering and non-engineering opportunities.

Background and Aims:We aimed to perform a network meta-analysis(NWM)to examine comparative effectiveness of non-selective beta blockers(NSBBs)on prophylaxis of gastroesophageal variceal bleeding(GVB)and mortality ***:MEDLINE(OVID)and EMBASE databases were searched for eligible randomized clinical trials(RCTs)from inception to July 3,*** of interest included primary/secondary prophylaxis of GVB,failure to achieve hepatic venous pressure gradient(HVPG)decremental response,liver-related and all-cause mortality.A Bayesian NWM was performed to derive relative risk(RR)with 95%credible intervals(CrIs).The ranking probability of each NSBB was assessed by surface under cumulative ranking curve(SUCRA).Results:Thirty-three RCTs including 3,188 cirrhosis patients with gastroesophageal varices were *** with placebo,nadolol ranked first for reducing variceal bleeding[RR:0.25,(95%CrI:0.11–0.51);SUCRA:0.898],followed by carvedilol[RR:0.33,(95%CrI:0.11–0.88);SUCRA:0.692]and propranolol[RR:0.52,(95%CrI:0.37–0.75);SUCRA:0.405].Carvedilol was more effective than propranolol in achieving HVPG decremental response[RR:0.43,(95%CrI:0.26–0.69)].Carvedilol ranked first for reducing all-cause mortality[RR:0.32,(95%CrI:0.17–0.57);SUCRA:0.963],followed by nadolol[RR:0.48,(95%CI:0.29–0.77);SUCRA:0.688],and propranolol[RR:0.77,(95%CI:0.58–1.02);SUCRA:0.337].Similar findings were observed for liver-related *** ranked the *** RR of adverse events was 4.38,(95%CrI:0.33–161.4);SUCRA:0.530,followed by propranolol[RR:7.54,(95%CrI:1.90–47.89);SUCRA:0.360],and nadolol[RR:18.24,(95%CrI:91.51–390.90);SUCRA:0.158].Conclusions:Carvedilol is the preferred NSBB with better survival benefit and lower occurrence of adverse events among patients with gastroesophageal varices.

The embolization of gastric varices is an established technique for acute bleeding in patients with portal hypertension. Here, we report an attempt to embolize a gastrorenal shunt to facilitate esophagectomy in a patient with an esophageal malignancy. To our knowledge, this is the first case in the literature to highlight the role of interventional medicine in the treatment of patients with esophageal malignancy.
