Colonic preparation before colonoscopy in constipated and non-constipated patients:A randomized study

AIM: To compare the efficacy of different doses of sodium phosphate(NaP) and polyethylenglicol(PEG) alone or with bisacodyl for colonic cleansing in constipated and non-constipated ***: Three hundred and forty-nine patients,older than 18 years old,with low risk for renal damage and who were scheduled for outpatient colonoscopy were randomized to receive one of the following preparations(prep): 90 mL of NaP(prep 1);45 mL of NaP + 20 mg of bisacodyl(prep 2);4 L of PEG(prep 3) or 2 L of PEG + 20 mg of bisacodyl(prep 4).Randomization was stratified by ***,endoscopists,endoscopists' assistants and data analysts were blinded.A blinding challenge was performed to endoscopist in order to reassure *** primary outcome was the efficacy of colonic cleansing using a previous reported *** outcomes were tolerability,compliance,side effects,endoscopist perception about the necessity to repeat the study due to an inadequate colonic preparation and patient overall ***: Information about the primary outcome was obtained from 324 patients(93%).There were no significant differences regarding the preparation quality among different groups in the overall *** was higher in the NaP preparations being even higher in half-dose with bisacodyl: 94%(prep 1),100%(prep 2),81%(prep 3) and 87%(prep 4)(2 vs 1,3 and 4,P < 0.01;1 vs 3,4,P < 0.05).The combination of bisacodyl with NaP was associated with insomnia(P = 0.04).In non-constipated patients the preparation quality was also similar between different groups,but endoscopist appraisal about the need to repeat the study was more frequent in the half-dose PEG plus bisacodyl than in whole dose NaP preparation: 11%(prep 4) vs 2%(prep 1)(P < 0.05).Compliance in this group was also higher with the NaP preparations: 95%(prep 1),100%(prep2) vs 80%(prep 3)(P < 0.05).Bisacodyl was associated with abdominal pain: 13%(prep 1),31%(prep 2),21%(prep 3) and 29%(prep 4),(2,4 vs 1,2

BACKGROUND Obesity is a risk factor for non-alcoholic fatty liver disease(NAFLD),although obese patients with NAFLD do not always develop significant *** distribution of body fat could predict the risk of NAFLD *** To investigate the role of bioelectrical impedance-estimated visceral fat(VF)in assessing NAFLD *** In this cross-sectional study,patients with biopsy-proven NAFLD were prospectively *** patients underwent anthropometric evaluation,blood tests and bioelectrical impedance *** Between 2017 and 2020,119 patients were included[66.4%male,56 years(SD 10.7),62.2%obese,61.3%with metabolic syndrome].Sixty of them(50.4%)showed significant fibrosis(≥F2)in liver ***,VF and metabolic syndrome were associated with significant fibrosis(61 years vs 52 years,16.4 vs 13.1,73.3%vs 49.2%,respectively;P<0.001 for all).In the multivariate analysis,VF and age were independently associated with significant fibrosis(VF,OR:1.11,95%CI:1.02-1.22,P=0.02;age,OR:1.08,95%CI:1.03-1.12,P<0.01).A model including these variables showed and area under the receiver operating characteristic curve(AUROC)of 0.75,which was not inferior to transient elastography or NAFLD fibrosis score *** developed a nomogram including age and VF for assessing significant fibrosis in routine *** VF is a surrogate marker of liver fibrosis in patients with *** impedance analysis is an inexpensive and simple method that can be combined with age to guide patient referral when other resources may be unavailable.

High strength nickel based alloys are used in a multitude of advanced systems where lightweight, high power density mechanical power transmission systems are required. Components such as gears, bearings and shafts could be made significantly smaller and more durable if a major improvement in nickel based alloy mechanical properties could be achieved. An important refinement in grain size (includes nanometric level) is thought to be a promising method for achieving fundamental improvements in mechanical properties. Grain size is known to have a significant effect on the mechanical behavior of materials. One of the most favorable methods of achieving extreme grain refinement is by subjecting the materials to severe plastic deformation (SPD). The principal micro-structural variations in superalloys are the precipitation amount and morphology, grain size and the distribution of carbide precipitation that could reduce the mechanical properties of the alloys. This work shows optical and transmission electron microscopy analysis and also hardness data after severe plastic deformation (pure shear stress).

AIM To characterize esophageal endoluminal landmarks to permit radial and longitudinal esophageal orientation and accurate lesion *** Distance from the incisors and radial orientation were estimated for the main left bronchus and the left atrium landmarks in 207 consecutive patients using white light examination. A sub-study was also performed using white light followed by endoscopic ultrasound(EUS) in 25 consecutive patients to confirm the *** scope orientation throughout the exam was maintained at the natural axis,where the left esophageal quadrant corresponds to the area between 6 and 9 o'clock. When an anatomical landmark was identified, it was recorded with a photograph and its quadrant orientation and distance from the incisors were determined. The reference points to obtain the distances and radial orientation were as follows: the midpoint of the left main bronchus and the most intense pulsatile zone of the left atrium. With the video processor system set to moderate insufflation, measurements were obtained at the end of the patients' air *** The left main bronchus and left atrium esophageal landmarks were identified using white light in 99% and 100% of subjects at a mean distance of 25.8 cm(SD2.3), and 31.4 cm(SD 2.4) from the incisors, respectively. The left main bronchus landmark was found to be a tubular, concave, non-pulsatile, esophageal external compression, occupying approximately 1/4 of the circumference. The left atrium landmark was identified as a round, convex, pulsatile, esophageal external compression, occupying approximately 1/4 of the circumference. Both landmarks were identified using white light on the anterior esophageal quadrant. Recent research has challenged the idea that cervid antlers are such costly traits,supporting the assertion early-life antler investment is an honest signal of adult phenotypic ***,inferences were made based on antler measurements while growing(velvet)and thus,studies on fully-formed clean antlers are needed to avoid possible bias caused by the inter-individual variation in antler growth *** studied a semi-captive population of European roe deer inhabiting a sub-Mediterranean area(Valsemana research station)and living under optimal conditions(ad libitum food supply and veterinary care).Based on repeated measurements taken from 146 individuals,we assessed whether allocation to secondary sexual traits during early life affected body mass or antler development during ***,we evaluated which body measurements better predicted future male ***,using 488 individuals,we described age-class-specific variation in male body measurements and assessed the decline in antler size between adult and senescent stages(*** of senescence).Results agree with the assertion that there is no negative effect of a high investment in antler during early life on adult antler size or body mass,but we suggest the use of clean antlers as a more robust and reliable *** variables that better predicted body mass during adulthood were yearling body mass and body size at 6 *** score between 10 and 18 months resulted in the best indicator of adult antler ***,we support the idea that the magnitude of senescence in antler size is smaller in environments with higher resource availability during winter.

The effect of chromium addition on the structure and electrical properties of CaCu3Ti4O12 is *** based on the formula CaCu3Ti4-xCrxO12-σ(x = 0, 0.025, 0.050 and 0.075 mol%) are prepared by solid-state reaction. Pellets sintered at 1070℃ for 12 h are characterised by X-ray diffraction, scanning electron microscopy/energy dispersive spectroscopy and impedance spectroscopy. Current densityelectric field, dielectric loss and permittivity measurements are also carried out. Rietveld refinement showed the substitution of Cr(Ⅲ) on the Ti(Ⅳ) site, with the maximum substitution for x = 0.025 mol%.The generation of electrical defects in the grain boundary region by chromium doping is responsible for increasing the electric breakdown field strength and nonlinearity coefficient at room temperature from 1723 V/cm and 5.29 for x = 0 mol% to 3431 V/cm and 8.16 for x = 0.025 mol%. This composition shows the greatest improvement in ceramic varistor parameters.
