BACKGROUND Peroneal tendon disorders are common causes of lateral hindfoot ***,total rupture of the peroneal longus tendon is *** treatment for this condition is usually a side-to-side tenodesis of the peroneal longus tendon to the peroneal brevis *** the traditional procedure involves a long lateral curved incision,this approach is associated with damage to the lateral soft tissues(up to 24%incidence).CASE SUMMARY A 50-year-old female had developed pain at the lateral aspect of the hindfoot 1 mo after an ankle sprain while walking in the *** treatments were anti-inflammatory drugs,ice,rest and Cam-walker *** physical exam,there was pain and swelling over the course of the peroneal *** instability and cavovarus foot deformity were ruled *** strength was weak(4/5).Imaging showed complete rupture of the peroneal longus tendon associated with a sharp hypertrophic peroneal *** repair was indicated after failure of conservative treatment(physiotherapy,rest,analgesics,and ankle stabilizer).A less invasive approach was performed for peroneal longus tendon debridement and side-to-side tenodesis to the adjacent peroneal brevis tendon,with successful clinical and functional *** Peroneus longus tendon tenodesis can be performed through a less invasive approach with preservation of the lateral soft tissue integrity.

Distribution of polychaetes in the shallow, sublittoral zone of Admiralty Bay, King George Island, Antarctica in the early and late austral summer

This study assessed the spatial distribution pa- ttern of soft-sediment polychaetes on the near-shore of Admiralty Bay, King George Island, Antarctica. In the early and late summer of 2003 /04, seven sites at three different depths (20,30 and 60 meters) were sampled using a van Veen grab. 8,668 individuals all told, belonging to 67 species and 23 families, were identified. The families Terebellidae, Syllidae and Maldanidae were the most speciose. Mean densities ranged from 45.2 to 388.1 ind. 0.1 m-2 in the early summer, and from 29 to 183 ind.0.1m-2 in the late. The species Aphelochaeta cincinnata, Levinsenia gracilis and Rhodine antarctica were the most frequent and abundant. Initially, mean biomass ranged from 0.11 to 5.27 g.0.1 m-2, in the early season and from 0.35 to 5.86 g.0.1 m-2 towards the end. Aglaophamus trissophyllus, Eupolymnia sp. and Barrukia cristata were the species with the highest biomass. Polychaete taxocoenosis structure remained similar in both periods. In the early summer, mean densities, biomass and number of species were lower at 30 meters and higher at 60, whereas in the late, these differences were higher among transects. Ice impacts, mainly anchor-ice, in the early summer, as well as icebergs later on, most likely caused the differences encountered.

The development of a Loop-Mediated Isothermal Amplification (LAMP) Procedure for Plague Diagnostic

Plague caused by Yersinia pestis is one of the infectious diseases subject to the International Health Regulations (IHR). Permanent monitoring of the focal plague areas is mandatory in order to enable prompt control measures to prevent the spread of the disease. Therefore, the availability of efficient diagnosis tests is of paramount importance. Here, we describe a loop-mediated isothermal amplification (LAMP)-based procedure for rapid Y. pestis detection. We constructed a set of LAMP primers, which were used in assays to establish the reaction conditions that would lead to the quick visualization of the results by evaluating the test tube with the naked eye. The primers were specifically designed to target the caf1 gene located on pFra/Tox (pMT), a prototypical plasmid of Y. pestis. The LAMP procedure was performed at 65°C for 45 min in a water bath and allowed for the detection of at least 10 pg of bacterial DNA. Due to its simplicity, specificity, sensitivity and rapidity, the LAMP technique is an additional tool that may be implemented in routine plague diagnoses, especially in emergencies.

Perivascular epithelioid cell tumor of the liver coexisting with a gastrointestinal stromal tumor

Approximately 10% of patients with gastrointestinal stromal tumors (GIST) develop other neoplasms, either synchronously or metachronously. In this report we describe coexistence of a gastrointestinal stromal tumor and a hepatic perivascular epithelioid cell tumor (PEComa) in a 51-year-old woman with no evidence of tuberous sclerosis. A subcapsular hepatic nodule (0.8 cm in diameter) was found during surgery for symptomatic gastric neoplasm (15 cm in diameter) arising from the lesser curvature. Both tumors revealed histomorphological and immunohistochemical features confirming a diagnosis of a small incidental hepatic PEComa and a high risky extramural gastric GIST, respectively. The patient remained disease-free 25 mo after surgery with no evidence of tumor recurrence or new neoplasms. To our knowledge, this is the first report of PEComa in a patient with GIST. Hepatic lesions detected synchronously or metachronously in patients with GISTs may represent histogenetically distinct lesions and should be sampled to confirm or exclude metastatic GISTs.

The percentage of diabetic patients with contaminated lesions increases from 3% to 10%. Treatment with herbal products shows benefits in their use, as well as antioxidant and antifungal activities. Objective: to evaluate the antimicrobial action of 10% banana peel gel and the contraction of diabetic and venous wounds. Methods: individual, analytical, interventional, longitudinal, prospective, randomized study from February to december 2015. Five patients were included in the study;3 with venous ulcer and 2 with diabetic wound. Application of 10% green banana peel gel and weekly samples were performed. After six days, the second collection was performed. The samples were seeded in the Mannitol salt agar culture medium, MacConkey agar and saboraund agar with chloramphenicol for isolation of cocci and Gram-positive and Gram-negative bacilli;and fungi. The total counts of bacteria were determined by PCA (Plate Count Agar) and measurement of the lesion margin. There was a reduction of microorganisms with the use of the gel in 53.57% of the patients, and reduction of wound areas in 48.1%.

Avocado is a fruit which had a caloric density of 1.7 kcal per gram and a half unit (~70 g) is composed by 114 kcal, 4.6 g of fibers, 345 mg of potassium, 19.5 mg of magnesium, 1.3 mg of vitamin E and 57 mg of phytosterols. Approximately 75% of fiber’s avocado contents are considered insoluble and 25% are soluble. The avocado contains lipids that consist of 71% from monounsaturated fatty acids (MUFA), 13% from polyunsaturated (PUFA) and 16% from saturated fatty acids (SFA). Recent researches have shown that avocado may improve hypercholesterolemia and may be useful in the treatment of hypertension and type 2 diabetes mellitus (T2DM). This way, avocado plays an important role in the cardiovascular health. This review summarizes the potential benefits of avocado consumption in the prevention of cardiovascular risk factors and metabolic diseases.

Applications of Power Rail Track Program to Tracings of Brazilian Railways

In Brazil, the use of software and applications in railway geometric designs is rare. Currently, after the division and privatization of the national rail network, the concessionaires who assumed the administration, are investing in modernization, recovery and extension of this transport model to make it more efficient. The PIL （Program of Investment in Logistics） has generated growth of the sector in response to the amount of investments, greatly enhancing the load capacity and optimizing the geometric design, which would allow speed of 80 km/h. Based on this information, the objective of this research is to verify the software applications of Power Rail Track Program of Bentley Systems, Inc. for geometric designs in future deployments of railways in Brazil. This software is based on numerical calculations, including roller adjustment by the least squares method to calculate and choose the best alignments, and regression tools, to a fast and effectively optimization of various parameters of the geometry for a future or existing railway. Different variables that allow you to optimize the geometric design are analyzed, considering several optional alignments and complex restrictions present in geometric design, taking into account variables such as circular curves, transition, tangents and rays. The software regression tools include curvature diagrams identifying points and curves, tangent and spirals. The analysis results are presented quickly and points can be added or deleted from the regression analysis, providing improved tracing. The software also allows to detail the elements of the executive project of the railway line such as rails, joints, turning points, waypoints and others. The result of this study has shown that the application of the Power Rail Track Program generates significant cost savings in time and increases in productivity, providing greater opportunity to optimize the geometry and reducing geometric design costs.

Hypermodern Hearts: What Makes Them Suffer and Become Ill?

In this research, we highlight the hypermodern individual, the ways he lives in the contemporary world and the psychosocial determinants involved in illness. We aim to reveal the impact of traumatic life events and the senses and meanings given by the patients to their pathologies. The research was carried out at two private hospitals in Brazil, in the years 2010 and 2011. The study is based on a qualitative method, through a semistructured interview with open-ended questions and the drawings-stories procedure, in an intentional sample, closed by saturation, during the period of hospitalization and right after leaving hospital (15 to 30 days). We used a qualitative content analysis and analysis of drawings under the framework of Analytical Psychology. Seven female patients that underwent surgery were the subjects of this research: two with Myocardial Infarction, one with Takotsubo Syndrome, three with Breast Cancer and one with both pathologies. Through the analysis of the interviews and the symbols presented in the drawings, we observed demands for hyperformance and hyperfection, linked with illness, when in excess. Traumatic life events, job stress, reactions to loss, and death of loved ones without the possibility of psychological elaboration may cause fragility and a lowering of the capacity of the immune system to react, causing damage to health. Through the symbolic trajectory of life, spontaneously presented in the drawings, we observed the three types of thinking in the structure of the psiqué which formed the basis for mental equilibrium: rational, symbolic and mythological. Infarction and breast cancer cause different impacts on women’s lives. Knowing how to deal with stressful events and being conscious of the meanings given to illness may contribute to making a better or worse experience in life. Such knowledge may be crucial for both primary and secondary prevention.

The Role of CD4+ and CD8+ T-Cells during Angiostrongylus vasorum Infection in Dogs

A determinant factor in the pathology of canine angiostrongylosis seems to be related to the location of the parasite in the definitive host. Their presence inside the arteries and its branches, promoting mechanical and metabolic action on the wall of the vessels, may alter its homeostasis. Bronchoalveolar lavage (BAL) is a procedure that retrieves cells and other elements from de lungs for evaluation, and helps in the diagnosis of many pulmonary diseases. The aim of this study was to evaluate CD4+/CD8+ lymphocyte profile during the infection by Angiostrongylus vasorum, using cells retrieved using BAL. The identification of subpopulations of T lymphocytes by evaluating the co-expression of CD4 and CD8 receptor proteins has shown that despite the increase in both populations, there was a predominance of CD4+ T-cells, instead of CD8+ T-cells. These increases of CD4+ T-cells associated with the increase of the ratio between CD4+/CD8+ suggest polarization of a Th2 response. However, the immune cells, signaling factors, and cytokines that mediate such immunity and how and where they act within the body remain largely undefined during angiostrongylosis.
