Concurrent renal cell carcinoma and hematologic malignancies: Nine case reports

BACKGROUND The presence of renal cell carcinoma(RCC)and hematologic malignancies(HM)in the same patient is rarely *** primary findings have been described in these patients,including male gender and lymphoid malignancy predominance,and the HM are usually diagnosed before or simultaneously with the *** is a lack of evidence about clinical outcomes in this *** report the common characteristics of 9 patients diagnosed with concurrent RCC and HM and their clinical course and response to *** SUMMARY Four(44%)patients were diagnosed with RCC prior to the HM,the diagnosis was simultaneous in 4(44%)patients,and 1(11%)patient was diagnosed with the HM prior to the *** patients were treated with cytotoxic chemotherapy or radiation between the diagnosis of RCC and *** unique features were seen in our case series,such as 3 simultaneous cancers in 1(11%)patient,a splenectomy leading to remission of diffuse large B cell lymphoma without the use of chemotherapy in 1(11%)patient,chemotherapy and rituximab for lymphoma resulting in a complete response in primary RCC in 1(11%)patient,and immunotherapy providing an excellent response for primary renal leiomyosarcoma in 1(11%)*** These findings highlight the potential role of immune system dysregulation in patients with the diagnosis of RCC and HM whereby the first malignancy predisposes to the second through an immunomodulatory *** have the potential of being confused with lymph node metastasis from kidney *** node biopsy may be necessary at the time of initial diagnosis or in cases of mixed response to ***-term medical surveillance is warranted when a patient is diagnosed with RCC or *** should be aware of the higher prevalence of male gender and lymphoid malignancy with concurrent RCC and HM and that either of these conditions may be diagnosed first or they may be diagnosed simultaneously.

Metastatic clear cell renal cell carcinoma in isolated retroperitoneal lymph node without evidence of primary tumor in kidneys: A case report

BACKGROUND Retroperitoneal lymph node dissection(RPLND)plays a diagnostic,therapeutic,and prognostic role in myriad urologic malignancies,including testicular carcinoma,renal cell carcinoma(RCC),and upper urinary tract urothelial *** represents 2%of all cancers with approximately 25%of patients presenting with advanced *** cell RCC(CCRCC)is the most common RCC,accounting for 75%-80%of all *** SUMMARY A 71-year-old man presented with a history of benign prostatic *** was asymptomatic without any hematuria,pain,or other urinary symptoms.A computed tomography(CT)scan of the abdomen and pelvis showed a 1.8 cm left retroperitoneal lymph *** was no evidence of renal pathology.A core biopsy was performed of the left para-aortic lymph *** the primary tumor site was unknown,the morphological and immunohistochemical features were most consistent with CCRCC.A RPLND was performed which revealed a single mass 5.5 cm in greatest dimension with extensive *** retroperitoneal lymph node was most compatible with CCRCC.A nephrectomy was not conducted as a renal mass had not been detected on any prior imaging *** patient did not receive any type of adjuvant *** patient underwent surveillance with serial CT scans with contrast of the chest,abdomen,and pelvis for the next 5 years,all of which demonstrated no recurrent or metastatic disease and no evidence of retroperitoneal *** Our unique case emphasizes the therapeutic role of metastasectomy in metastatic CCRCC even in the absence of primary tumor in the kidneys.

Personalized chemotherapy in clear cell adenocarcinoma of the urethra:A case report

BACKGROUND Clear cell adenocarcinoma of the urethra is a rare type of aggressive cancer with a poor *** cell carcinoma of the urethra represents less than 0.02%of all malignancies in *** account for 10%of female urethral carcinomas,of which 40%are the clear cell *** the presence or absence of certain mutations through genetic testing may predict whether a patient with cancer may benefit from a particular chemotherapy *** SUMMARY A 40-year-old woman presented with a 3-year history of slow urinary flow and a 3-mo history of urinary urgency and frequency as well as gross *** abdominal and pelvic computed tomography scan demonstrated enlarged lymph nodes in the abdomen and pelvis.A biopsy of a left inguinal lymph node microscopically confirmed a metastatic adenocarcinoma of the *** genetic testing determined personalized *** was treated successfully with a non-platinum-based chemotherapy consisting of paclitaxel and *** 3 cycles of paclitaxel and bevacizumab,she attained significant clinical improvement,and response by FDG-Positron emission tomography(PET)imaging showed a definite improvement in size and metabolic *** achieved complete response after 6 cycles of therapy by PET *** patient concluded 11 cycles of paclitaxel and bevacizumab,and a subsequent PET scan confirmed progression of metastatic *** patient was then treated with two cycles of doxorubicin after which a PET scan revealed a mixed response to the *** We report the first case of a patient with metastatic clear cell adenocarcinoma of the urethra who underwent personalized chemotherapy after testing for cancer gene *** unique case represents the safe and effective use of nonplatinum-based chemotherapy in clear cell adenocarcinoma of the urethra.

Thromboembolic events in metastatic testicular cancer treated with cisplatin-based chemotherapy

BACKGROUND Testicular germ cell tumor(TGCT)is the most curable solid tumor and most common cancer among men 18-39 *** cisplatin-based chemotherapy has significantly lengthened the survival of patients with TGCT,it is associated with a high rate of thromboembolic events(TEE).AIM To summarize our single-center experience highlighting patients who were diagnosed with TGCT and received platinum-based chemotherapy,with special attention to those patients who suffered a *** A retrospective analysis of the medical records and imaging studies of 68 consecutive individuals who were diagnosed with TGCT and received platinumbased chemotherapy at our Institution in a metropolitan community between January 1,2014 and December 31,*** A total of 19(28%)patients experienced a TEE following orchiectomy which occurred during chemotherapy in 13(68%)of these *** with a higher pathologic stage(stage III)were significantly(P=0.023)more likely to experience a TEE compared to patients who had a lower ***,patients who were treated with 3 cycles of bleomycine,etoposide,and cisplatin and 1 cycle of etoposide and cisplatin or 4 cycles of etoposide and cisplatin were significantly 5(P=0.02)times more likely to experience a TEE compared to patients who were treated with only 3 cycles of bleomycine,etoposide,and *** Due to numerous factors that predispose to a TEE such as large retroperitoneal disease,higher clinical stage,greater number of chemotherapy cycle,central venous catheter,cigarette smoking,and possible cannabis use,high-risk ambulatory patients with TGCT treated with cisplatin-based chemotherapy may benefit from prophylactic *** studies to evaluate the safety and efficacy of prophylactic anticoagulants are warranted in this young patient population generally devoid of medical co-morbidities.

Racial and Ethnic Differences in Orthorexia Nervosa Symptomatology and Other Disordered Eating

While some research has explored racial and ethnic differences in disordered eating, this study may be the first to examine these differences in orthorexia nervosa, involving obsessive-compulsive thoughts and behaviors concerning healthy eating, which negatively impact one’s life. Adult participants, recruited from college courses and social media, completed an online survey with the Orthorexia Nervosa Inventory (ONI) and the Eating Attitudes Test-26 (EAT-26). Regarding racial and ethnic background, 743 were White, 249 were Hispanic, 87 were Black, 61 were Asian or Pacific Islander, and 110 were biracial/multiracial. A MANCOVA revealed that the racial and ethnic groups did not differ on the ONI subscales assessing orthorexic behaviors, impairments, and emotions, after accounting for gender, BMI, and EAT-26 total scores that were covariates. In contrast, a second MANCOVA did reveal group differences on the EAT-26 subscales, after accounting for gender, BMI, and ONI total scores that were covariates. Black participants scored significantly lower than the other racial and ethnic groups on the subscale assessing dieting behaviors characteristic of anorexia nervosa, and the subscale assessing binge-eating and purging behaviors characteristic of bulimia nervosa. Further, Hispanic participants scored significantly lower than White participants on the latter subscale. These findings suggest that while orthorexic symptomatology does not differ based on race and ethnicity, a Black race and Hispanic ethnicity may be protective factors against disordered eating, perhaps related either to cultural norms concerning body image or to the resiliency and social support among the Black and Hispanic communities.

Spinal cord injuries （SCI） in rodents have been created by laceration, contusion, compression, or intramedullary injection of toxic agents. The choice of an appropriate SCI model should be made for each study based on the experimental design, with care taken to avoid unintended complications such as hemorrhage. Technical comments will be made in this communication describing the 1） importance of vertebral stabi- lization, 2） injury preparation, and 3） landmarks to improve the preci- sion and reproducibility of the SCI.

Ticino in southern Switzerland played host this summer to a most extraordinary meeting of minds. About eighty scientists from all over the world came together in the exquisite setting of Monte Verita to discuss possibly the coldest spell in our planet＇s long history： the Cryogenian Period. ‘Snowball Earth 2006＇ was organized by a research group at the ETH in Zurich under the leadership of Professor Philip Allen, now at Imperial College, London, and was the first conference to focus solely on this topic. The Cryogenian global freeze of about 7204535 million years ago closely preceded--some would say ominously so--the first fossil evidence for animal life on Earth, and as such has attracted the attention of natural scientists worldwide. Such a resurgence of interest in deep Earth history has not been witnessed since the great K/T boundary debate of the early 1980s. On the first day of the conference, on a hot and sultry evening during the hottest July on record, it was ice of a different kind that was being broken high above Lago Maggiore. The first hesitant introductions over chilled white wine made it clear that a tluly multidisciplinary throng had been invited for what promised to be a vigorous debate on the causes and effects of Neoproterozoic climate change. Gathered for the ＇snowball fight＇ were geologists, palaeobiologists, geochemists, geophysicists and climate modellers, all of whom would, one-by-one over the coming five days, borrow centre stage to offer their unique perspective on this most polarizing of topics.
