Prognostic value of circulating tumor cells combined with neutrophil-lymphocyte ratio in patients with hepatocellular carcinoma

BACKGROUND Circulating tumor cell(CTC)count and neutrophil-to-lymphocyte ratio(NLR)are both closely associated with the prognosis of hepatocellular carcinoma(HCC).AIM To investigate the prognostic value of combining these two indicators in HCC.METHODS Clinical data were collected from patients with advanced HCC who received im-mune therapy combined with targeted therapy at the Department of Oncology,the Affiliated Hospital of Southwest Medical University,Sichuan,China,from 2021 to 2023.The optimal cutoff values for CTC programmed death-ligand 1(PD-L1)(+)>1 or CTC PD-L1(+)≤1 and NLR>3.89 or NLR≤3.89 were evaluated using X-Tile software.Patients were categorized into three groups based on CTC PD-L1(+)counts and NLR:CTC-NLR(0),CTC-NLR(1),and CTC-NLR(2).The relationship between CTC-NLR and clinical variables as well as survival rates was assessed.RESULTS Patients with high CTC PD-L1(+)expression or NLR at baseline had shorter median progression-free survival(m-PFS)and median overall survival(mOS)than those with low levels of CTC PD-L1(+)or NLR(Pline CTC-NLR score may indicate low survival.CTC-NLR may serve as an effective prognostic indicator for patients with advanced HCC receiving immunotherapy combined with targeted therapy.

GW842166X Alleviates Osteoarthritis by Repressing LPS-mediated Chondrocyte Catabolism in Mice

Objective To explore the role and underlying mechanism of GW842166X on osteoarthritis and osteoarthritis-associated abnormal catabolism.Methods The extracted mouse chondrocytes were treated with GW842166X followed by lipopolysaccharide(LPS).The chondrocytes were divided into the control group,LPS group,LPS+50 nmol/L GW842166X group,and LPS+100 nmol/L GW842166X group.The cytotoxicity of GW842166X was tested using the CCK-8 assay.Western blot,RT-qPCR,and EliSA were applied to evaluate the expression of the inflammatory biomarkers in mouse chondrocytes.The expression of extracellular matrix molecules was detected by the Western blot,RT-qPCR,and immunofluorescence.Additionally,the activity of NF-κB was checked by the Western blot and immunofluorescence.The mouse Hulth models were generated to examine the in vivo effects of GW842166X on osteoarthritis.Hematoxylin and eosin staining,safranin O/fast green staining,and immunohistochemistry were applied to detect the histological changes.Results GW842166X below 200µmol/L had no cytotoxicity on the mouse chondrocytes.LPS-induced high expression of TGF-β1,IL-10,TNF-α,and IL-6 was significantly reduced by GW842166X.In addition,GW842166X upregulated the expression of aggrecan and collagen type III,which was downregulated after the LPS stimulation.The upregulated expression of ADAMTS-5 and MMP-13 by LPS stimulation was dropped in response to the GW842166X treatment.Furthermore,LPS decreased the IκBαexpression in the cytoplasm and increased the nuclear p65 expression.However,these changes were reversed by the GW842166X pretreatment.Moreover,the damages in the knees caused by the Hulth surgery in mice were restored by GW842166X.Conclusion GW842166X impeded the LPS-mediated catabolism in mouse chondrocytes,thereby inhibiting the progression of osteoarthritis.

To explore the specimen size effect of mechanical behavior of ultrafine-grained(UFG)materials with different structures,UFG Al sheets processed by equal channel angular pressing(ECAP)were selected as target materials and the dependency of tensile behavior on sheet thickness(t)was systematically investigated.The strength and ductility of ECAPed UFG Al sheets were improved synchronously as t increased from 0.2 to 0.7 mm,and then no apparent change occurred when t reached to 0.7 and 1.0 mm.The corresponding microstructure evolved from dislocation networks in equiaxed grains into the walls and subgrains and finally into the dominated cells in elongated grains or subgrains.Meanwhile,dense shear lines(SLs)and shear bands(SBs)were clearly observed and microvoids and cracks were initiated along SBs with the increase of t.These observations indicated that the plastic deformation of UFG Al sheets was jointly controlled by shear banding,dislocation sliding,and grain-boundary sliding.Furthermore,the propagation of SBs became difficult as t increased.Finally,the obtained results were discussed and compared with those of annealed UFG Al and UFG Cu.

地膜覆盖保墒已成为盐渍化土壤种植中重要的农艺措施,而盐渍化与微塑料双重胁迫对土壤微生物的影响越来越受到重视.为探究聚乙烯微塑料对盐渍化土壤微生物群落的影响,通过室内模拟盐渍化土壤环境中微塑料污染的方法,探究不同类型(氯盐类和硫酸盐类)和不同含量(弱、中、强)的盐渍化土壤赋存不同丰度聚乙烯(PE)微塑料(土样干重的1%和4%)条件下对土壤微生物群落的影响.结果表明,PE微塑料会降低盐渍化土壤微生物群落多样性和丰富度,且硫酸盐类盐渍土处理受到的影响更强烈.赋存PE微塑料后不同处理微生物组成基本一致,但其相对丰度会发生变化,硫酸盐类盐渍土处理中各菌群相对丰度的变化较氯盐类盐渍土处理更强;门水平上,变形菌门相对丰度与赋存PE微塑料丰度呈正相关,而拟杆菌门、放线菌门和酸杆菌门相对丰度与赋存PE微塑料丰度呈负相关;科水平上,黄杆菌科、食碱菌科、盐单胞菌科和鞘脂单胞菌科相对丰度随赋存PE微塑料丰度增大而增大.KEGG代谢通路预测显示,赋存PE微塑料会降低微生物新陈代谢和遗传信息等功能相对丰度,硫酸盐类盐渍土对新陈代谢功能的抑制作用强于氯盐类盐渍土,而对遗传信息功能的抑制效果弱于氯盐类盐渍土;新陈代谢功能二级通道中氨基酸代谢、碳水化合物代谢、能量代谢等功能受到抑制,推测新陈代谢功能的降低可能是由于上述二级代谢通路相对丰度降低引起的.试验结果可为微塑料和盐渍化双重污染条件下对土壤环境的影响研究提供理论依据.

Variation of the uniaxial tensile behavior of ultrafine-grained pure aluminum after cyclic pre-deformation

To explore the influence of cyclic pre-deformation on the mechanical behavior of ultrafine-grained（UFG）materials with a high stacking fault energy（SFE）,UFG Al processed by equal-channel angular pressing（ECAP）was selected as a target material and its tensile behavior at different pre-cyclic levels D（D=N_i/N_f,where N_i and N_f are the applied cycles and fatigue life at a constant stress amplitude of 50 MPa,respectively）along with the corresponding microstructures and deformation features were systematically studied.The cyclic pre-deformation treatment on the ECAPed UFG Al led to a decrease in flow stress,and a stress quasi-plateau stage was observed after yielding for all of the different-state UFG Al samples.The yield strengths_（YS）,ultimate tensile strengths_（UTS）,and uniform straineexhibited a strong dependence on D when D≤20%;however,when D was in the range from 20%to 50%,no obvious change in mechanical properties was observed.The micro-mechanism for the effect of cyclic pre-deformation on the tensile properties of the ECAPed UFG Al was revealed and compared with that of ECAPed UFG Cu through the observations of deformation features and microstructures.

High-temperature fatigue behavior of 15CrNbTi ferritic stainless steel

In order to investigate the high-temperature fatigue and fatigue-creep behavior of 15CrNbTi ferritic stainless steel at 800°C,fatigue tests without holding time in laboratory air and with 10 s holding time induced at maximum cyclic stress under argon atmosphere were conducted on an electrohydraulic servo fatigue testing machine.In addition,the morphologies of fatigue fracture surfaces were observed,and the precipitates and dislocation substructures were characterized.The experimental results show that the fatigue limit of 15CrNbTi ferritic stainless steel is 30 MPa at 800°C.The fatigue life of the experimental steel obviously decreases if holding time of 10 s is introduced at maximum cyclic stress.Under two test conditions,the fatigue cracks transgranularly initiate at the free surface of fatigue specimens and then propagate in a transgranular mode.It is dem on strated that three kinds of precipitates inclu ding(Nb,Ti)C phase,Fe2Nb phase and Fe3Nb3C phase can be observed in 15CrNbTi ferritic stainless steel,and the Fe2Nb and Fe3Nb3C phases are mainly precipitated during the fatigue tests.In the fatigue deformation area,both dislocation tangles and dislocation arrays are observed.Furthermore,the sub-grains which form during the cycling deformation with holding time of 10 s are also found.Moreover,the formation of sub-grains leads to the deterioration of the fatigue life of 15CrNbTi steel.

Corneal blindness and current major treatment concerngraft scarcity

According to World Health Organization, the global prevalence of blindness in 2010 was 39 million people, among which 4% were due to corneal opacities. Often, the sole resort for visual restoration of patients with damaged corneas is corneal transplantation. However, despite rapid developments of surgical techniques, instrumentations and immunosuppressive agents, corneal blindness remains a prevalent global health issue. This is largely due to the scarcity of good quality corneal grafts. In this review, the causes of corneal blindness, its major treatment options, and the major contributory factors of corneal graft scarcity with potential solutions are discussed.

Comparison of Intraperitoneal and Intratesticular GY Y4137 Therapy for the Treatment of Testicular Ischemia Reperfusion Injury in Rats

The efficacy of intraperitoneal GY Y4137 therapy and intratesticular GY Y4137 therapy in an experimental rat model was investigated.Four groups were set up as the sham-operation group,torsion/detorsion(T/D)group,T/D plus intraperitoneal GY Y4137(G-IP)group,and T/D plus intratesticular GY Y4137(G-IT)group.In order to establish a testicular T/D model,the left testis was opcrated and the rotation reached 720°clockwise which lasted 1 h before reperfiusion.The G-IP group accepted 100 umol/kg of GY Y4137 intraperitoneally 30 min after testicular rotation,while the G-IT group was treated with the same dose by intratesticular injection.Six h after detorsion,the testis was collcted and subsequently asssed.The T/D group showed signifcant changes in histology and an enhancement in the level of oxidative stress and apoptosis compared to the sham-operation group.The expression of Caspase-3 and Bax turmed out to be strengthened by T/D and relatively decreased with GY Y4137 treatment in both the G-IP and G-IT groups.Moreover,the Bcl-2 expression was inhibited in the T/D group,and promoted by GYY4137 in the G-IP and G-IT groups.GYY4137,moderating these observed changes,displayed a more protective cffect with G-IT therapy than G-IP therapy.This study indicated that the efficacy of intratesticular therapy with GYY4137 is better than that of intraperitoneal therapy,which may provide a more valuable approach for testicular torsion therapy.

Evaluation of the association of C5 with neovascular age-related macular degeneration and polypoidal choroidal vasculopathy

Background:Neovascular age-related macular degeneration(AMD)and polypoidal choroidal vasculopathy(PCV)are sight-threatening maculopathies with both environmental and genetic risk factors.We have previously shown relative risks posed by genes of the complement pathways to neovascular AMD and PCV.Methods:In this study,we investigated the haplotype-tagging single nucleotide polymorphisms(SNPs)in the complement component 5(C5)gene in 708 unrelated Chinese individuals:200 neovascular AMD patients,233 PCV patients and 275 controls.Six tagging SNPs in C5 were genotyped.Univariate single SNP association analysis,haplotype-based association analysis and gene-gene interaction analysis between C5 and other AMD-associated genes were performed.Results:The results revealed none of the six tagging SNPs of the C5 gene had a significant association with neovascular AMD or PCV(P>0.05).We also found insignificant haplotype-based association,and no significant SNPSNP interaction between C5 and other genes(including C2-CFB-RDBP-SKIV2L,SERPING1,CETP,ABCG1,PGF,ANGPT2,CFH and HTRA1)for neovascular AMD and PCV.Conclusions:This study showed no statistical significance in the genetic association of C5 with neovascular AMD or PCV in a Hong Kong Chinese population.Further studies in large samples from different populations are warranted to elucidate the role of C5 in the genetic susceptibility of AMD and PCV.

Clinical features and treatment outcomes of endogenous Klebsiella endophthalmitis:a 12-year review

AIM:To identify the clinical features and treatment outcomes of endogenous Klebsiella pneumoniae endophthalmitis and investigate prognostic factors of poor visual outcome.METHODS:The clinical records of all patients diagnosed with endogenous Klebsiella endophthalmitis between January 2007 to December 2018 in Prince of Wales Hospital,Hong Kong,China were retrospectively reviewed.Thorough ophthalmological examination findings were recorded in the case note,including visual acuity testing,slit-lamp examination,indirect ophthalmoscopy and B-scan ultrasonography if media opacity precluded fundus viewing.RESULTS:A total of 18 eyes in 14 patients were identified.Bilateral involvement was noted in 4 patients(28.6%).Hepatobiliary sepsis was the source in 9 patients(64.3%).Culture of intraocular fluid was positive in 5 out of 18 eyes(27.8%).Mortality was noted in 2 patients(14.3%).Mean final visual acuity was 20/1500.Six out of 16 eyes had total loss of sight(37.5%)and 3 eyes required evisceration(18.8%).Multivariate linear regression revealed poor presenting visual acuity(P=0.031)and lack of fundus view due to vitritis(P=0.02)as prognostic factors of poor visual outcome.CONCLUSION:Visual outcome of endogenous Klebsiella endophthalmitis is poor.Poor presenting visual acuity and lack of fundus view predict poor visual outcome.High index of suspicion for endophthalmitis is important in Klebsiella sepsis patients with complaints of ocular symptoms.Ophthalmological screening is recommended in noncommunicable patients with Klebsiella sepsis.
