Surgery Versus Stereotactic Radiosurgery for Single Synchronous Brain Metastasis from Non-Small Cell Lung Cancer

Objective： The aim of this study is to compare the effectiveness of surgery with stereotactic radiosurgery （SRS） for patients with a single synchronous brain metastasis from successfully treated non-small cell lung cancer. Methods： Between 1995 and 2002, 53 patients underwent resection of both primary non-small cell lung cancer and the associated single brain metastasis. There were 33 men and 20 women with a mean age of 57 years （range, 32-85 years）. At the time of diagnosis, 42 patients experienced lung cancer related symptoms, whereas 11 patients experienced brain metastases-related symptoms. 42 patients had received thoracic surgery first, and 11 patients had undergone neurosurgery or radiosurgery first. Pneumonectomy was performed in 9 out of 42 patients （21.4%）, lobectomies in 30 （71.4%）, and wedge resection in 3 （7.2%）. 48 patients （90.5%） underwent complete lymphadenectomy. 35 patients underwent brain metastasectomy. 18 underwent SRS. Results： There was no postoperative mortality and severe complications after either lung or brain surgery. Histology showed 34 adenocarcinomas, 16 squamous cell carcinomas, and 3 large cell lung cancers. 15 patients （28.3%） had no evidence of lymph node metastases （No）, 20 patients （37.7%） had hilar metastases （N1）, and 18 patients （34%） had mediastinal metastases （N2）. The 1-, 2-, 3- and 5-year overall survival rates were 49%, 19%, 10%, and 5%, respectively. The corresponding data for neurosurgery group were 55%, 17%, 11%, and 6%, respectively. The median survival time was 13 months. For SRS group the corresponding data were 44.8%, 20.9% 10.5%, and 2%, respectively. The median survival time was 14 months. The differences between the two groups were not significant （P〉0.05）. In lymph node negative patients （No）, the overall 5-year survival rate was 10%, as compared with a 1% survival rate in patients with lymph node metastases （N1-2）. 目的探讨单、双侧肺减容(LVRS)术后早期肺功能及肺血流动力学的不同变化。方法 86例重度慢性阻塞性肺气肿患者(COPD)行 LVRS 手术,单侧61例、双侧25例,术前、术后3、6个月分别测量肺功能[第1秒用力呼气量(FEV_1)、残气量(RV)、肺总量(TLC)]、动脉血气[动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)]、心脏超声多普勒检查[心输出量(CO)、心脏指数(CI)、射血分数(EF)并计算肺动脉压(PAP)],并对其结果进行比较分析。结果 80例患者痊愈出院,6例死亡;单侧 LVRS 术后3、6个月的 FEV,较术前有明显提高(P0.05)。结论单、双侧 LVRS 治疗重度 COPD 患者是安全有效的,术后早期均可明显改善患者的肺功能,但双侧手术效果优于单侧;单、双侧 LVRS 对肺血流动力学无明显负影响,术前术后无明显改变。

Objective Coronavirus disease 2019(COVID-19)and tuberculosis(TB)are major public health and social issues *** long-term follow-up of COVID-19 with pulmonary TB(PTB)survivors after discharge is *** study aimed to comprehensively describe clinical outcomes,including sequela and recurrence at 3,12,and 24 months after discharge,among COVID-19 with PTB *** From January 22,2020 to May 6,2022,with a follow-up by August 26,2022,a prospective,multicenter follow-up study was conducted on COVID-19 with PTB survivors after discharge in 13 hospitals from four provinces in *** outcomes,including sequela,recurrence of COVID-19,and PTB survivors,were collected via telephone and face-to-face interviews at 3,12,and 24 months after *** Thirty-two COVID-19 with PTB survivors were *** median age was 52(45,59)years,and 23(71.9%)were *** them,nearly two-thirds(62.5%)of the survivors were moderate,three(9.4%)were severe,and more than half(59.4%)had at least one comorbidity(PTB excluded).The proportion of COVID-19 survivors with at least one sequela symptom decreased from 40.6%at 3 months to 15.8%at 24 months,with anxiety having a higher proportion over a *** and amnesia recovered at the 12-month follow-up,while anxiety,fatigue,and trouble sleeping remained after ***,one(3.1%)case presented two recurrences of PTB and no re-positive COVID-19during the follow-up *** The proportion of long symptoms in COVID-19 with PTB survivors decreased over time,while nearly one in six still experience persistent symptoms with a higher proportion of *** recurrence of PTB and the psychological support of COVID-19 with PTB after discharge require more attention.

The rumen is the hallmark organ of ruminants and hosts a diverse ecosystem of microorganisms that facilitates efficient digestion of plant *** analyzed 897 transcriptomes from three Cetartiodactyla lineages:ruminants,camels and cetaceans,as well as data from ruminant comparative genomics and functional assays to explore the genetic basis of rumen functional *** identified genes with relatively high expression in the rumen,of which many appeared to be recruited from other *** genes show functional enrichment in ketone body metabolism,regulation of microbial community,and epithelium absorption,which are the most prominent biological processes involved in rumen *** modes of genetic change underlying rumen functional innovations were uncovered,including coding mutations,genes newly evolved,and changes of regulatory *** validated that the key ketogenesis rate-limiting gene(HMGCS2)with five ruminant-specific mutations was under positive selection and exhibits higher synthesis activity than those of other *** newly evolved genes(LYZ1 and DEFB1)are resistant to Gram-positive bacteria and thereby may regulate microbial community ***,we confirmed that the changes of regulatory elements accounted for the majority of rumen gene *** results greatly improve our understanding of rumen evolution and organ evo-devo in general.

Malnutrition is a common comorbidity among patients with ***,no nutrition-screening tool has been recognized in this population.A quick and easy screening tool for nutrition with high sensitivity and easy-to-use is *** on the previous 25 nutrition-screening tools,the Delphi method was made by the members of the Chinese Society of Nutritional Oncology to choose the most useful item from each *** to these results,we built a nutrition-screening tool named age,intake,weight,and walking(AIWW).Malnutrition was defined based on the scored patient-generated subjective global assessment(PG-SGA).Concurrent validity was evaluated using the Kendall tau coefficient and kappa consistency between the malnutrition risks of AIWW,nutritional risk screening 2002(NRS-2002),and malnutrition screening tool(MST).Clinical benefit was calculated by the decision curve analysis(DCA),integrated discrimination improvement(IDI),and continuous net reclassification improvement(c NRI).A total of 11,360 patients(male,n=6,024(53.0%)were included in the final study cohort,and 6,363 patients had malnutrition based on *** on AIWW,NRS-2002,and MST,7,545,3,469,and1,840 patients were at risk of malnutrition,*** sensitivities of AIWW,NRS-2002,and MST risks were 0.910,0.531,and 0.285,and the specificities were 0.768,0.946,and *** Kendall tau coefficients of AIWW,NRS-2002,and MST risks were 0.588,0.501,and 0.326,*** area under the curve of AIWW,NRS-2002,and MST risks were0.785,0.739,and 0.630,*** IDI,c NRI,and DCA showed that AIWW is non-inferior to NRS-2002(IDI:0.002(-0.009,0.013),c NRI:-0.015(-0.049,0.020)).AIWW scores can also predict the survival of patients with *** missed diagnosis rates of AIWW,NRS-2002,and MST were 0.09%,49.0%,and 73.2%,*** showed a better nutritionscreening effect than NRS-2002 and MST for patients with cancer and could be recommended as an alternative nutritionscreening tool

目的提取小鼠的主要组织相容性复合物（MHC）基因各个位点,构建质粒,以用于转基因实验.方法提取小鼠RNA,逆转录cDNA,以嵌套聚合酶链反应（PCR）的方法扩增,将产物与T载体连接、克隆和测序;用内切酶切下,再与表达型载体连接,再次测序挑选克隆.结果嵌套PCR后,产物通过测序证实为目的基因序列,与T载体、表达型载体的连接和克隆后,经测序证实并挑选得到了正确的克隆.结论 MHC各位点基因可以通过嵌套PCR扩增获得;构建的表达型载体质粒可用于今后转入受者的实验.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in June 2012, and the data have been released to the public as the LAMOST Pilot Data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in June 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

The yangtze River is the mother river of *** promote the aquatic ecosystem protection of the great river,the Project of yangtze Fisheries Resources and Environment Investigation(2017-2021)supported by the Ministry of Agriculture and Rural Affairs,*** carried out by 24 institutes and universities that located in the yangtze River basin surveys the status of(1)fish species composition and spatial distribution,(2)current fish abundance,(3)endangered fishes,(4)yangtze finless porpoise,(5)aquatic eco-environments,(6)water-level fluctuation areas,(7)capture fisheries and recreational fisheries of the yangtze River mainstream and 10 of its main tributaries,including Yalong River,Heng River,Min River(including Dadu River),Chishui River,Tuo River,Jialing River,Wu River,Han River,Dongting Lake and Poyang *** results showed that there were 443 fish species(belonging to 163 genus,37 families,and 18 orders)before 2017,but only 323 fish species(including 15 new recorded exotic species)were recorded in the project of *** them,Cyprinus carpio,Carassius auratus,Hypophthalmichthys molitrix,Pelteobagrus fulvidraco,Coilia brachygnathus,Silurus asotus,Saurogobio dabryi,Ctenopharyngodon idellus,Pelteobagrus nitidus,Hemiculter leucisculus,Siniperca chuatsi,Coreius heterodon,Culter alburnus,Parabramis pekinensis,and Aristichthys nobilis were the dominant species across the whole yangtze River *** is estimated that there were 886 million individuals weighing 124.8 million kg,merely equivalent to 27.3%of the resources in 1950s,30.9%of the resources in 1960s,or 58.7%of the resources in *** the new list of protected fishes that recorded in the yangtze River system,only 15 of 29 were collected in this project of *** gladius has been affirmed to be Extinct by International Union for Conservation of Nature(IUCN).The wild individuals of Tenualosa reevesii and Luciobrama macrocephalus have disappeared for many years and maybe have been extinct
