To the Editor： A 73-year-old man presented to Department of Cardiology at Xinhua Hospital, Shanghai Jiaotong University School of Medicine with a history of anterior wall myocardial infarction （MI） and multimorphic ventricular tachycardia （VT） in approximately September 2013. His VT burden became progressive increase despite treatment with percutaneous coronary intervention （PCI） and multiple antiarrhythmic medications such as beta blockers, amiodarone, magnesium sulfate, and so on.

Pitting corrosion of 316L stainless steel in NaCl solution was investigated by means of staircase potential electrochemical impedance spectroscopy（SPEIS）.The investigation focused on the transition of stainless steel from the passive state to pitting *** on the evolution of electrical parameters of the equivalent electrical circuit,it is suggested that the most probable mechanism of pit creation is the film breaking *** result demonstrates that staircase potential electrochemical impedance spectroscopy is an effective method for the investigation of pitting corrosion.

Background Cholesterol-lowering therapy with statins has been reported to reduce the morbidity and mortality of cardiovascular diseases. This study aimed to investigate the effects of combined application of extended-release niacin and atorvastatin on lipid profile modification and the risks of adverse events in patients with coronary artery disease. Methods Consecutive 108 patients with coronary artery disease and serum total cholesterol （TC）〉 3.5 mmol/L were randomized into two groups： group A using atorvastatin and group B using extended-release niacin （niacin ER） and atorvastatin. Plasma lipid profile, glucose, and adverse events were assessed at the hospitalization, and 6 and 12 months after treatment. In addition, clinical cardiovascular events were evaluated after 12 months of treatment. Results The levels of TC, low density lipoprotein cholesterol （LDL-C） were significantly decreased （P 〈0.05） in groups A and B, but the levels of high density lipoprotein cholesterol （HDL-C） and ApoA increased by 29.36% and 40.81% respectively after 12 months of treatment in group B （P 〈0.01）. The medications were generally well tolerated in the two groups. No significant difference of adverse events was found between the two groups （group A： 3.2% vs group B 5.1%, P 〉0.05）. Conclusions Combined use of extended-release niacin with atorvastatin was superior to atorvastatin monotherapy alone in lipid profile regulation. Combination therapy with niacin ER and atorvastatin was well tolerated and safe in patients with coronary artery disease.

Background：A high ablation success rate for ventricular arrhythmia （VA） from outflow tract has been achieved,but some of them cannot be eliminated from *** investigated the association between adenosine sensitivity and ablation success/recurrence rates with a nonirrigated or an irrigated ***：According to adenosine test,all patients were divided into a sensitive group （S group） or an insensitive group （Ⅰ group）.The patients of each group were randomized into a nonirrigated catheter （NA） subgroup or an irrigated catheter （IA） subgroup with a 2∶1 ***：In S group of 122 patients （84 in NA subgroup）,the ablation success rate was similar between two subgroups （94.7% vs.90.5%,P 〉 0.05）,but in Ⅰ group of 94 patients （60 in NA subgroup）,it was higher in IA subgroup （94.1%） than that in NA subgroup （73.3%,P 〈 0.05）.The success rate using nonirrigated catheter was significantly higher in S group （90.5%） than that in Ⅰ group （73.3%,P 〈 0.01）,and the recurrence rate was lower in S group than that in Ⅰ group （1.3%,vs.13.6%,P 〈 0.05）.On the contrary,the success rate and the recurrence rate using irrigated catheter were similar between S group and 1 group （94.7%,94.1%,P 〉 0.05,vs.2.8%,6.3%,P 〉 0.05）.Conclusions：Adenosine insensitivity is associated with a lower success rate and a higher recurrence rate for VA patients undergoing nonirrigated catheter ***,irrigated catheters should be the first choice for VA ablation in adenosine insensitive patients.

Background Atrial tachycardia or flutter is common in patients after orthotopic heart transplantation. Radiofrequency catheter ablation to treat this arrhythmia has not been well defined in this setting. This study was conducted to assess the incidence of various symptomatic atrial arrhythmias and the efficacy and safety of radiofrequency catheter ablation in these patients. Methods Electrophysiological study and catheter ablation were performed in patients with symptomatic tachyarrhythmia. One Halo catheter with 20 poles was positioned around the tricuspid annulus of the donor right atrium, or positioned around the surgical anastomosis when it is necessary. Three quadripolar electrode catheters were inserted via the right or left femoral vein and positioned in the recipient atrium, the bundle of His position, the coronary sinus. Programmed atrial stimulation and burst pacing were performed to prove electrical conduction between the recipient and the donor atria and to induce atrial arrhythmias. Results Out of 55 consecutive heart transplantation patients, 6 males [（58±12） years] developed symptomatic tachycardias at a mean of （5±4） years after heart transplantation. Electrical propagation through the suture line between the recipient and the donor atrium was demonstrated during atrial flutter or during recipient atrium and donor atrium pacing in 2 patients. By mapping around the suture line, the earliest fragmented electrogram of donor atrium was assessed. This electrical connection was successfully ablated in the anterior lateral atrium in both patients. There was no electrical propagation through the suture line in the other 4 patients. Two had typical atrial flutter in the donor atrium which was successfully ablated by completing a linear ablation between the tricuspid annulus and the inferior vena cava. For accurately identifying the distribution charac-teristic of Gaussian-like noises in unmanned aerial vehicle(UAV)state estimation,this paper proposes a non-parametric scheme based on curve similarity *** the framework of the pro-posed scheme,a Parzen window(kernel density estimation,KDE)method on sliding window technology is applied for roughly esti-mating the sample probability density,a precise data probability density function(PDF)model is constructed with the least square method on K-fold cross validation,and the testing result based on evaluation method is obtained based on some data characteristic analyses of curve shape,abruptness and *** com-parison simulations with classical methods and UAV flight exper-iment shows that the proposed scheme has higher recognition accuracy than classical methods for some kinds of Gaussian-like data,which provides better reference for the design of Kalman filter(KF)in complex water environment.

Background： The CHA2DS2-VASc score is used clinically for stroke risk stratification in patients with atrial fibrillation （AF）. We sought to investigate whether the CHA2DS2-VASc score predicts stroke and death in Chinese patients with sick sinus syndrome （SSS） after pacemaker implantation and to evaluate whether the predictive power of the CHA2DS2-VASc score could be improved by combining it with left atrial diameter （LAD） and amino-terminal pro-brain natriuretic peptide （NT-proBNP）. Methods： A total of 481 consecutive patients with SSS who underwent pacemaker implantation from January 2004 to December 2014 in our department were included. The CHA2DS2-VASc scores were retrospectively calculated according to the hospital medical records before pacemaker implantation. The outcome data （stroke and death） were collected by pacemaker follow-up visits and telephonic follow-up until December 3 l, 2015. Results： During 2151 person-years of follow-up, 46 patients （9.6%） suffered stroke and 52 （10.8%） died. The CHA2DS2-VASc score showed a significant association with the development of stroke （hazard ratio [HR] 1.45, 95% confidence interval [CI] 1.20-1.75, P 〈 0.00 1） and death （HR 1.45, 95% CI 1.22-1.71, P 〈 0.001）. The combination of increased LAD and the CHA2DS2-VASc score improved the predictive power for stroke （C-stat 0.69, 95% CI 0.61-4）.77 vs. C-stat 0.66, 95% CI 0.57-0.74, P = 0.013）, and the combination of increased NT-proBNP and the CHA2DS2-VASc score improved the predictive power for death （C-stat 0.70, 95% CI 0.64-0.77 vs. C-stat 0.67, 95% CI 0.60--0.75, P= 0.023）. Conclusions： CHA2DS2-VASc score is valuable for predicting stroke and death risk in patients with SSS after pacemaker implantation. The addition of LAD and NT-proBNP to the CHA2DS2-VASc score improved its predictive power for stroke and death, respectively, in this patient cohort. Future prospective studies are warranted to validate the benefit of adding LAD and NT-proBNP to the CHA2DS2-VASc score for

A prospective study comparing endoscopic subcutaneous mastectomy plus immediate reconstruction with implants and breast conserving surgery for breast cancer

Background Breast conserving surgery （BCS） has been the standard surgical procedure for the treatment of early breast cancer. Endoscopic subcutaneous mastectomy （ESM） plus immediate reconstruction with implants is an emerging procedure. The objective of this prospective study was to evaluate the clinical outcomes of these two surgical procedures in our clinical setting. Methods From March 2004 to October 2007, 43 patients with breast cancer underwent ESM plus axillary lymph node dissection and immediate reconstruction with implants, while 54 patients underwent BCS. The clinical and pathological characteristics, surgical safety, and therapeutic effects were compared between the two groups. Results There were no significant differences in the age, clinical stage, histopathologic type of tumor, operative blood loss, postoperative drainage time, and postoperative complications between the two groups （P 〉0.05）. The postoperative complications were partial necrosis of the nipple and superficial skin flap in the ESM patients, and hydrops in the axilla and residual cavity in the BCS patients. There was no significant difference in the rate of satisfactory postoperative cosmetic outcomes between the ESM （88.4%, 38/43） and BCS （92.6%, 50/54） patients （P 〉0.05）. During follow-up of 6 months to 4 years, all patients treated with ESM were disease-free, but 3 patients who underwent BCS had metastasis or recurrence -- one of these patients died of multiple organ metastasis. Conclusions After considering the wide indications for use, high surgical safety, and favorable cosmetic outcomes, we conclude that ESM plus axillary lymph node dissection and immediate reconstruction with implants -- the new surgery of choice for breast cancer -- warrants serious consideration as the prospective next standard surgical procedure.
