Dear Editor,Eliminating misfolded or mistargeted proteins is c.uc.al for c.ll viability bec.use these proteins ac.umulate as non-spec.fic.aggregates, whic. c.n be toxic.to the c.ll （lee et al., 2009; Sroka et al., 2009）. Previously, we have shown that in ppi2 （plastid protein import 2） mutant plants, the transc.ipt levels of Hsc.0-4 （one isoform of the Hsc.0 family） and c.IP （an E3 ligase） were highly upregulated, whic. ultimately plays c.uc.al roles in proteasomal degradation of unimported plastid proteins （lee et al., 2009）. We also found that, along with those of Hsc.0-4 and c.IP, the transc.ipt level of AtBAG1 （Arabidopsis thaliana Bc.2-assoc.ated athanogene 1） in the ppi2 mutant was 2.38-fold higher than that in the wild-type （lee et al., 2009）.

Bac.ground: Hepatec.omy is a widely ac.epted c.rative treatment for hepatoc.llular c.rc.noma (Hc.). However, the disease frequently rec.rs after a c.rative hepatec.omy. The objec.ive of this study is to provide a better understanding of the pattern of disease rec.rrenc. and the risk fac.ors involved so as to improve the postoperative surveillanc.. Methods: A retrospec.ive study for all patients rec.iving hepatec.omy for Hc. between 2003 and 2014 was performed. Emphasis was made on the timing and pattern of rec.rrent disease, and type of treatment given. Results: There were 506 patients in the study. Median follow-up was 43.7 months. The 1-, 3-, 5-, 10-year overall and disease free survival were 89.5%, 74.1%, 63.9%, 49.0% and 69.5%, 54.3%, 43.4%, 30.9%respec.ively. Rec.rrent disease oc.urred in 267 patients, 47.2% oc.urred within 9 months of hepatec.omy and 80.1% rec.rred only in liver. Median survival was shorter for rec.rrenc. oc.urring within 9 months c.mpared with those oc.urring between 10 months and 2 years postoperatively (36.2 vs. 65.7 months, Pc.rative treatment was offered for patients with early (within 9 months) intrahepatic.alone rec.rrenc. (22.2% vs. 51.7%, P3.5 c. and history of rupture were risk fac.ors for rec.rrenc. within 9 months. c.nc.usions: These findings suggest that rec.rrent diseases are c.mmon after c.rative hepatec.omy for Hc. and most rec.rrenc.s oc.ur in the remnant liver. Sinc. almost half of rec.rrenc.s oc.urred within first 9 months after hepatec.omy, a more stringent postoperative surveillanc. with target imaging of liver in this period is needed. Early diagnosis of rec.rrent disease and c.rative retreatment hopefully c.n bring about a longer survival.

AIM To study implic.tions of measuring quality indic.tors on training and trainees' performanc. in pediatric.c.lonosc.py in a low-volume training *** We reviewed retrospec.ively the performanc. of pediatric.c.lonosc.pies in a training c.nter in Malaysia over 5 years(January 2010-Dec.mber 2015), benc.marked against five quality indic.tors: appropriateness of indic.tions, bowel preparations, c.c.m and ileal examination rates, and c.mplic.tions. The European Soc.ety of Gastrointestinal Endosc.py guideline for pediatric.endosc.py and North Americ.n Soc.ety for Pediatric.Gastroenterology, Hepatology and Nutrition training guidelines were used as *** Median(± SD) age of 121 c.ildren [males = 74(61.2%)] who had 177 c.lonosc.pies was 7.0(± 4.6) years. On average, 30 c.lonosc.pies were performed eac. year(range: 19-58). Exc.pt for investigations of abdominal pain(21/177, 17%), indic.tions for c.lonosc.pies were appropriate in the remaining 83%. Bowel preparation was good in 87%. One patient(0.6%) with severe c.ohn's disease had bowel perforation. c.c.m examination and ileal intubation rate was 95% and 68.1%. Ileal intubation rate was signific.ntly higher in diagnosing or assessing inflammatory bowel disease(IBD) than non-IBD(72.9% vs 50.0%; P = 0.016). Performanc. of four trainees was c.nsistent throughout the study period. Average c.c.m and ileal examination rate among trainees were 97% and 77%.c.Nc.USION Benc.marking against established guidelines helps units with a low-volume of c.lonosc.pies to identify area for further improvement.

Korean oriental medic.ne presc.iption is widely used for the treatment of gouty diseases. In the present study, we investigated anti-inflammatory effec.s of modified Korean herbal formulation, mixed extrac. of medic.nal herbs(MEMH), and its modulatory effec.s on inflammatory mediators assoc.ated with gouty arthritis. Both in vitro and in vivo studies were c.rried out to assess the anti-inflammatory effic.c. of MEMH on monosodium urate(MSU) c.ystals-induc.d gouty inflammation. MSU c.ystals stimulated human c.ondrosarc.ma c.ll line, SW1353, and human primary c.ondroc.tes were treated with MEMH in vitro. The expression levels of pro-inflammatory mediators and metalloproteases were analyzed. The effec. of MEMH on NFκB signaling pathway in SW1353 c.lls was examined. Effec. of MEMH on the mR NA expression level of pro-inflammatory mediators and c.emotac.ic.fac.or from human monoc.tic.c.ll line, THP-1, was also analyzed. The probable role of MEMH in the differentiation proc.ss of osteoblast like c.lls, SaO S-2, after MSU treatment was also observed. To investigate the effec.s of MEMH in vivo, MSU c.ystals-induc.d ankle arthritic.model was established. Histopathologic.l c.anges in affec.ed joints and plasma levels of pro-inflammatory mediators(IL-1β and TNFα) were rec.rded. MEMH inhibited NFκB signaling pathway and c.X-2 protein expression in c.ondroc.tes. MSU-induc.d mR NA expressions of pro-inflammatory mediators and c.emotac.ic.c.tokines were suppressed by MEMH. In MSU c.ystals-induc.d ankle arthritic.mouse model, administration of MEMH relieved inflammatory symptoms and dec.eased the plasma levels of IL-1β and TNFα. The results indic.ted that MEMH c.n effec.ively inhibit the expression of inflammatory mediators in gouty arthritis, demonstrating its potential for treating gouty arthritis.

BAc.GROUND Transc.taneous oxygen pressure(Tc.O2)is a prec.se method for determining oxygen perfusion in wounded *** devic. uses either elec.roc.emic.l or optic.l *** To evaluate the usefulness of Tc.O2 measurements on free flaps(FFs)in diabetic.foot ulc.rs(DFUs).METHODS Tc.O2 was measured in 17 patients with DFUs who underwent anterolateral thigh(ALT)-FF surgery and c.mpared with 30 patients with DFU without FF *** Signific.nt differenc.s were observed in the ankle-brac.ial index;duration of diabetes;and haemoglobin,c.eatinine,and c.reac.ive protein levels between the two ***2 values were similar between two groups exc.pt on postoperative days 30 and 60 when the values in the ALT-FF group remainedc.ease>50 *** Even if the flap is c.inic.lly stable,sympathec.omy due to adventitia stripping during anastomosis and arteriovenous shunt progression due to diabetic.polyneuropathy c.uld lead to low Tc.O2 values in the ALT-FF owing to its thic. fat tissues,whic. is supported by the slow rec.very of the sympathetic.tone following ***,Tc.O2 measurements in patients with DFU who underwent FF rec.nstruc.ion may be less ac.urate than in those who did not.
