Clinical and demographic features of patients undergoing videocapsule endoscopy management:A descriptive study

BACKGROUND Video-capsule endoscopy(VCE)is an efficient tool that has proven to be highly useful in approaching several gastrointestinal *** was implemented in Colombia in 2003,however current characterization of patients undergoing VCE in Colombia is limited,and mainly comes from two investigations conducted before the SARS-CoV-2 pandemic *** To describe the characteristics of patients undergoing VCEs and establish the main indications,findings,technical limitations,and other outstanding *** A descriptive study was carried out using data from reports of VCE(PillCam SB3 system)use in a Gastroenterology Unit in Bogotá,Colombia between September 2019 and January *** and clinical variables such as indication for the VCE,gastric and small bowel transit times(GTT,SBTT),endoscopic preparation quality,and limitations were described[n(%),median(IQR)].RESULTS A total of 133 VCE reports were *** were in men with a median age of 70 *** majority had good preparation(96.2%),and there were technical limitations in 15.8%of *** main indications were unexplained anemia(91%)or occult bleeding(23.3%).The median GTT and SBTT were 14 and 30 minutes,*** frequencies of bleeding stigma(3.79%)and active bleeding(9.09%)were low,and the most frequent abnormal findings were red spots(28.3%),erosions(17.6%),and vascular ectasias(12.5%).CONCLUSION VCE showed high-level *** main indication was unexplained *** bleeding was the most frequent *** with artificial intelligence,VCE can improve diagnostic precision and targeted therapeutic interventions.

Further Studies of the Cap Pushing Response in Honey Bees (Apis mellifera)

The Cap Pushing Response (CPR) is a free-flying technique used to study learning and memory in honey bees (Apis mellifera). The series of experiments outlined in this paper aimed to test whether honey bees exhibit the cognitive concept of “expectancy” utilizing the CPR in a weight differentiation paradigm. Five previous experiments in our laboratory have explored whether the concept of expectancy can account for honey bee performance and have all failed to support the cognitive interpretation. The first experiment examined if bees could differentiate between the two caps in the amount of force they used to push the cap and the distance the cap was pushed when the caps were presented one at a time. The second experiment explored cap weight preference by presenting bees with a choice between the two caps. The third and fourth experiments tested the bee’s ability to expect reward or punishment based on cap weight. Results revealed that bees were found to have a strong preference for the light cap and therefore were not able to expect reward or punishment based on cap weight. These experiments contribute to the debate on whether bees have “cognitive” representations and continue to support the behaviorist interpretation.

Later-Life Marital Dissolution in Italy:An Emerging Phenomenon?

The phenomenon of marital dissolution in later life,also referred to as“gray divorce”,is described on the rise in contemporary Western *** article contributes to the study of marriage breakdown in older age,with a specific focus on Italian ***,the temporal trends of this phenomenon are reconstructed using official *** data reveal that,although still relatively rare,the dissolution of marriage in later life is expanding in *** analysis of the period from 1974 to 2015 indicates a rise in the average age at separation,a higher percentage of spouses over 50 years on the total number of couples obtaining legal separation,and an increase in the proportion of Italian spouses separating after many years of ***,data from the national survey“Family and Social Subjects”,conducted in 2016 by the Italian National Institute of Statistics,are utilized to explore the relationship between specific traits of those who separated and their tendency to dissolve marriages before or after the age of *** data suggest that individuals with characteristics such as lower educational attainment,residence in the South and Islands,and adherence to more traditional family models are more likely to experience separation in later life.

Neuropsychological Profile of a Patient with Acquired Brain Damage Following Vascular Lesion of the Left Anterior Cingulate Cortex

Stroke is a physiological alteration associated with changes in blood flow that can result in sudden-onset cognitive impairment. It has a heterogenous clinical presentation with varying degrees of severity correlated with specific central nervous system zones or areas, and its prognosis is uncertain. This case study describes a 62-year-old male patient with acquired brain damage of the anterior cingulate cortex as a result of an ischemic event in the territory of the left anterior cerebral artery. Cognitive function was assessed using the neuropsychological executive function and frontal lobe test battery (BANFE-2) as well as other neuropsychological tests. The results show a profile of higher mental functions characterized by the presence of dysexecutive syndrome with marked behavioral alteration and diencephalic amnesia. .

Body Contour in the Post-Bariatric Male Patient

Background: Obesity is currently considered a public health problem. Bariatric procedures have become an important part of obesity management and, consequently, the number of male patients seeking post-bariatric reconstructive procedures have increased. Therefore, the clinical approach and understanding of the body contour of this population have become more relevant. The goal of post-bariatric reconstruction is to enhance the male silhouette through removal of skin and adipose tissue excess, and abdominal rectus diastasis repair. Material and Methods: We conducted a retrospective study at the Plastic and Reconstructive Surgery Department of the National Medical Center “20 de Noviembre”. All male patients referred to our department to start a post-bariatric reconstruction protocol from January 2018 to December 2022 were included in this study. Results: In total, 15 patients who underwent corporal contouring procedures were included;median age was 49.2 years with minimum of 33 years, and a maximum of 57 years. Median Body mass index was 28.4 kg/m2 with minimum of 22 kg/m2 and maximum of 38 kg/m2. All patients were treated 18 months after their bariatric surgery. All patients underwent an abdominoplasty as a body contouring procedure. 4 (26.7%) patients presented complications related to the surgery. Conclusion: We described a comprehensive and systematic approach to massive weight loss for male patients, suggesting an abdominal marking based on the patient’s clinical features and the expected results avoiding feminization of the abdominal body contour.

Piezo proteins are mechanically activated ion channels,which are required for mechanosensing functions in a variety of cell *** we and others have previously demonstrated that the expression of Piezo1 in osteoblast lineage cells is essential for boneanabolic processes,there was only suggestive evidence indicating a role of Piezo1 and/or Piezo2 in *** we addressed the question if and how chondrocyte expression of the mechanosensitive proteins Piezo1 or Piezo2 controls physiological endochondral ossification and pathological osteoarthritis(OA)*** with chondrocyte-specific inactivation of Piezo1(Piezo1^(Col2a1Cre)),but not of Piezo2,developed a near absence of trabecular bone below the chondrogenic growth plate ***,all Piezo1^(Col2a1Cre) animals displayed multiple fractures of rib bones at 7 days of age,which were located close to the growth *** skeletal growth was only mildly affected in these mice,OA pathologies were markedly less pronounced compared to littermate controls at 60 weeks of ***,when OA was induced by anterior cruciate ligament transection,only the chondrocyte inactivation of Piezo1,not of Piezo2,resulted in attenuated articular cartilage ***,osteophyte formation and maturation were also reduced in Piezo1^(Col2a1Cre) *** further observed increased Piezo1 protein abundance in cartilaginous zones of human ***,we identified Ptgs2 and Ccn2 as potentially relevant Piezo1 downstream genes in ***,our data do not only demonstrate that Piezo1 is a critical regulator of physiological and pathological endochondral ossification processes,but also suggest that Piezo1 antagonists may be established as a novel approach to limit osteophyte formation in OA.

Significant advances in surgical techniques and relevant medium-and long-term outcomes over the past two decades have led to a substantial expansion in the indications for major liver *** support these outstanding results and to reduce perioperative complications,anesthesiologists must address and master key perioperative issues(preoperative assessment,proactive intraoperative anesthesia strategies,and implementation of the Enhanced Recovery After Surgery approach).Intensive care unit monitoring immediately following liver surgery remains a subject of active and often unresolved *** postoperative complications,posthepatectomy liver failure(PHLF)occurs in different grades of severity(A-C)and frequency(9%-30%),and it is the main cause of 90-d postoperative ***,recently redefined with pragmatic clinical criteria and perioperative scores,can be predicted,prevented,or *** review highlights:(1)The systemic consequences of surgical manipulations anesthesiologistsmust respond to or prevent,to positively impact PHLF(a proactive approach);and(2)the maximal intensivetreatment of PHLF,including artificial options,mainly based,so far,on Acute Liver Failure treatment(s),to buytime waiting for the recovery of the native liver or,when appropriate and in very selected cases,toward *** a clinical context requires a strong commitment to surgeons,anesthesiologists,and intensivists towork together,for a fruitful collaboration in a mandatory clinical continuum.

Extravasated contrast volumetric assessment on computed tomography angiography in gastrointestinal bleeding:A useful predictor of positive angiographic findings

BACKGROUND Gastrointestinal bleeding(GIB)is a severe and potentially life-threatening condition,especially in cases of delayed *** tomography angiography(CTA)plays a pivotal role in the early identification of upper and lower GIB and in the prompt treatment of the *** To determine whether a volumetric estimation of the extravasated contrast at CTA in GIB may be a predictor of subsequent positive angiographic *** In this retrospective single-centre study,35 patients(22 men;median age 69 years;range 16-92 years)admitted to our institution for active GIB detected at CTA and further submitted to catheter angiography between January 2018 and February 2022 were ***-three(65.7%)patients underwent endoscopy before *** volumetry was evaluated in both arterial and venous phases via a semi-automated dedicated *** rate was obtained from volume change between the two phases and standardised for unit *** were divided into two groups,according to the angiographic signs and their concordance with *** Upper bleeding accounted for 42.9%and lower GIB for 57.1%.Mean haemoglobin value at the admission was 7.7 g/dL.A concordance between positive CTA and direct angiographic bleeding signs was found in 19(54.3%)*** no significant differences in terms of bleeding volume in the arterial phase(0.55 mL vs 0.33 mL,P=0.35),a statistically significant volume increase in the venous phase was identified in the group of patients with positive angiography(2.06 mL vs 0.9 mL,P=0.02).In the latter patient group,a significant increase in bleeding rate was also detected(2.18 mL/min vs 0.19 mL/min,P=0.02).CONCLUSION In GIB of any origin,extravasated contrast volumetric analysis at CTA could be a predictor of positive angiography and may help in avoiding further unnecessary procedures.

Environmental parameters assessment of a new diffuser for air cooling/heating system:Measurements and numerical validation

Air distribution of HVAC systems is the most popular type used in the building sector,having a relevant impact on indoor air quality and occupant *** types of research developed optimal solutions for the HVAC system’s design,focusing on specific components of the distribution system,on the airflow and geometry of ducts,on the size of ducts,on the shape and position of air ***,few works in literature proposed a globally experimental and simulation analysis of an air distribution system with a variable mass flaw *** this line,the presented research investigates the potentialities of a new ceiling diffuser,installed in an exhibition *** system provides a variable mass flow rate thanks to its configuration,providing adequate thermal comfort.A warm wall is chosen as the heating *** tests are carried out,six for cooling and two for heating with different volumetric air rates and supply air temperature of the *** combination of two methods,the measurement campaigns and the computational fluid dynamics(CFD)technique represent a suitable approach to examine the thermal indoor *** general,results show a strong capability of this diffuser to provide a uniform temperature and velocity field inside the ***,experimental and numerical data are significantly comparable with an average deviation of 1%for the velocity and lower 1%for the temperature,guaranteeing an optimal distribution of the understudy environmental parameters on the vertical and horizontal planes.
