A 64-year-old woman with IgA kappa multiple myeloma was treated with thalidomide-dexa- methasone. Due to progression of the disease, bortezomib, doxorubicin and dexamethasone were administered, followed by autologous stem cell transplantation. Although near-complete remission was achieved, 5 months later, neuro- logical symptoms appeared and the patient was diagnosed with multiple myeloma with cells in- filtrating the spinal cord. Bis-chloronitrosourea, bortezomib and lenalidomide were then admin- istered and although the patient remained neu- rologically asymptomatic, she died 3 months later becausee of disease progression. Lena- lidomide entered into the cerebrospinal fluid (confirmed by ultrasensitive high-performance liquid chromatography), although did not im- prove the poor prognosis of multiple myeloma involving the central nervous system.

Health Effects of Climate and Air Pollution in Buenos Aires: A First Time Series Analysis

Background: The impact of urban air pollution and temperature changes over health is a growing concern for epidemiologists all over the world and particularly for developing countries where fewer studies have been performed. Aim: The main goal of this paper is to analyze the short term effects of changes in temperature and atmospheric carbon monoxide on daily mortality in Buenos Aires, Argentina. Methods: We conducted a time series study focused on three age groups, gender, and cardiovascular and respiratory mortality, with lags up to four days and temporal variables as modifiers. Results: Temperature correlates positively with total mortality for summer months, with a RR = 1.0184 (95%, CI 1.0139, 1.0229) on the same day for each 1℃ increase. In winter this relationship reverses, as 1?C temperature increase exhibit a protective effect with a RR = 0.9894 (95%, CI 0.9864, 0.9924) at the 3 day lag. Carbon monoxide correlates always positively with mortality, with a RR = 1.0369 (95%, CI 1.0206, 1.0534) for each 1 ppm increase, on the previous day. Conclusions: Climate and pollution parameters measured in Buenos Aires City exhibit a correlation with health outcomes. The impacts of temperature and carbon monoxide vary with age and gender, being elderly the most susceptible subgroup. One day after an increase in CO of 1 ppm, about 4% extra deaths can be expected. The correlation found between increases in CO and mortality for greater lags may be ascribed to the role of CO as a chemical marker of urban air pollution, indicating the co-presence of other pollutants.

ng>Introduction:ng> Post-hospital syndrome (PHS) is defined as a period of vulnerability during the first 30 days after a patient is discharged from hospital, in which multiple factors come into play. Hyponatremia is the most frequent hydroelectrolytic disorder in hospitalized patients and may be related to the appearance of PHS. ng>Objective: ng>The objective is to estimate the prevalence of PHS that is assessed as the rate of readmissions in the first 30 days after discharge, in patients with hyponatremia. ng>Material and Methods:ng> It is a descriptive observational study of patients with hyponatremia who were discharged from 1 September 2010 to 2 February 2020 at the Internal Medicine Service of the Hospital University of San Juan (Alicante, Spain). ng>Results:ng> Of the 25 included patients, 5 (20%) were readmitted within a month of discharge, after a mean of 11.4 days (standard deviation [SD] 5.1). The overall mortality of the study was 20% (n = 5), with one case of death in the first 30 days post-hospitalization (4%). In 12 patients (48%) the origin of the hyponatremia was undetermined. The most frequently recorded etiology for the condition was pharmacological (n = 7, 28%), and there was pronounced variability in its clinical and laboratory study. The most widely used corrective measure was drug withdrawal, in 16 patients (64%). Water intake restriction was the most common treatment after discharge (5 patients, 20%), followed by urea (2 patients, 8%), while tolvaptan was not used. ng>Conclusion: ng>Hyponatremia may be the cause of PHS, which could increase the rate of early readmission. Hyponatremia is an underdiagnosed and undertreated entity, so it is necessary to apply an appropriate system to optimize its management and, in future studies, to assess its impact on PHS.

Roots of Zeyheriamontana, a species native to the savanna (Cerrado) region of central Brazil, produce lapachol, a naphthoquinone with anticancer activity. Lapachol is also the precursor of β-lapachone, a novel drug candidate for preventive and adjuvant cancer therapies. The leaves of Z. montana are a renewable source of ursolic acid and oleanoic acid, compounds known for their anticancer, antioxidant and antimicrobial properties. The potential prophylactic use of β-lapachone, as well as the medicinal properties of ursolic acid, highlights the importance of this study on ***’s germplasm conservation. Multiple shoots were induced on Woody Plant media with supplemented 0.1 mg·L-1 of thidiazuron (TDZ). Rooting was promoted on half strength WP (Woody Plant media containing 1.0 mg·L-1 of Indolbutiric acid-IBA). Plantlet acclimatization to ex-vitro condition was done at a 70% success rate using different substrates. It was possible to store ***’s elite germplasm using in vitro cultures of media containing 2% sucrose plus 4% sorbitol for six months without subcultures.

nutraceutic therapies such as berberine, bitter melon,Gymnema sylvestre, Irvingia gabonensis, resveratrol and ursolic acid have been shown to help control metabolic syndrome(MetS). The effect of berberine on glucose and lipid metabolism, hypertension, obesity and MetS has been evaluated in animal models and humans. Most clinical trials involving bitter melon have been conducted to evaluate its effect on glucose metabolism; nevertheless, some studies have reported favorable effects on lipids and blood pressure although there is little information about its effect on body weight. Gymnema sylvestre helps to decrease body weight and blood sugar levels; however, there is limited information on dyslipidemia and hypertension. Clinical trials of Irvingia gabonensis have shown important effects decreasing glucose and cholesterol concentrations as well decreasing body weight. Resveratrol acts through different mechanisms to decrease blood pressure, lipids, glucose and weight, showing its effects on the population with Met S. Finally, there is evidence of positive effects with ursolic acid in in vitro and in vivo studies on glucose and lipid metabolism and on body weight and visceral fat. Therefore, a review of the beneficial effects and limitations of the above-mentioned nutraceutic therapies is presented.
