During the prodromal stage of Alzheimer’s disease (AD), neurodegenerative changes can be identified by measuring volumetric loss in AD-prone brain regions on MRI. Cognitive assessments that are sensitive enough to measure the early brain-behavior manifestations of AD and that correlate with biomarkers of neurodegeneration are needed to identify and monitor individuals at risk for dementia. Weak sensitivity to early cognitive change has been a major limitation of traditional cognitive assessments. In this study, we focused on expanding our previous work by determining whether a digitized cognitive stress test, the Loewenstein-Acevedo Scales for Semantic Interference and Learning, Brief Computerized Version (LASSI-BC) could differentiate between Cognitively Unimpaired (CU) and amnestic Mild Cognitive Impairment (aMCI) groups. A second focus was to correlate LASSI-BC performance to volumetric reductions in AD-prone brain regions. Data was gathered from 111 older adults who were comprehensively evaluated and administered the LASSI-BC. Eighty-seven of these participants (51 CU;36 aMCI) underwent MR imaging. The volumes of 12 AD-prone brain regions were related to LASSI-BC and other memory tests correcting for False Discovery Rate (FDR). Results indicated that, even after adjusting for initial learning ability, the failure to recover from proactive semantic interference (frPSI) on the LASSI-BC differentiated between CU and aMCI groups. An optimal combination of frPSI and initial learning strength on the LASSI-BC yielded an area under the ROC curve of 0.876 (76.1% sensitivity, 82.7% specificity). Further, frPSI on the LASSI-BC was associated with volumetric reductions in the hippocampus, amygdala, inferior temporal lobes, precuneus, and posterior cingulate.

Purpose: To comparatively evaluate the per-formance of three intraoral image receptors of different technology when exposed to different X-ray beam spectra, dose and dose rate levels using a stepwedge phantom. Materials and methods: The intraoral radiographic receptors evaluated were: the Kodak Insight F speed class film, the Kodak RVG 6000, and the Duerr Vis-tascan Combi PSP system. A dental quality control phantom made of Plexiglas, containing an aluminium stepwedge with 12 steps and 7 holes drilled in each step was radiographed using a dental X-ray unit offering a wide range of tube potential, tube current and exposure time settings. The visibility of the holes in the images produced with each one of the three receptors was assessed by three independent observers. For each image the total image quality score (TS) was derived from the summation of the number of visible holes in each step. The numbers of perceptible holes in each experimental condition (TSs) were statistically analyzed through use of analysis of variance. Intraobserver and interobserver agree-ment was also measured. Results: Vistascan ex-hibited the most extended useful exposure range, followed by RVG 6000 and Insight. RVG 6000 exhibited the largest TS values in all tube potential settings except 70 kV where the Vistascan performed better. Insight performed better than Vistascan only at 60 and 63 kV. Vistascan performed better at 66 and 70 kV, Insight at 60 and 66 kV, whereas RVG performed equally well at all tube potential settings, except than at 52 and 70 kV. For the Insight the largest TS values were obtained with the smallest ESAK values whereas with the Vistascan the largest TS were obtained with ESAK values that where the largest observed. Conclusions: The performance of all receptors tested was greatly dependent on the exposure parameters and mainly on the kV settings. Congenital Nephrotic Syndrome Type 1 (Congenital Nephrotic Syndrome of the Finnish Type—CNF) is an autosomal recessive disorder encoding by nephrin gene mutation which is a (transmembrane protein 1-homolog—NPHS1) structural component of the slit diaphragm responsible for the proper functioning of the renal filtration barrier. In NPHS1 kidneys there is an effacement of the foot processes of the podocytes and impaired glomerular filtration barrier leading to antenatal manifestations and end-renal stage of disease after birth. We present a case of this disease where sonographic appearance of the fetal kidneys had alerted the experts for further genetic investigation for congenital nephrotic syndrome.

The aim of this review study is to elucidate the effects that phosphodiesterase 5 （PDE5） inhibitors exert on spermatozoa motility, capacitation process and on their ability to fertilize the oocyte. Second messenger systems such as the cAMP/adenylate cyclase （AC） system and the cGMP/guanylate cyclase （GC） system appear to regulate sperm functions. Increased levels of intracytosolic cAMP result in an enhancement of sperm motility and viability. The stimulation of GC by low doses of nitric oxide （NO） leads to an improvement or maintenance of sperm motility, whereas higher concentrations have an adverse effect on sperm parameters. Several in vivo and in vitro studies have been carried out in order to examine whether PDE5 inhibitors affect positively or negatively sperm parameters and sperm fertilizing capacity. The results of these studies are controversial. Some of these studies demonstrate no significant effects of PDE5 inhibitors on the motility, viability, and morphology of spermatozoa collected from men that have been treated with PDE5 inhibitors. On the other hand, several studies demonstrate a positive effect of PDE5 inhibitors on sperm motility both in vivo and in vitro. In vitro studies of sildenafil citrate demonstrate a stimulatory effect on sperm motility with an increase in intracellular cAMP suggesting an inhibitory action of sildenafil citrate on a PDE isoform other than the PDE5. On the other hand, tadalafil＇s actions appear to be associated with the inhibitory effect of this compound on PDE11. In vivo studies in men treated with vardenafil in a daily basis demonstrated a significantly larger total number of spermatozoa per ejaculate, quantitative sperm motility, and qualitative sperm motility; it has been suggested that vardenafil administration enhances the secretory function of the prostate and subsequently increases the qualitative and quantitative motility of spermatozoa. Antiretroviral naive and treated patients: Discrepancies of B cell subsets during the natural course of human immunodeficiency virus type 1 infection

AIM To evaluate alterations of memory B cell subpopulations during a 48-wk period in human immunodeficiency virus type 1(HIV-1) patients. METHODS Forty-one antiretroviral na?ve and 41 treated HIV-1 patients matched for age and duration of HIV infection were recruited. All clinical, epidemiological and laboratory data were recorded or measured. The different B cell subsets were characterized according to their surface markers: Total B cells(CD19^+), memory B cells(CD19^+CD27^+, BMCs), resting BMCs(CD19^+CD27^+CD21^(high), RM), exhausted BMCs(CD19^+CD21^(low)CD27-, EM), IgM memory B(CD19^+CD27^+IgM^(high)), isotype-switched BMCs(CD19^+CD27^+IgM-, ITS) and activated BMCs(CD19^+CD21^(low+)CD27^+, AM) at baseline on week 4 and week *** Mean counts of BMCs were higher in treated patients. There was a marginal upward trend of IgM memory B cell proportions which differed significantly in the treated group(overall trend, P = 0.004). ITS BMC increased over time significantly in all patients. Naive patients had of ^(low)er levels of EM B cells compared to treated, with a downward trend, irrespectively of ^(high)ly active antiretroviral therapy(HAART) intake. Severe impairment of EM B cells was recorded to both treated(P = 0.024) and naive(P = 0.023) and patients. Higher proportions of RM cells were noted in HAART group, which differed significantly on week 4^(th)(P = 0.017) and 48th(P = 0.03). Higher levels of AM were preserved in HAART naive group during the whole study period(week 4: P = 0.018 and 48: P = 0.035). HIV-RNA viremia strongly correlated with AM B cells(r = 0.54, P = 0.01) and moderately with RM cells(r =-0.45, P = 0.026) at *** HIV disrupts memory B cell subpopulations leading to impaired immunologic memory over time. BMC, RM, EM and ITS BMC were higher in patients under HAART. Activated BMCs(AM) were higher in patients without HAART. Viremia correlated with AM and RM. Significant depletion was recorded in EM B cells irrespectively of HAART

In this paper the author draws extensively upon her personal experience in witnessing storytelling in the oncological wing of the Laiko Hospital of Athens during her hospitalization in September-October 2004. More specifically, she concentrates on the presence of an inspired woman folk storyteller who, also a patient visited in the same period the hospital for personal treatment. Having the ability to tell a story skillfully, the specific storyteller ultimately tried with her narrations to ease the pain, both physical and spiritual, of patients that suffered from heavy or light forms of cancer, regardless of the fact that she herself was facing similar health problems. This essay sheds light on the persistence and function of tradition in our days and examines how illness and misfortune are ascribed and how the storyteller and listeners are connected. Her purpose is to find out what and how people learn and teach each other under special circumstances. This question has not been absent from the work of folklorists. The fieldwork was based on qualitative research, and the ethnographic method and collecting were used.
