Objective: To evaluate transperineal laser ablation (TPLA) with Echolaser® (Echolaser® TPLA, Elesta S.p.A., Calenzano, Italy) as a treatment for benign prostatic hyperplasia (BPH) and prostate cancer (PCa) using the Delphi consensus ***: Italian and international experts on BPH and PCa participated in a collaborative consensus project. During two rounds, they expressed their opinions on Echolaser® TPLA for the treatment of BPH and PCa answering online questionnaires on indications, methodology, and potential complications of this technology. Level of agreement or disagreement to reach consensus was set at 75%. If the consensus was not achieved, questions were modified after each round. A final round was performed during an online meeting, in which results were discussed and ***: Thirty-two out of forty invited experts participated and consensus was reached on all topics. Agreement was achieved on recommending Echolaser® TPLA as a treatment of BPH in patients with ample range of prostate volume, from 80 mL (80%), comorbidities (100%), antiplatelet or anticoagulant treatment (96%), indwelling catheter (77%), and strong will of preserving ejaculatory function (100%). Majority of respondents agreed that Echolaser® TPLA is a potential option for the treatment of localized PCa (78%) and recommended it for low-risk PCa (90%). During the final round, experts concluded that it can be used for intermediate-risk PCa and it should be proposed as an effective alternative to radical prostatectomy for patients with strong will of avoiding urinary incontinence and sexual dysfunction. Almost all participants agreed that the transperineal approach of this organ-sparing technique is safer than transrectal and transurethral approaches typical of other techniques (97% of agreement among experts). Regional neuromuscular regulation within rectus femoris muscle following three-month limb-loaded walking in older adults

Limb-loaded walking using ankle weights has been widely applied to increase exercise intensity in older *** changes in the activation pattern between proximal(RFP)and distal(RFD)regions of the rectus femoris(RF)muscle is key to clarifying gait deficits in older *** aim of this study was to determine regional neuromuscular regulation within the RF muscle following three-month limb-loaded walking in older *** study participants were 22 healthy older adults(696.3 years)who walked at least 160 min per *** electromyography(EMG)and motion capture were used to measure the neuromuscular activities of RFP and RFD and generate kinematic data on the left lower extremity on walking for 240 s at the preferred gait speed on a treadmill at pre-and post-intervention,*** rectified values(ARV)of RFP and RFD were normalized by maximum values of ARV during a gait cycle within ten consecutive gait *** ARV of RFP was greater than RFD at 30%–40%and 70%–90%of the gait cycle and hip joint flexion at 0%–100%,and the walking speed and swing timing at post-were greater than at pre-intervention(p0.05).The activity of RFP compared with RFD and hip joint flexion were increased following limb-loaded walking intervention in older adults.

目的评价naldemedine单、多次给药在中国健康成人受试者中的安全性、耐受性和药代动力学特征。方法10例中国健康受试者单、多次口服naldemedine后,用液相色谱质谱联用仪检测血浆中naldemedine以及代谢物(nor-naldemedine)的浓度,经Phoenix WinNonlin 8.3.1软件计算药代动力学参数。结果相比于单次给药,多次给药后naldemedine的tmax(单次为1.06 h,多次为1.08 h)、C_(max)(单次为4.16 ng·mL^(-1),多次为4.11 ng·mL^(-1))均无明显变化,AUC0-τ高于单次给药(单次为23.88 ng·h·mL^(-1),多次为28.93 ng·h·mL^(-1)),t_(1/2)稍有延长(单次为9.26 h,多次为12.50 h),多次给药后有轻微蓄积(C_(max)的蓄积比为0.99,AUC的蓄积比为1.22);nor-naldemedine的t_(1/2)(单次为18.10 h,多次为29.30 h)、C_(max)(单次为0.19 ng·mL^(-1),多次为0.38 ng·mL^(-1))和AUC0-τ(单次为3.13 ng·h·mL^(-1),多次为6.02 ng·h·mL^(-1))均明显升高,tmax(单次为3.49 h,多次为3.56 h)无明显变化。代谢物与naldemedine的C_(max)比值和AUC比值明显升高,多次给药后存在蓄积(C_(max)的蓄积比为2.01,AUC的蓄积比为1.95)。所有治疗后出现的不良事件均为轻度。结论中国健康受试者连续多次(第1天单次、第4~13天每天1次)口服naldemedine片0.2 mg后,在体内消除较快,有轻度蓄积,在多次给药阶段第2次给药前达到稳态,且有良好的安全性和耐受性。

Atmospheric correction for HY-1C CZI images using neural network in western Pacific region

Since the anti-inflammatory effect of hydrogen has been widely known,it was supposed that hydrogen could suppress tissue damage by inhibiting virus-related inflammatory ***,hydrogen is slightly soluble in water,which leads to poor effect of oral hydrogen-rich water *** this study,the nano-bubble hydrogen water(nano-HW)(about 0.7 ppm)was prepared and its therapeutic effect against viral infection was investigated by utilizing spring viraemia of carp virus(SVCV)-infected zebrafish as ***-month-old zebrafish were divided into nano-HW treatment-treated group and aquaculture water treated group(control group).The results revealed that the cumulative mortality rate of SVCV-infected zebrafish was reduced by 40%after treatment with nano-bubble hydrogen water,and q RT-PCR results showed that SVCV replication was significantly *** examination staining showed that SVCV infection caused tissue damage was greatly alleviated after treatment with nano-bubble hydrogen ***,SVCV infection caused reactive oxygen species(ROS)accumulation was significantly reduced upon nano-HW *** level of proinflammatory cytokines IL-1β,IL-8,and TNF-αwas remarkably reduced in the nano-HW-treated group in vivo and in *** together,our data demonstrated for the first time that nano-HW could inhibit the inflammatory response caused by viral infection in zebrafish,which suggests that nano-HW can be applied to antiviral research,and provides a novel therapeutic strategy for virus-caused inflammation related disease.

Prognostic Impact of Surgical Margin Width in Hepatectomy for Colorectal Liver Metastasis

As for resection for colorectal liver metastasis (CRLM), secur-ing an adequate surgical margin is important for achieving a better prognosis. However, it is often difficult to achieve ade-quate margins for the resection of CRLM. So the current sur-vival impact of sub-centi/millimeter surgical margins in he-patectomy for CRLM should be evaluated. In the current era of multidisciplinary treatment options, this review focused on the prognostic impact of a sub-centi/millimeter surgical margin width in hepatectomy for CRLM. We systematically reviewed retrospective studies that clearly described the sur-gical margin width for hepatectomy for CRLM. We selected studies conducted since 2000 that involved patients diag-nosed as having CRLM. We focused on studies that investi-gated not only surgical margins, but also microscopic surgical curability such as R0 (microscopically complete resection) or R1 (microscopically incomplete resection), which clearly de-scribe their definitions. Based on our literature review, 1, 2, or 5 mm was considered the minimum surgical margin width for hepatectomy for CRLM. Although a surgical margin width of 1 mm is acceptable for hepatectomy for CRLM, submil-limeter margins, which are defined as R1 in many reports, are only acceptable for limited patients such as those who have undergone preoperative chemotherapy. Zero-mm mar-gins are also acceptable in limited patients such as those who show a good response to preoperative chemotherapy. New chemotherapy agents have been reported to reduce the prognostic impact of a narrow surgical margin width. The incidence of margin recurrence, which is a major concern regarding R1 resection of CRLM, is about 20-30% according to the majority of earlier reports. As evaluations of the actual prognostic impact of the surgical margin remain difficult, fur-ther study is warranted.
