Surgery for Pericardial Syndromes in Adults and Children: Clarification of Questionable Aspects

Background: The knowledge on pericardial disease has increased but the European Society of Cardiology in the last guidelines 2015 stated a section of perspective and unmet needs referring to the surgical management as one of these needs. Here, we present an institutional experience to contribute with other studies in explanation of questionable aspects about their surgical management. Methods: Among 127 cases (93 adults and 34 children) that were diagnosed as pericardial syndrome, we retrospectively analyzed 45 cases (40 adults and 5 children) operated for pericardial syndrome from May 2012 to June 2019. Echocardiogram was the main preoperative diagnostic tool. Surgical approach was selected according to each diagnosis. Postoperative clinical assessment, recurrence and mortality rate were the main determinants of outcome. Results: Regarding pericardial effusions, the mean preoperative medical treatment period was 17.7 ± 21.9 days and pericardial window through thoracotomy was the common approach (54.5%). In constrictive pericarditis, infection was the main etiology (40%), mean preoperative medical treatment period was 16 ± 8.8 days and complete pericardiectomy was the surgical procedure for most cases. Trans-sternal drainage was the standard approach for cardiac tamponade. Comparison of efficiencies and costs of different weed control methods in paddy production in Iran

Using of efficient and inexpensive method for weed control has an important role in increasing efficiency and reducing costs of *** this study,field experiments were carried out to evaluate the efficiency and costs of several weed control methods in paddy production during summer in Mazandaran,the north province of Iran in *** weed control treatments(T1 to T7)were applied at three stages of rice growth(20 and 35 days after transplanting and grain harvesting time).Weed density,weed dry weight,labor used and economical comparison were determined in the basis of a complete randomized block design with three *** showed that there were no significant differences among efficiencies of six treatments(except for weedy check method(T6)treatment).The minimum weed density and dry weight were obtained at herbicide+hand weeding once(T5)and herbicide application once(T7)treatments during all three *** costs of weed control were different among the *** of control method has an important role in reducing *** weeding twice method(T1),because of its high weeding cost,hard work and worker restriction in the required time,is not *** weeding twice method(T2),due to its zero environment pollution,reducing costs of weed control and high benefit/cost,is the best and most appropriate *** of weed damage in weedy check method(T6)varied between 30.5%and 45.3%.

Bathymetric and Geochemical Analysis of Lake Al-Saad, Abha, Kingdom of Saudi Arabia Using Geoinformatics Technology

This study investigates the potential for remote sensing of lake water bathymetry and geochemical by 1) examining the empirical based technique for retrieving depth information from passive optical image worldview-2 satellite data, 2) performing atmospheric correction, 3) assessing the accuracy of spectrally based depth retrieval under field condition via field measurement, 4) producing bathometry and geochemistry mapping by examining spectral variations for identifying pairs of wavelengths that produce strong linear correlation coefficient between the band ratio. The results indicate that optical remote sensing of bathymetry and geochemical investigation is not only feasible but more accurate under conditions of typical lake water, supporting field survey. The Pearson correlation matrix (R) between the examined water samples/depth and the TOA reflectance values of the worldview-2 (WV-2) satellite data have been investigated and found good correlation. The models developed using the combination of different band pairs also show high accuracy. Cartographical maps were generated depending on the linear correlation coefficient between the measured parameters and the TOA reflectance values of the worldview-2 data. The investigation shows that dissolved oxygen (DO) of the lake water is slight lower than the permissible limit of Saudi standards for lake water. The shallow water has high DO concentration, whereas the deeper shows significantly lower down. Electrical conductivity measurements serve as a useful indicator of the degree of mineralization in the water sample. All the samples which have EC exceed limit. The spatial distribution of EC and TDS inferred that the EC and TDS concentration is the highest at the eastern part of the lake whereas concentration drops down towards the southern side. This study confirms that remote sensing incorporated with GIS and GPS could afford an integrated scheme for mapping water quality and bathometry of the surface water.

3D-Analysis of Soil-Foundation-Structure Interaction in Layered Soil

The analysis of building structure in contact with soil involves an interactive process of stresses and strains developed within the structure and the soil field. The response of Piled-Raft Foundation system to the structure is very challenging because there is an important interplay between the component of building structure and the soil field. Herein, soil-foundation-structure interaction of buildings founded on Piled-Raft Foundation is evaluated through 3D-Nonlinear Finite Element Analyses using PLAXIS3D FOUNDATION code. The soil settlements and forces demand of the high-rise building structures and foundation is computed. The parametric study affecting the soil-foundation-structure response has been carried out. The parameters such as construction phasing, sequential loading, building aspect ratios, soil failure models and thickness proportion of soil field stiff layer, are considered. It is concluded that the interaction of building foundation-soil field and super-structure has remarkable effect on the structure.

Administration of Zinc with Paroxetine Improved the Forced Swim Test Behavioral Pattern of Treated Mice in Acute and Sub-Acute Study

Despite progressive improvement in treating major depressive disorder (MDD), it remains mostly unresponsive to one antidepressant medication. Zinc is a brain highly abundant trace metal, a brain derived neurotrophic factor (BDNF) inducer, a modulator of synaptic plasticity and potent suppressor of the NMDA receptors. We proposed that co-administration of zinc with the antide-pressants may represent a valuable regimen to improve the efficacy of these drugs. This work has been implemented to evaluate the behavioral changes of acute and sub-acute co-administration of zinc with Paroxtine in mice. Methods: The animals were injected intra-peritoneal with either Paroxtine (20 mg/kg) which was a selective serotonin reuptake inhibitor (SSRI), zinc sulfate (30 mg/kg) or Paroxtine in combination with zinc for one day and one week (once daily). The pattern of the animal behavior was assessed in the forced swim test (FST). Results and Discussion: The behavioral patterns of the animals in the FST include immobility, swimming and climbing. Successful antidepressant should decrease the immobility time with either increase in swimming and/or climbing behavior based on the drug pharmacological activity. Our results revealed a significant decrease of immobility and increase of swimming behavior indicating serotonin-dependent pharmacological activity of Paroxtine or zinc alone as well as in the animals treated with zinc in combination with Paroxtine. There was no significant difference in the animals’ behavior between acute and sub-acute treatment with zinc or even upon its addition to paroxetine. Our data support the concept that co-administration of zinc may provide further antidepressant activity. Zinc may offer additional clinical value particularly in geriatric patients or other populations where zinc level has shown dramatic decrease.

Double diabetes in Saudi Arabia: A new entity or an underestimated condition

AIM To determine the clinical and biological characteristics of double diabetes(DD) among young people in Saudi *** This was a retrospective descriptive chart review study including 312 young newly diagnosed diabetic patients(aged 12-20 years), whom were admitted over a five year period(January 2009 to December 2013). Family history of diabetes mellitus(DM)(first degree), physical body mass index(BMI), acanthosis nigricans, history of auto-immune disease and laboratory information for glycosylated hemoglobin, basal C peptide level and diabetes autoantibody response(anti-GAD, anti-IA2 and anti-ICA) were collected from medical report. A mean follow-up of 3 years for these patients was *** Patients were categorized into 4 groups, based on the autoantibody response(Ab+ or Ab-) and C-peptide secretion(β+ for fasting level 0.4-2.1 ng/mL and β- if < 0.4 ng/mL). Group1(type 1a): Ab+ β-(21%), group 2(type 1b): Ab- β-(9%), group 3(DD): Ab+ β+(31%)and group 4(classic type 2 DM): Ab- β+(39%). The mean age of the DD patients in our study was 15.1 ± 6.4 years. A total of 41% of the study population presented with diabetic ketoacidosis and 61% of the study population presented with positive family history of DM. The mean BMI was 26.8 kg/m2 with 64% of overweight or obese patients. Ninety two percent of the patients were started on insulin at the time of diagnosis. During a mean follow-up of 3 years, only 32% of the patients with DD required insulin and 78% were on metformin alone or with insulin. CONCLUSION Our findings enable us to arrive at the conclusion that almost one-third of the young Saudi diabetic patients reveal atypical forms of DM(double diabetes) expressing features resulting from both T1D and T2D.

Comparative Analysis of High-Rise Reinforced Concrete Structures According to International Seismic Design Codes

The field of earthquake engineering and seismology is of great importance to structural engineers around the world. The location, size and consequences of an earthquake are variable depending on several conditions. Surface conditions, boundary/fault type, distance from the boundary and hypocenter are all elements that dictate the outcome of a seismic event. The paper presents a comparison of seismic provisions of two seismic design codes—-EC8 (Eurocode 8) and IBC (International Building Code) 2006, to a high-rise reinforced concrete building. The building is irregular and composes of 20 floors. The equivalent lateral force analysis was performed using the well-known structure program—-ETABS (Extended 3D Analysis of Building Systems). Based on the analysis results (inter-storey index, global damage index, storey displacement, inter-storey drift ratio and base shear), EC8 was found to be conservative when compared with IBC 2006. The conclusion is that for the design and analysis of high-rise reinforced concrete buildings with certain irregularity, EC8 provisions were considered to be conservative.
