Enteric neuropathology of congenital intestinal obstruction:A case report

Experimental evidence indicates that chronic mechanical sub-occlusion of the intestine may damage the enteric nervous system （ENS）, although data in humans are lacking. We here describe the first case of enteric degenerative neuropathy related to a congenital obstruction of the gut. A 3-year and 9-mo old girl began to complain of vomiting, abdominal distension, constipation with air-fluid levels at plane abdominal radiology. Her subsequent medical history was characterized by 3 operations： the first showed dilated duodeno-jejunal loops in the absence of occlusive lesions; the second （2 years later） was performed to obtain full-thickness biopsies of the dilated intestinal loops and revealed hyperganglionosis at histopathology; the third （9 years after the hyperganglionosis was identified） disclosed a Ladd＇s band which was removed and the associated gut malrotation was corrected. Repeated intraoperative full-thickness biopsies showed enteric degenerative neuropathy along with reduced interstitial cells of Cajal network in dilated loops above the obstruction and a normal neuromuscular layer below the Ladd＇s band. One year after the latest surgery the patient tolerated oral feeding and did well, suggesting that congenital （partial） mechanical obstruction of the small bowel in humans can evoke progressive adaptive changes of the ENS which are similar to those found in animal models of intestinal mechanical occlusion. Such ENS changes mimic neuronal abnormalities observed in intestinal pseudoobstruction.

Correlative light-electron microscopy(CLEm)requires the availability of robust probes which are visible both in light and electron *** we demonstrate a CLEm approach using small gold nanoparticles as a single *** gold nanoparticles bound to the epidermal growth factor protein were located with nanometric precision background-free in human cancer cells by light microscopy using resonant four-wave mixing(FWm),and were correlatively mapped with high accuracy to the corresponding transmission electron microscopy *** used nanoparticles of 10 nm and 5 nm radius,and show a correlation accuracy below 60 nm over an area larger than 10 pm size,without the need for additional fiducial *** accuracy was improved to below 40 nm by reducing systematic errors,while the localisation precision is below 10 ***-resolved FWm correlates with nanoparticle shapes,promising for multiplexing by shape recognition in future *** to the photostability of gold nanoparticles and the applicability of FWm microscopy to living cells,FWm-CLEm opens up a powerful alternative to fluorescence-based methods.

The increased use of mechanical circulatory support has led to a rise in infectious complications in the recent years predisposing this population of patients to higher morbidity and mortality. In this study, we report a significantly lower rate of driveline infections of 0.12 episodes/patient-year as compared to the rates reported in the existing literature in patients with the HmII (Heartmate II). The study is limited by the fact that it is based on a small population of patients and was conducted retrospectively making recall bias hard to rule out. It is strictly restricted to one type of continuous flow pump (HmII) solely to decrease variations in the data reviewed. The exact cause of the low infection rate noted in this study is difficult to define. However, detailed teaching to the caregivers and the patient as well as close follow-ups in the perioperative period may substantially contribute to the outcome noted. Continued research limited to similar continuous flow ventricular assist devices in larger study populations would shed light on defining causes of infections in this population and developing robust algorithms to prevent such complications.

The wall shear stress （WSS） that a moving fluid exerts on a surface affects many processes including those relating to vascular function. WSS plays an important role in normal physiology （e.g. angiogenesis） and affects the microvasculature＇s primary function of molecular transport. Points of fluctuating WSS show abnormalities in a number of diseases; however, there is no established technique for measuring WSS directly in physiological systems. All current methods rely on estimates obtained from measured velocity gradients in bulk flow data. In this work, we report a nanosensor that can directly measure WSS in microfluidic chambers with sub-micron spatial resolution by using a specific type of virus, the bacteriophage m13, which has been fluorescently labeled and anchored to a surface. It is demonstrated that the nanosensor can be calibrated and adapted for biological tissue, revealing WSS in micro-domains of cells that cannot be calculated accurately from bulk flow measurements. This method lends itself to a platform applicable to many applications in biology and microfluidics.

Neuromyelitis optica is a clinical syndrome characterised by acute transverse myelitis plus an acute or subacute optic neuritis with or without recovery. Alt hough once believed to be a variant of multiple sclerosis, diagnostic criteria h ave recently been proposed for neuromyelitis optica, making it a clinically dist inct syndrome. The term gluten sensitivity refers to a state of heightened immun ological responsiveness to ingested gluten in genetically susceptible individual s, as indicated by circulating antibodies to gliadin. Several neurological compl ications have been described associated with gluten sensitivity ranging from per ipheral neuropathy and cerebellar ataxia to an increased risk of epilepsy. Altho ugh myelopathy has been described in some case reports of coeliac disease, neuro myelitis optica has never been described in association with gluten sensitivity. We describe two cases of gluten sensitivity presenting as neuromyelitis optica with no previous history of significant gastrointestinal symptoms. Gluten sensit ivity was confirmed by immunological and histological studies.
