In live donor liver transplantation, anatomical anomalies of the portal vein are more frequently encountered in right lobe than left lobe grafts. Of these, a dual portal vein is one of the most common anatomical anomalies encountered. We hereby report our method of using a recipient portal vein patch after venoplasty for reconstruction in a right lobe graft with separate anterior and posterior portal vein branches.

Unequal Distribution of Innovation Efforts for Neglected Tropical Diseases: The Role of Funding Evaluation Criteria

Background: International research and innovation efforts for neglected tropical diseases have increased in recent decades due to disparities in overall health research funding in relation to global burden of disease. however, within the field of neglected tropical diseases some seem far more neglected than others. In this research the aim is to investigate the distribution of resources and efforts, as well as the mechanisms that underpin funding allocation for neglected tropical diseases. Methodology: A systematic literature review was conducted to establish a comprehensive overview of known indicators for innovation efforts related to a wide range of neglected tropical diseases. Articles were selected based on a subjective evaluation of their relevance, the presence of original data, and the breadth of their scope. This was followed by thirteen in-depth open-ended interviews with representatives of private, public and philanthropic funding organizations, concerning evaluation criteria for funding research proposals. Results: The findings reveal a large difference in the extent to which the individual diseases are neglected with notable differences between absolute and relative efforts. Criteria used in the evaluation of research proposals relate to potential impact, the probability of success and strategic fit. Private organizations prioritize strategic fit and economic impact;philanthropic organizations prioritize short-term societal impact;and public generally prioritize the probability of success by accounting for follow-up funding and involvement of industry. Funding decisions of different types of organizations are highly interrelated. Conclusions: This study shows that the evaluation of funding proposals introduces and retains unequal funding distribution, reinforcing the relative neglect of diseases. societal impact is the primary rationale for funding but application of it as a funding criterion is associated with significant challenges. The Dnieper River headwaters are in Russia’s Valdai hills and the river flows south to the Black sea. The Dnieper River provides a waterway in which to transport goods to and from various European nations. In addition, the dams on the river provide hydro power. There are approximately 2260 km of Dnieper waterways in Russia, in Belarus, and within Ukraine. The Dnieper River has numerous urban centers including smolensk in Russia, Mogilev in Belarus and Kiev and Zaporizhzhya in Ukraine. The worst nuclear accident in history unfolded, in the Dnieper River watershed, in northern Ukraine as a reactor at the Chernobyl nuclear power plant exploded and burned. After an accident, such as Chernobyl, radionuclide contaminated bodies of water via direct deposition from the air, discharge as effluent or indirectly from catchment basin washout. When radionuclides contaminate large bodies of water, they are quickly dispersing and accumulate in water bottom sediments, benthos, aquatic plants, and bottom feeding fish. The main pathways to humans are through contamination of drinking-water, from use of water for irrigation of food crops, and consumption of contaminated fish. Kakhovka Dam on the Dnieper River was destroyed during the Russian-Ukraine conflict and the dam needs to rebuild as soon as possible. Perhaps lessons learned by the Us Army Corps of Engineers (UsACE), after using TNT to blow up the Birds Point front line levee on the Mississippi River in May of 2011, can be applied to the man-induced 2023 Kakhovka Dam breach. The Birds Point man-induced levee breaches and subsequent flooding of farmland resulted in the loss of the 2011 crops and damaged the future soil productivity. The strong current and sweep of the water through the three man-induced levee breaches on the New Madrid floodway levee created deep gullies, displaced tons of soil, and damaged irrigation equipment, farms, and homes. Non-communicable diseases (NCDs) are a significant global health challenge, contributing to 50% of worldwide morbidity and 63% of mortality. The burden is particularly substantial in low—and middle-income countries (LMICs), where 80% of NCD-related deaths occur. A quasi-experimental study addressed this challenge from May 2022 to March 2023. This study utilized a non-equivalent pre-and post-test design, with 300 participants in the quantitative and 70 in the qualitative. The study employed multistage cluster and random sampling to select ten community units, resulting in 150 community health volunteers (ChVs) in the control unit and 150 in the intervention group. Data collection was facilitated through the KOBO app. Qualitative data analysis involved six homogeneous focus group discussions (FGDs) and ten key informant interviews (KIIs), audio-recorded, transcribed, and analyzed using N-Vivo 12. Despite efforts to implement screening programs and improve linkages to care, significant barriers persist. This article reviews these barriers, drawing on current literature and empirical evidence. Key obstacles identified include limited awareness, inadequate healthcare infrastructure, cultural beliefs, financial constraints, fragmented healthcare systems, and challenges linking individuals to appropriate care services. The article explores strategies to overcome these barriers, emphasizing the importance of collaborative approaches involving stakeholders at various levels. Addressing these challenges aims to strengthen NCD screening and linkages to care, ultimately improving health outcomes for populations globally. several recommendations emerge from the study’s findings and literature review.
