BACKGROUNd magnetically controlled growing rods(mCGR) are a novel treatment option for early onset scoliosis(EOS). Although the complication profile with mCGR use has been reviewed, these reviews do not take into account important implants modifications, termed iterations, that were made due to early on postoperative complications is not well reported or *** To assess the effect of mCGR implant iterations on post-operative complications in *** A systematic review was performed to identify studies investigating mCGR specifically for the treatment of EOS, refined to those reporting the implant iteration, specifically the incorporation of the keeper plate to the implant *** with mixed implant iteration usage were excluded. Complications following surgery were recorded as well as potential risk factors and compared between implant *** Although 20 articles were identified for inclusion, 5 included mixed implant iteration leaving a total of 271 patients identified through 15 clinical studies thatmet inclusion criteria. The average follow-up was 25.4-mo. Pre-keeper plate implants were utilized in 3 studies with a total of 49 patients. Overall, 115(42.4%)post-operative complications were identified, with 87% defined as major. The addition of the keeper plate significantly decreased the rate of post-operative complications per study(35.7% vs 80.6%, P = 0.036), and the rate of distraction failure(8.1% vs 40.8%, P = 0.02). Unplanned reoperation occurred in 69(26.7%)patients but was not different between implant iteration cohorts(25.5% without keeper plate vs 27.1% with keeper plate, P = 0.92).CONCLUSION mCGR for EOS has a cumulative complication rate of 42.4% but this is significantly reduced to 35.7% when reviewing only keeper-plate enabled implants. However, 25% of published articles included mixed implant *** studies should discern between implants iterations when reporting on the usage of mCGR for EOS.

Human milk is generally considered to be insufficient in vitamin d. However, research completed in the 1980s showed that the vitamin d content of human milk is directly related to maternal serum vitamin d levels and therefore may potentially be adequate if the mother’s vitamin d levels are sufficient. Approximately one-third of the adult population, which includes breastfeeding women, in the United States have vitamin d insufficiency or deficiency. Among infants, 90.4% of breastfed infants are vitamin d deficient compared to 15.4% of formula fed infants. The American Academy of Pediatrics (AAP) has therefore recommended all breastfed infants be directly supplemented with 400 IU per day of vitamin d to decrease the risk of vitamin d insufficiency and rickets. According to the AAP, compliance rates with this recommendation ranges between 2% - 36%. Because the recommendation to supplement may undermine breastfeeding, many pediatricians do not inform their patients of the need to supplement. Additionally, some parents are concerned about directly supplementing their infant as risks may include allergic reactions to the ingredients, aspiration pneumonia, accidental overdose, and changes in intestinal flora and pH which may compromise the immune benefits of human milk. A literature review was conducted to examine the effect of maternal supplementation with vitamin d during lactation on human milk vitamin d content and maternal and infant serum vitamin d levels. Although there is no current consensus regarding dosage and timing of maternal vitamin d supplementation, the literature suggests that high-dose vitamin d supplementation of the lactating mother is as effective at maintaining infant vitamin d levels as direct infant supplementation, while also correcting the mother’s vitamin d deficiency.

Sand wave migration near the southeastern corner of martha's Vineyard, massachusetts, USA

Sand waves of approximately 2 m in height were observed to migrate nearly 40 m with counterclockwise rotation between two bathymetric surveys performed three months apart near the southeastern corner of martha’s Vineyard, massachusetts. The region is characterized by strong tidal currents, intermittent energetic surface wave events, and shallow water with local depth ranging from 2 to 7 m. This study uses the process-based model, delft3d, with a three-dimensional approach to examine the sand wave dynamics by incorporating surface waves, winds, currents, and bathymetric observations. The model successfully simulates sand wave migration in comparisons to observations. model sensitivity analyses show that the sand wave migration reduces by 65% with the absence of the surface waves. The modeled sand wave migration speed is correlated with the tidal current Shields parameter, and sharp increases in migration speed occur when the wave-driven Shields parameter increases in response to energetic surface wave events. The combined effect of tides, surface waves, and bathymetry is the origin of the rotational aspect of the sand wave, using the Shields parameter as an indicator of tidal currents and surface wave influence on sand wave dynamics.

We report a significantly-enhanced bioinformatics suite and database for proteomics research called Yale Protein Expression database（YPEd） that is used by investigators at more than 300 institutions worldwide. YPEd meets the data management, archival, and analysis needs of a high-throughput mass spectrometry-based proteomics research ranging from a singlelaboratory, group of laboratories within and beyond an institution, to the entire proteomics community. The current version is a significant improvement over the first version in that it contains new modules for liquid chromatography–tandem mass spectrometry（LC–mS/mS） database search results, label and label-free quantitative proteomic analysis, and several scoring outputs for phosphopeptide site localization. In addition, we have added both peptide and protein comparative analysis tools to enable pairwise analysis of distinct peptides/proteins in each sample and of overlapping peptides/proteins between all samples in multiple datasets. We have also implemented a targeted proteomics module for automated multiple reaction monitoring（mRm）/selective reaction monitoring（SRm） assay development. We have linked YPEd＇s database search results and both label-based and label-free fold-change analysis to the Skyline Panorama repository for online spectra visualization. In addition, we have built enhanced functionality to curate peptide identifications into an mS/mS peptide spectral library for all of our protein database search identification results.

Complement receptor 2 （CR2/Cd21） is predominantly expressed on the surface of mature B cells where it forms part of a coreceptor complex that functions, in part, to modulate B-cell receptor signal strength. CR2/Cd21 expression is tightly regulated throughout B-cell development such that CR2/Cd21 cannot be detected on pre-B or terminally differentiated plasma cells. CR2/Cd21 expression is upregulated at B-cell maturation and can be induced by IL-4 and Cd40 signaling pathways. We have previously characterized elements in the proximal promoter and first intron of CR2/Cd21 that are involved in regulating basal and tissue-specific expression. We now extend these analyses to the CR2/Cd21 core promoter. We show that in mature B cells, CR2/~d21 transcription proceeds from a focused TSS regulated by a non-consensus TATA box, an initiator element and a downstream promoter element. Furthermore, occupancy of the general transcriptional machinery in pre-B versus mature B-cell lines correlate with CR2/Cd21 expression level and indicate that promoter accessibility must switch from inactive to active during the transitional B-cell window.

during food shortages, the gray mouse lemur（microcebus murinus） of madagascar experiences daily torpor thereby reducing energy expenditures. The present study aimed to understand the impacts of torpor on the immune system and antioxidant response in the gut of these *** interaction may be of critical importance given the trade-off between the energetically costly immune response and the need to defend against pathogen entry during hypometabolism. The protein levels of cytokines and antioxidants were measured in the small intestine（duodenum, jejunum,and ileum） and large intestine of aroused and torpid lemurs. While there was a significant decrease of some pro-inflammatory cytokines（IL-6 and TNF-a） in the duodenum and jejunum during torpor as compared to aroused animals, there was no change in anti-inflammatory cytokines. We observed decreased levels of cytokines（IL-12p70 and m-CSF）, and several chemokines（mCP-1and mIP-2） but an increase in mIP-1a in the jejunum of the torpid animals. In addition, we evaluated antioxidant response by examining the protein levels of antioxidant enzymes and total antioxidant capacity provided by metabolites such as glutathione（and others）. Our results indicatedthat levels of antioxidant enzymes did not change between torpor and aroused states, although antioxidant capacity was significantly higher in the ileum during torpor. These data suggest a suppression of the immune response, likely as an energy conservation measure, and a limited role of antioxidant defenses in supporting torpor in lemur intestine.

decidual natural killer （dNK） cells express an array of activation receptors to regulate placental immunity and development during early pregnancy. We investigated the functional character of human dNK cells during the first and second trimester of gestation and the interaction between dNK and trophoblast cells. Although the frequency of Cd56＋Cd16-dNK among the total Cd45＋ leukocytes did not change over this period, the expression of the activating receptors, NKp80 and NKG2d, was greatly upregulated. We observed a significantly higher number of extravillous trophoblast cells in proximity to the dNK cells in the first trimester in comparison with the second trimester decidua. NKG2d expression by first trimester dNK cells was decreased when co-cultured with the HTR-8 trophoblast cell line. In the second trimester, functional markers of dNK activation, i.e., angiogenic factor production （e.g., vascular endothelial growth factor, interleukin-8, interferon-gamma）, remained stable despite an increase in NKp80 or NKG2d surface expression. Furthermore, the degranulation capacity of dNK cells, as assessed by Cd107a, was decreased in the second trimester. We suggest that in the first trimester, trophoblast-dNK interactions generate a population of dNK cells with a suppressed activating phenotype. In the second trimester, the loss of trophoblast-dNK interactions led to the inhibition of dNK cell function, although their activating receptor expression was increased. We speculate that during pregnancy, two mechanisms operate to modulate the dNK cell activation：suppression of activating receptor levels in the first trimester by trophoblasts and disengagement of receptor-ligand coupling in the second trimester.
