In the Algorithm for Diagnosis and Treatment in the Japanese Evidence-Based Clinical Practice Guidelines for Hepatocellular Carcinoma, the treatment strategy is determined by three major factors: liver function and the number and size of tumors. The algorithm is quite simple, consisting of fewer components than the Barcelona-Clinic Liver Cancer staging system. In this article, we describe the roles of the treatment algorithmin hepatectomy and perioperative management of hepatocellular carcinoma.

The global distribution of hepatocellular carcinoma(HCC) varies markedly among regions, and patients in East Asia and Central Africa account for about 80% of all cases. The risk factors are hepatitis B, hepatitis C, alcohol, and etc. The risk of carcinogenesis further increases with progression to hepatic cirrhosis in all liver disorders. Radical treatment of HCC by liver resection without causing liver failure has been established as a safe approach through selection of an appropriate range of resection of the damaged liver. This background indicates that both evaluation of hepatic functional reserve and measures against concomitant diseases such as thrombocytopenia accompanying portal hypertension, prevention of rupture of esophageal varices, reliable control of ascites, and improvement of hypoalbuminemia are important issues in liver resection in patients with hepatic cirrhosis. We review the latest information on perioperative management of liver resection in HCC patients with hepatic cirrhosis.

Our purpose was to determine how enamel matrix derivative (EMD) affects the expression ofosteogenesis- and chondrogenesis-related transcription factors in undifferentiated mesenchymal cells.C3H10T1/2 cell line,a typical pluripotential mesenchymal cell line,was cultured with or without EMD forup to 7 *** of mRNAs encoding osteogenesis-and chondrogenesis-related transcription factors(Runx2, Osterix,AJ18,Dlx5,Msx2,Sox5,Sox9 and Zfp60) was measured using real-time polymerasechain ***2 and Sox9 protein expression and the presence of bone morphogenetic protein (BMP)-6-like molecules in EMD were determined by Western *** substantially increased mRNA levels ofosteogenesis-and chondrogenesis-related transcription *** also induced Runx2 and Sox9 *** blotting analysis of EMD using anti-BMP-6 antibody revealed immunoreactive bandscorresponding to about 14 kDa and 60 *** results suggest that EMD stimulates osteogenesis- andchondrogenesis-related transcription factors,and these activities may be mediated,at least in part,by BMP-6in EMD.

An ultra-thin flattened heat pipe has been developed with a centered wick *** structure is essential to make the heat pipe ***,the centered wick structure reduces the evaporation and condensation surface areas of the wick structure because it is sandwiched between heat pipe *** this study,because detailed discussion has not been made,heat transfer experiments were conducted for the wick structure sandwiched between two solid *** study focused on the evaporation heat transfer characteristics from the sandwiched wick *** experiments were conducted with three wick structures,that is,strip-shaped sintered copper powders with thicknesses of 0.5,1.0,and 1.5 *** was used as working *** capillary pumping performance,that is,the liquid lifting velocities of the three wick structures were the *** experimental results of the three wick structures were compared regarding the relation between the evaporation heat transfer rate and the superheat of the working *** heat transfer experiments were also conducted when one of the solid walls was removed from the wick *** was confirmed that even if the wick structure was sandwiched between the solid walls,sufficient evaporation of the working fluid occurred from the thin sides of the wick structure.

Optimal Design of TMD Under Long-Term Nonstationary Wave Loading

Traditionally, the use of a tuned mass damper (TMD) is to improve the surviability of the primary structure under extraordinary loading environment while the design loading condition is described by either a harmonic function or a stationary random process that can be fully characterized by a power spectral density (PSD) function. Aiming at prolonging the fatigue life of an offshore platform, this study considers an optimal design of TMD for the platform under long-term nonstationary loading due to long-term random sea waves characterized by a probabilistic power spectral density (PPSD) function In principle, a PPSD could be derived based on numerous ordinary PSD functions; and each of them is treated as realization of the corresponding PPSD. This study provides a theoretical development for the optimal TMD design by minimizing the cost function to be the mean square value of the expected long term response. A numerical example is presented to illustrate the developed design procedure.

Objective： To investigate the influence of the depth of skin press in blunt tip placebo acupuncture needles on patient blinding and its relationship to needle ***： Forty healthy volunteers were enrolled as subjects for patient blinding. Four acupuncturists applied the following needles randomly at three points in each forearm： 0.18 mm and 0.25 mm diameter penetrating needles inserted to a depth of 5 mm, and 0.18 mm and 0.25 mm diameter skin-touch needles depressing the skin at the acupoint to a depth of 1 mm and 2 mm from the skin surface. The subjects reported their guesses at the nature of needles they received, and rated needle pain and de qi. A blinding index was calculated to define the success of blinding for ***： The blinding status of subjects for 1 mm press needles of 0.18 mm diameter was ＂random guess＂,but ＂unblinded＂ for 1 mm press needles of 0.25 mm diameter. For 2 mm press needles of both diameters,the blinding status was ＂opposite guess＂ and the blinding status for penetrating needles of both diameters was ＂unblinded.＂ The percentages of ＂felt pain＂ with 2 mm press needles of both diameters were similar to that with penetrating needles, but those were not similar for 1 mm press needles. The frequency of de qi occurrence with 2 mm press needles of 0.18 mm diameter was similar to that of penetrating needles of both ***： Placebo needles of 2 mm press made more subjects guess that the needles penetrated the skin than 1 mm press needles. The use of small diameter needles increased patient blinding.

Gastric cancer is one of the most common neoplasms in Japan, and it is also the second leading cause of cancer-related deaths worldwide. Nowadays, infection with Helicobacter pylori (H. pylori) is a known risk factor for the development of gastric cancer. Therefore, gastric cancer should be considered as an infectious disease, and in fact, prophylactic eradication of H. pylori may prevent the development of metachronous gastric carcinoma. Before the role of H. pylori was understood, a different approach was used. Recently even after the cancer has developed, some newer therapeutic approaches have been pursued. These newer treatments have been summarized as "minimally invasive therapies" and use endoscopic or laparoscopic techniques. In addition, robotic approaches are being developed that seem to hold a great potential to change the surgical approach. Since basic understanding and treatment of the disease have both changed signif icantly over the last decade, we present a review of current advances in gastric cancer research and therapy.

The reflection and diffraction of a planar shock wave around a circular cylinder are a typical problem of the complex nonlinear shock wave phenomena in *** has long been studied experimentally,analytically as well as *** in 1987 obtained clear experimental pictures ofisopycnics in shock tube under the condi- tion that the impinging shock wave propagates as far as 3 diameters away from the *** know more complete- ly the whole unsteady process,it is desirable to get experimental results in a region which is more than 10 diameters away from the *** is what has been done in this paper by using the pulsed laser holographic interferometry for several shock Mach numbers of the impinging shock. Results for several moments are shown,giving more know- ledge about the whole unsteady flow *** is useful for a reliable and complete understanding of the changing force acting on the cylinder,and provides interesting data to check the performance of many recently developed high resolution numerical methods for unsteady shock wave calculation.

In Xenopus, injection of S-adenosylmethionine decarboxylase (SAMDC) mRNA into fertilized eggs or2-cell stage embryos induces massive cell dissociation and embryo-lysis at the early gastrula stage due toactivation of the maternal program of apoptosis. We injected SAMDC mRNA into only one of the animal side blastomeres of embryos at different stages of cleavage, and examined the timing of the onset of theapoptotic reaction. In the injection at 4- and 8-cell stages, a considerable number of embryos developed intotadpoles and in the injection at 16- and 32-cell stages, all the embryos became tadpoles, although tadpolesobtained were sometimes abnormal. However, using GFP as a lineage tracer, we found that descendant cellsof the blastomere injected with SAMDC mRNA at 8- to 32-cell stages are confined within the blastocoel atthe early gastrula stage and undergo apoptotic cell death within the blastocoel, in spite of the continueddevelopment of the injected embryos. In a recent study aimed at validating the Park sham needle for use as a double-blind control intervention,To and Alexander concluded that the practitioner who was performing acupuncture recognized the type of needle[1].This paper may lead the reader to misunderstand the design and purpose of placebo/sham acupuncture[2–6]which have been developed in recent decades for the purpose of acupuncture research.
