Exploring the Pathways of Leprosy Patients from Their Communities to a Diagnosis in the Districts of Mayuge, Yumbe and Kasese-Uganda

Background: Leprosy is known to cause disability that leads to severe outcomes like stigma, discrimination, mental health problems and participation restriction. Furthermore, in cases of infectious leprosy, longer delays increase the risk for the spread of the disease. Despite being preventable and curable, a significant proportion of new leprosy patients (39%) in 2019 had grade 2 (Described as Visible disability) at the time of diagnosis signifying late presentation. The aim of this study was to describe patient journeys from first symptoms suggestive of leprosy to a diagnosis and individual and community level factors associated with health seeking behavior of leprosy patients. Methods: This was a cross-sectional explorative study implemented in Kasese, Mayuge and Yumbe districts .A structured questionnaire was used to collect quantitative data. Qualitative assessment included patients, family members, health workers, voluntary health teams and the district health team. Descriptive statistics were presented in terms of percentages, frequency tables, pie Charts and graphs for easy interpretation and discussion. Results: The results indicate that 53% of the respondents identified as female. The median age of the respondents being 34 years, with a range of 1 to 76 years (Mean: 44.7, Mode: 65, Standard-Deviation: 19.6, Kurtosis: 0.6). The most common first symptom noticed by respondents was skin lesions (65%) followed by deformities (18%) (P value = 0.05%) occurring mostly in the feet (P-value = 0.48). Majority (52%) of the patients had taken more than 24 months (SD 18.72 OR 2.75) for a diagnosis to be made with a maximum delay of over 60 months. The most common cause of delay in seeking health care was lack of knowledge on leprosy (P value=Conclusions: There was a delay of 2 years in seeking health care for the majority of the patients. Immune response to inactivated bacterial vector carrying the recombinant K39 antigen of Leishmania infantum in mice

The Effect of a Novel Complex, Composed of Ceramide, Energizing Peptide and Camu Camu Extract, on Epidermal Barrier Function and Dermal Antiaging Properties in Ex Vivo Human Skin Small Live Cohort

The skin’s primary function is to protect the body against a spectrum of environmental stressors, including mechanical insults, microorganisms, chemicals, and allergens. Located in the outermost layers, the primary structures and components responsible for the skin’s barrier function are susceptible to environmental variables, dermatological conditions, and the aging process. The ensuing alterations to structure, composition, and organizational attributes of the epidermal barrier can impact its integrity and functionality. The aim of this study was to assess the effect of a novel complex composed of a ceramide, energizing peptide, and Camu Camu extract (SUPCERATTM complex) on specific markers of epidermal barrier integrity, as well as epidermal and dermal function. All the experiments were conducted on fresh human abdominal skin explants. Intradermal production of hyaluronic acid, epidermal claudin-1, and ceramide synthase 3 expressions, as well as epidermal lipids content were assessed using specific fluorescent stainings on ex vivo skin after the application of the complex or placebo. Additionally, dermal elastase and collagenase activities were assessed using in tubo enzymatic assays. Lastly, the effect of a cosmetic cream containing SUPCERATTM complex was assessed using subjective global Aesthetic Improvement Scale (gAIS) in a small cohort of patients after 60 days of use. The application of the SUPCERATTM complex on ex vivo skin led to significant increase in dermal hyaluronic acid content and epidermal activity of claudin-1, ceramide synthase 3 and epidermal ceramide content. Furthermore, in tubo enzymatic assays demonstrated inhibition of both dermal elastase and collagenase activities. In addition, the patient-reported results indicated significant improvements in skin quality and appearance. .

Analysis of Changes of Extreme Temperature during June to August Season over Tanzania

Natural and human systems are exposed and vulnerable to climate extremes, which contributes to the repercussions of climate variability and the probability of disasters. The impacts of both natural and human-caused climate variability are reflected in the reported changes in climate extremes. Particularly at the local community levels in the majority of the regions, there is currently a dearth of information regarding the distribution, dynamics, and trends of excessive temperatures among the majority of Tanzanians. Over the years 1982-2022, this study examined trends in Tanzania’s extreme temperature over the June to August season. Based on the distinction between absolute and percentile extreme temperatures, a total of eight ETCCDI climate indices were chosen. Mann-Kendall test was used to assess the presence of trends in extreme climatic indices and the Sen’s Slope was applied to compute the extent of the trends in temperature extremes. The study showed that in most regions, there is significant increase of warm days and nights while the significant decrease of cold days and nights was evident to most areas. Moreover, nighttime warming surpasses daytime warming in the study area. The study suggests that anthropogenic influences may contribute to the warming trend observed in extreme daily minimum and maximum temperatures globally, with Tanzania potentially affected, as indicated in the current research. The overall results of this study reflect patterns observed in various regions worldwide, where warm days and nights are on the rise while cold days and nights are diminishing.

Near-Surface Soil Chemical Properties as Affected by Cover Crops Over Time in the Lower Mississippi River Valley

Typical row-crop agricultural practices can potentially be harmful to soil health and future sustainability. The use of cover crops (CC) as a mechanism to improve soil health on a wide scale remains underutilized. Soil health remains a major concern for the sustainability of agricultural productivity, therefore, research into CC implementation as a mean to preserve or improve soil health is warranted. The objective of this study was to evaluate the effects of CC on the soils in the eastern Arkansas portion of the Lower Mississippi River Valley (LMRV) over time for various chemical soil parameters, including pH, soil organic matter (SOM), soil elemental contents (i.e., P, K, Ca, Mg, S, Na, Fe, Mn, Zn, Cu, and B), soil respiration, and a generalized soil health score index. Soil pH decreased over time under both CC and no-cover-crop (NCC) treatments, by −0.3 and −0.2, respectively. Soil OM decreased over time under NCC by −0.1%, but did not differ between CC treatments. Soil N availability decreased over time under NCC (−22.6 kg·ha−1), but did not change over time under CC. Soil respiration decreased over time under both CC and NCC, by −76.1 mg·L−1 and −77.3 mg·L−1, respectively, though there was no effect of CC treatment. The Haney soil health score index decreased under CC (−7.0) and NCC (−6.8) without an effect from CC treatment. Results of the study place emphasis on the temporal nature of soil health as influenced by cover crops and their potential to improve soil health.

Feasibility Study of Assist-Linked Brief Intervention for Substance Use Disorders in a Rural Community of Plateau State, Nigeria

Objective: We studied the feasibility of using Alcohol, Smoking, and Substance Involvement Screening Test (ASSIST) linked with Brief Intervention (ALBI) in treating Substance Use Disorder in Nigeria. There is a clear gap in meeting the needs for treatment and care for people with drug use disorders in Nigeria with many users reporting a self-perceived need for treatment and about 40% of them wanting to receive drug treatment but were unable to access such services. Methods: ASSIST questionnaire was used to gather baseline data, and ALBI along with Motivational Interviewing (MI) was used to intervene for 6 weeks following which ASSIST score was repeated and compared with baseline. Results: Majority of the participants were aged 21 - 30 years, mostly males with (61.3%) lacking steady jobs. Lifetime prevalence of any drug use was (88.2%). Three months prevalence was nicotine (72.0%), alcohol (66.7%), cannabis (47.3%), opioids (11.8%) among others. The ASSIST scores were mostly on the severe ranges. ASSIST result after six weeks showed marked reduction. The Mean Difference (MD) in the ASSIST scores after intervention was significant for tobacco, opioids, alcohol, and cannabis. The Cohen’s D effect size was large for tobacco (1.08), alcohol (0.92) and cannabis (0.73) but low for other substances. Conclusion: ALBI along with MI was helpful in combating the menace of substance use.

Mathematics is a key factor in achieving the Sustainable Development goals (SDgs), because of its applicability to real situations. To achieve the set goals in SDg, this paper suggests some mathematical methods that will be useful for solving real situations in relation to goals 2 and 12 of SDgs approved by UN when modeled mathematically. The Northwest Corner Method (NWCM), Least Cost Method (LCM), and Vogel Approximation Method (VAM), which are the initial solution methods were examined to ascertain the ideal route of transporting commodities from production facilities to requirement destination while the optimal solution methods involve Stepping Stone Method (SSM), and Modified Distribution Method (MDM), that give the feasible solution which will enhance minimum transportation cost were also thoroughly defined. Subsequent research shall focus on application of the methods in relation to SDgs problems in comparison with other existing methods.
