Prognostic value of post-treatment ^(18)F-FDG PET/CT for advanced head and neck cancer after combined intra-arterial chemotherapy and radiotherapy

Objective：To clarify the prognostic value of post-treatment 18F-fluorodeoxyglucose （FDG） positron emission tomography （PET）/computed tomography （CT） in patients with advanced head and neck squamous cell carcinoma （HNSCC） after combined intra-arterial chemotherapy and radiotherapy （IACR）.Methods：Thirty-six patients with HNSCC who underwent IACR were *** period from the end of IACR to the last post-treatment 18F-FDG PET/CT examination was 8-12 *** patient-based and lesion-based analyses were used to evaluate the PET/CT *** lesion-based analysis,36 regions （12 lesions of recurrences and 24 scars at primary sites） were *** Kaplan-Meier method was used to assess the overall survival （OS） stratified by 18F-FDG uptake or visual interpretation ***：Twelve patients with recurrence were identified by six months after *** sensitivity and specificity in the patient-based analysis were 67％（8/12） and 88％（21/24）,*** mean OS was estimated to be 12.1 months （95％ CI,6.3-18.0 months） for the higher maximum standardized uptake value （SUVmax） group （n=7） and 44.6 months （95％ CI,39.9-49.3 months） for the lower SUVmax group （n=29）.OS in the higher SUVmax group （cut-off point,6.1） or positive visual interpretation group was significantly shorter than that in the lower SUVmax or negative visual interpretation group （P＜0.001 and P＜0.05,respectively）.Conclusions：The SUVmax and visual interpretation of HNSCC on post-IACR 18F-FDG PET/CT can provide prognostic survival estimates.

目的;F-fluciclovine(anti-;F-FACBC)氨基酸代谢显像诊断脑肿瘤的效能较好,而;FFDG诊断脑肿瘤存在局限性。该研究通过Ⅱa期临床研究来评价anti-;F-FACBC在脑胶质瘤患者肿瘤中的分布和安全性。方法 5例脑胶质瘤拟行手术切除的患者术前静脉注射anti-;F-FACBC后行PET显像,同时行钆增强T;加权（CE-T;W）MRI。根据CE-T;MRI和anti-;F-FACBC PET显像结果,取术中标本行组织病理学分析。比较MRI和PET/CT图像上的肿瘤范围。Anti-;F-FACBC显像时绘制肿瘤和正常脑组织的时间-放射性曲线,在肿瘤、对侧正常脑组织和小脑部位勾画ROI,并计算上述部位SUV。根据患者主观症状、客观体征、心电图、生命体征、实验室检查结果和不良反应发生情况评价anti-;F-FACBC的安全性。结果所有患者的胶质瘤病灶均摄取anti-;F-FACBC,CE-T;W MRI也可显示病灶,但前者显像的病灶范围大于后者。组织病理学结果证实,有2例患者的病灶在anti-;F-FACBC显像的病灶范围内,但在CE-T;W MRI显示的范围外。2例患者出现轻度不良反应:1例头部隐痛到轻度头痛,1例全身不适;上述症状均自行缓解。轻度头痛和anti-;F-FACBC的因果关系无法排除。该研究还发现,肿瘤与正常组织存在较高的T/N比值。结论 Anti-;F-FACBC PET/CT显像可以显示胶质瘤的病灶范围,其准确性比CE-T;W MRI高。Anti-;F-FACBC用于脑胶质瘤患者显像有较好的安全性。

Usefulness of Y-shaped sheaths in CT angiography for examination of liver tumors

AIM: To conduct a single-stage, combined computed tomography (CT) arterial portography (CTAP) and CT arteriography (CTA) imaging operation, we used Y-shaped sheaths with 2 valves, which allowed the insertion of 2 catheters simultaneously. METHODS: Of 1254 patients who underwent abdominal angiography for transarterial embolization and/or intraarterial chemotherapy in our department from May 2002 to November 2009, 664 patients in whom Y-shaped sheaths with 2 valves were used underwent CT angiography using a combination of CTA and CTAP. The Seldinger method was used to insert a 10 cm Y-shaped short sheath with 2 valves into the femoral artery. Under radiographic guidance, a 3.2 French (Fr) catheter was placed in the celiac artery or proper hepatic artery, and a second 3.2 Fr catheter was then placed distal to the inferior pancreaticoduodenal artery of the superior mesenteric artery. CTAP was then performed followed by CTA 10 min later. Photographs were taken during the early and late phases of the ***: Insertion of 3.2 Fr catheters was not possible in 6 of 664 (0.9%) patients with strong curvature of the femoral artery and 4 of 664 (0.6%) patients with strong curvature of the abdominal aorta. In addition, performing CTAP and CTA as a single-stage combined intervention was not possible in 14 of 664 (2.1%) patients whose right hepatic artery originated from the superior mesenteric artery and in 8 of 664 (1.2%) patients whose left hepatic artery branched from the left gastric artery. There were no sheath-related complications such as those related to arterial dissection or hemostasis. CONCLUSION: Although transfers to and from the CT room were necessary for anatomically variant patients, CT angiography using the Y-shaped sheath for combined CTAP and CTA was considered useful.

A 67-year-old man visited our hospital with a history of continuous hematochezia leading to hemorrhagic *** abdominal computed tomography scan re-vealed a large mass in the ascending colon invading the duodenum and pancreatic head as well as extra-vasation of blood from the gastroduodenal artery(GDA) into the *** revealed an irregular ulcerative lesion and stenosis in the ascending ***,right hemicolectomy combined with pylorus-preser ving pancreaticoduodenectomy was ***,the tumor was classified as a moderately differentiated ***,cancer cells were mainly located in the colon but had also invaded the duode-num and pancreas and involved the ***,the tumor cells were positive for cytokeratin(CK)20 and carcinoembryonic antigen(CEA)but not forCK7 and carbohydrate antigen(CA)*** patient died 23 d after the surgery because he had another episode of arterial bleeding from the anastomosis *** En bloc resection of the tumor with pancreatico duodenectomy and colectomy performed for locally advanced colon cancer can ensure long-term survival,patients undergoing these procedures should be carefully monitored,particularly when the tumor involves the main artery.
