Are there sex-based disparities in cataract surgery?

AIM:To investigate sex-based differences in the occurrence of intra-operative and post-operative complications and associated visual outcomes following cataract ***:This was a retrospective study of patients who had phacoemulsification cataract surgery at the University of Colorado School of *** collected included the patient’s health history,ocular comorbidities,operative and post-operative complications,and the post-operative best corrected visual acuity(BCVA).The data were analyzed using univariate and multivariable logistic regression with generalized estimating equations to account for the correlation of some patients having two eyes included in the ***:A total of 11977 eyes from 7253 patients were included in the *** comorbidities differed by sex,with males having significantly higher percentages of traumatic cataracts(males 0.7%vs females 0.1%),prior ocular surgery(6.7%vs 5.5%),and mature cataracts(2.8%vs 1.9%).Conversely,females had significantly higher rates of pseudoexfoliation(2.0%vs 3.2%).In unadjusted analysis,males had higher rates of posterior capsular rupture(0.8%vs 0.4%)and vitreous loss(1.0%vs 0.6%),but this difference was not significant after adjustment for *** had a significantly increased risk of post-operative retinal detachment,but in multivariable analysis this was no longer *** were significantly less likely to undergo post-operative neodymium-doped yttrium aluminum garnet(Nd:YAG)laser capsulotomy for posterior capsule opacification(OR=0.8,95%CI=0.7-0.9,P=0.0005).The BCVA was slightly worse for males pre-operatively;but post-operatively,both sexes exhibited similar visual acuity of Snellen equivalent 20/***:The study finds that in a cohort of patients presenting for cataract surgery,sex differences exist in pre-operative comorbidities and surgical characteristics that contribute to higher rates of some complications for ***,observed surgical complicatio

Upper gastrointestinal function and glycemic control in diabetes mellitus

Recent evidence has highlighted the impact of glycemic control on the incidence and progression of diabetic micro- and macrovascular complications, and on cardiovascular risk in the non-diabetic population. Postprandial blood glucose concentrations make a major contribution to overall glycemic control, and are determined in part by upper gastrointestinal function. Conversely, poor glycemic control has an acute, reversible effect on gastrointestinal motility. Insights into the mechanisms by which the gut contributes to glycemia have given rise to a number of novel dietary and pharmacological strategies designed to lower postprandial blood glucose concentrations.

Pediatric inflammatory bowel disease

Inflammatory bowel disease is an important cause of gastrointestinal pathology in children and adolescents. The incidence of pediatric inflammatory bowel disease is increasing; therefore, it is important for the clinician to be aware of the presentation of this disease in the pediatric population. laboratory tests, radiology studies, and endoscopic procedures are helpful in diagnosing inflammatory bowel disease and differentiating between Crohn＇s disease and ulcerative colitis. Once diagnosed, the goal of medical management is to induce remission of disease while minimizing the side effects of the medication. Specific attention needs to be paid to achieving normal growth in this susceptible population. Surgical management is usually indicated for failure of medical management, complication, or malignancy. Algorithms for diagnostic evaluation and treatment of pediatric inflammatory bowel disease are presented. The specific psychosocial issues facing these patients are also discussed in this review as are the future goals of research in the complex problem of pediatric inflammatory bowel disease.

Hyperhomocysteinemia as a potential contributor of colorectal cancer development in inflammatory bowel diseases: A review

Homocysteine is an amino acid generated metabolically by the S-adenosylmethionine-dependent transmethylation pathway. In addition to being a well-known independent risk factor for coronary heart disease, is also a risk factor for cancer. Patients suffering from inflammatory bowel diseases(IBD) including ulcerative colitis and Crohn's disease are at increased risk of developing colorectal cancer in comparison to healthy individuals. Furthermore, the risk of hyperhomocysteinaemia is significantly higher in IBD patients when compared with controls. In the present article, we review the mechanisms in which hyperhomocysteinemia may contribute to increased risk of colorectal cancer in IBD patients.

Metadata requirements analysis for the emerging Sensor Web

The Sensor Web has emerged from Earth Science research with the development of Web technology,to achieve process automation,sensor interoperation,and service *** promises require the discovery of the right sensor at the right time and the right location with the right ***,for sensor,platform,and data,are crucial for achieving such ***,analysis and practical use of these metadata reveals that the metadata and their associations are not applicable or suitable for the Sensor *** shortfalls are(1)the nonstandard metadata expression language;(2)the missing link between sensor and domain knowledge;(3)the insufficiency in the information for geographic locating and sensor tasking;and(4)the enhanced requirements on the quality,security,and ownership of both sensors and their sensed *** paper reviews the current standards that have metadata components for the sensor and its platform,especially those from ISO TC211,Open Geospatial Consortium Inc.,and The National Aeronautics and Space Administration Global Change Master Directory.A recommendation on metadata that meets the requirement of crossmission sensor discovery in a pervasive Web environment is derived from *** recommendation addresses issues on language formalization,sensor geolocation,semantics,quality,and *** of the emerging semantic Web technology for enabling robust discovery of sensor are discussed.

Microencapsulated paraffin as a tribological additive for advanced polymeric coatings

Numerous tribological applications,wherein the use of liquid lubricants is infeasible,require adequate dry *** the use of polymers as an effective solution for dry sliding tribological applications,their poor wear resistance prevents the utilization in harsh industrial *** methods are typically implemented to tackle the poor wear performance of polymers,however sacrificing some of their mechanical/tribological ***,we discussed the introduction of a novel additive,namely microencapsulated phase change material(MPCM)into an advanced polymeric ***,paraffin was encapsulated into melamine-based resin,and the capsules were dispersed in an aromatic thermosetting co-polyester(ATSP)*** found that the MPCM-filled composite exhibited a unique tribological behavior,manifested as“zero wear”,and a super-low coefficient of friction(COF)of *** developed composite outperformed the state-of-the-art polytetrafluoroethylene(PTFE)-filled coatings,under the experimental conditions examined herein.
