Hepatocellular carcinoma （HCC） is the most common primary liver tumor. It is the seventh world＇s most common cancer and rank second of all cancer deaths （I）. It incurs a global health burden, with 782,451 new cases diagnosed worldwide in the year 2012 and a morality of 745,517： a high incidence to morality ratio of 1.05. Majority of patients resides in the developing world and often presented late resulting in poor prognosis. Early detection is thus crucial for capturing early disease whereby curative resection can offer the best survival outcome （2）.

BACKGROUND Repeated liver resection is an effective treatment for recurrent hepatocellular carcinoma(HCC).However,few studies have compared the outcome of laparoscopic repeat hepatectomy(LRH)and open repeat hepatectomy(ORH)for recurrent HCC,and few of those have included cirrhotic *** To compare short-term and long-term outcomes of cirrhotic patients with LRH and ORH for recurrent *** We retrospectively analysed the clinical records retrieved from a prospectively collected database of all patients who underwent hepatectomy for posthepatectomy recurrent HCC at our institute between May 2006 and June *** of recurrent HCCs larger than 7 cm were *** demographics,operative details,perioperative outcomes,pathologic details,disease-free survival(DFS),and overall survival(OS)data of LRH and ORH were *** Data from 29 patients with LRH and 22 with ORH were *** LRH group showed significantly better outcomes for blood loss(median 300 mL vs 750 mL,P=0.013)and length of hospital stay(median 5 d vs 7 d,P=0.003).The 1-,3-and 5-year OS rates in the LRH group were 100.0%,60.0%and 30.0%,respectively;the corresponding rates in the ORH group were 81.8%,36.4%and 18.2%(P=0.336).The 1-,3-and 5-year DFS rates in the LRH group were 68.2%,27.3%and 4.5%,respectively;the corresponding rates in the ORH group were 31.3%,6.3%and 6.3%(P=0.055).There were no significant differences in overall and DFS between the two *** Laparoscopic re-resection should be considered for patients presenting with recurrent HCC less than or equal to 7 cm after previous hepatectomy.

BACKGROUND Needle-knife precut papillotomy(NKP)is typically performed ***,it remains unclear whether pancreatic stent(PS)placement can improve the outcomes of *** To explore whether PS placement improves the success rate of NKP in patients with difficult biliary *** This single-center retrospective study included 190 patients who underwent NKP between January 2017 and December 2021 after failed conventional biliary *** cases with incidental pancreatic duct cannulation during conventional biliary cannulation,the decision for pre-NKP PS placement was made at the endoscopist's *** primary outcome was the difference in the NKP success rate between patients with and without PS placement;the secondary outcome was the adverse event *** Among the 190 participants,82 received pre-NKP PS(PS-NKP group)whereas 108 did not[freehand or freehand NKP(FH-NKP)group].Post-NKP selective biliary cannulation was successful in 167(87.9%)patients,and the PS-NKP had a significantly higher success rate than the FH-NKP group(93.9%vs 83.3%,P=0.027).The overall adverse event rates were 7.3%and 11.1%in the PS-NKP and FH-NKP groups,respectively(P=0.493).A periampullary diverticulum(PAD)and significant intraoperative bleeding during NKP were independently associated with NKP failure;however,a pre-NKP PS was the only predictor of NKP *** the 44 participants with PADs,the PS-NKP group had a non-significantly higher NKP success rate than the FH-NKP group(87.5%and 65%,respectively;P=0.076).CONCLUSION PS significantly improved the success rate of NKP in patients with difficult biliary cannulation.

BACKGROUND:To investigate the prognostic value of the peripheral perfusion index(PPI)in patients with septic ***:This prospective cohort study,conducted at the emergency intensive care unit of Peking University People's Hospital,recruited 200 patients with septic shock between January 2023 and August *** patients were divided into survival(n=84)and death(n=116)groups based on 28-day *** evaluations included laboratory tests and clinical scores,with lactate and PPI values assessed upon admission to the emergency room and at 6 h and 12 h after *** factors associated with mortality were analyzed using univariate and multivariate Cox regression *** operator characteristic(ROC)curve was used to assess predictive *** rates were compared,and Kaplan-Meier survival plots were ***:Compared to the survival group,patients in the death group were older and had more severe liver damage and coagulation dysfunction,necessitating higher norepinephrine doses and increased fl uid *** lactate levels and lower PPI levels at 0 h,6 h,and 12 h were observed in the death *** Cox regression identifi ed prolonged prothrombin time(PT),decreased 6-h PPI and 12-h PPI as independent risk factors for *** area under the curves for 6-h PPI and 12-h PPI were 0.802(95%CI 0.742-0.863,P<0.001)and 0.945(95%CI 0.915-0.974,P<0.001),respectively,which were superior to Glasgow Coma Scale(GCS),Sequential Organ Failure Assessment(SOFA)scores(0.864 and 0.928).Cumulative mortality in the low PPI groups at 6 h and 12 h was signifi cantly higher than in the high PPI groups(6-h PPI:77.52%vs.22.54%;12-h PPI:92.04%vs.13.79%,P<0.001).CONCLUSION:PPI may have value in predicting 28-day mortality in patients with septic shock.
