Enhanced osseointegration of drug eluting nanotubular dental implants: An in vitro and in vivo study

Faster and predictable osseointegration is crucial for the success of dental implants, especially in patients with compromised local or systemic conditions. Despite various surface modifications on the commercially available Titanium (Ti) dental implants, the bioactivity of Ti is still low. Thus, to achieve both biological and therapeutic activity on titanium surfaces, surface modification techniques such as titanium nanotubes have been studied as nanotube surfaces can hold therapeutic drugs and molecules. The main aim of the present research work is to study the early osseointegration around the novel Simvastatin drug eluting nanotubular dental implant. In the present research, the titanium nanotubes were fabricated on the screw-shaped dental implant surface and the Simvastatin drug was loaded into the nanotubes using the ultrasonication dip method. In vitro and In vivo studies were carried out on the modified dental implants. In vitro cell culture study reported enhanced osteogenic activity on the drug-loaded nanotube surface implants. The in vivo animal studies were evaluated by micro-CT, histopathology, and reverse torque removal analysis methods. The test results showed faster osseointegration with the strong interface on the Simvastatin drug-loaded implant surface at 4 weeks of healing as compared to the control implants.

Identification of SNP and InDel variations in the promoter and 5′ untranslated regions of γ-tocopherol methyl transferase(ZmVTE4) affecting higher accumulation of α-tocopherol in maize kernel

Deficiency of vitamin-E or tocopherol causes neurological and cardiovascular disorders in humans. Though maize kernel is rich in total tocopherol, the level of α-tocopherol possessing the highest vitamin-E activity is low. Mutant allele of ZmVTE4 with deletion of 7 bp and 118 bp (0/0: most favorable haplotype) in 5′UTR and promoter region, respectively significantly enhances α-tocopherol in maize kernel than wild type haplotype (7/118).α- tocopherol estimation in 15 diverse inbreds revealed that mean α-/γ-tocopherol and α-/ total-tocopherol was much higher in genotypes with favorable haplotype (1.51 and 0.41) than unfavorable class (0.19 and 0.13), respectively. However even within favorable class,α- tocopherol ranged from 4.76 to 30.07 μg g 1. Sequence analysis of part of 5′UTR and promoter of ZmVTE4 among the genotypes with favorable haplotype revealed 14 SNPs (SNP1 to SNP14) and eight InDels (InDel1 to InDel8). SNP7 at 606 bp (G to A), and InDels viz., InDel1 (27 bp), InDel4 (27 bp) and InDel8 (14 bp) differentiated low and high α-tocopherol accumulating inbreds with favorable haplotype. Hence the newly identified SNP and InDels in addition to the already reported InDels can be useful in selection of favorable genotypes with higher α-tocopherol in maize.

Chronic hepatitis B(CHB) is a major global health problem affecting an estimated 350 million people with more than 786000 individuals dying annually due to complications, such as cirrhosis, liver failure and hepatocellular carcinoma(HCC). Liver transplantation(LT) is considered gold standard for treatment of hepatitis B virus(HBV)-related liver failure and HCC. However, post-transplant viral reactivation can be detrimental to allograft function, leading to poor survival. Prophylaxis with high-dose hepatitis B immunoglobulin(HBIG) and anti-viral drugs have achieved remarkable progress in LT by suppressingviral replication and improving long-term survival. The combination of lamivudine(LAM) plus HBIG has been for many years the most widely used. However, life-long HBIG use is both cumbersome and costly, whereas long-term use of LAM results in resistant virus. Recently, in an effort to develop HBIG-free protocols, high potency nucleos(t)ide analogues, such as Entecavir or Tenofovir, have been tried either as monotherapy or in combination with low-dose HBIG with excellent results. Current focus is on novel antiviral targets, especially for covalently closed circular DNA(ccc DNA), in an effort to eradicate HBV infection instead of viral suppression. However, there are several other molecular mechanisms through which HBV may reactivate and need equal attention. The purpose of this review is to address post-LT HBV reactivation, its risk factors, underlying molecular mechanisms, and recent advancements and future of anti-viral therapy.

Effectiveness of lukewarm water compress on reduction of breast pain and breast engorgement among post-cesarean section mothers

Objective:The study aimed to assess the effectiveness of lukewarm water compress on breast pain and breast engorgement among post-cesarean primiparous *** and Methods:This quasi-experimental posttest-only design was conducted with 60 post-cesarean section primiparous mothers between October 2018 and January 2019.A simple random sampling technique was used to allocate 30 participants to both experimental and control *** experimental group received lukewarm water compress using a sponge cloth for 20 min twice a day on the second,third,and fourth postnatal *** control group received routine hospital *** pain and engorgement were assessed using the Visual Analog Pain Scale and Breast Engorgement Assessment Scale on the third,fourth,and fifth postnatal ***:Significant differences were observed between the experimental and control groups on day 1 and day 3 in terms of breast pain and engorgement scores among post-cesarean section mothers(P0.05).Conclusion:Lukewarm water compress is effective in reducing breast pain and engorgement in post-cesarean primiparous *** research can include randomized controlled trials to evaluate the effectiveness of different therapies in treating breast pain and engorgement.

Meige syndrome is an idiopathic dystonia characterized by combination of blepharospasm and involuntary movements of the lower facial and/or masticatory (jaw) muscles. The condition is rare and has a variety of clinical presentations which often lead to its misdiagnosis. We report a case of Meige syndrome repeatedly misdiagnosed and treated unsuccessfully as conversion disorder.

knowledge Attitude and Practice towards Infection Control Measures amongst Medical Students in a Medical Teaching Tertiary Care Hospital

Introduction: Health care associated infections (HAIs) are likely to be the most common complication of hospital care. World Health Organization (WHO) estimates these infections to occur among 7% - 12% of the hospitalized patients globally, with more than 1.4 million people suffering from infectious complications acquired in the hospital at any time. keeping this in mind, the present study was conducted to delineate the level of knowledge, attitudes, and practices (kAP) for the use of proper disinfection procedures among the final year undergraduate medical students in a tertiary care hospital and teaching institute. Methodology: Present study was conducted at Government Medical College, Surat, a tertiary care hospital and teaching institute. The participants who gave consent were provided with a pre-tested questionnaire that included several questions on knowledge attitude and practices on hospital acquired infections and its control practices. Analysis of the answers was done based on kAP score. Result: 80 students who consented were included in the study. 75% of the study population had correct knowledge about the most common health care associated infections (HAIs), being infection of venous access, urinary tract infection, respiratory infections and surgical site infections, based on their current knowledge on hospital acquired infections. Majority of study participants agreed with the fact that the inappropriate application of disinfection procedures increases the risk for a health care worker of either acquiring or transmitting a HAI from/to a patient. >90% participants had a good attitude towards risk of getting or transmitting any infectious disease by a Health care worker (HCW) while working and the utility of the application of disinfection procedures during work would reduce the rates of HAI.
