AIm: To prospectively determine the safety and tolerability of oral L-selenomethionine(SLm) with concurrent chemoradiation(CCRT) for Stage Ⅲ non-small cell lung cancer(NSCLC) and estimate if the incidence and/or severity of adverse events could be reduced by its ***: Sixteen patients with stage Ⅲ NSCLC were accrued to this single arm, phase Ⅱ study. CCRT consisted of radiation given at 2 Gy per fraction for 30-33 fractions, 5 d per week with concurrent weekly Ⅳ paclitaxel 50 mg/m2 followed by carboplatin dosed at an area under the time-concentration curve of 2. SLm was dosed in a loading phase at 4800 μg twice daily for one week prior to CCRT followed by once daily dosing during treatment. RESULTS: No selenium-related toxicity was observed. Analysis revealed grade 3 or higher esophagitis in 3 of 16 patients(19%), pneumonitis in 0, leukopenia in 2(12.5%), and anemia in 1(6%); the latter two were significantly reduced when compared to the protocolstated expected rate of 35%(P = 0.045 for leukopenia, and P mia). median overall survival was 14.9 mo and median failure-free survival was 9 mo(95%CI: 3.3-21.5).CONCLUSION: There may be some protective benefit of selenium in the setting of CCRT for inoperable NSCLC. The data suggests decreased rates of myelosuppression when compared to similarly-treated historical and contemporary controls. Further evaluation of selenium in this setting may be warranted.

Objective: To investigate the molecular epidemiology and antimicrobial resistance patterns of methicillin-resistant Staphylococcus aureus(mRSA) among healthcare workers and ***: mRSA isolates were recovered from nasal swabs collected at a tertiary care hospital of Nepal and confirmed on the basis of Gram staining, conventional biochemical tests, and PCR amplification of mec A gene. PCRs were also used for detection of the different resistance genes and staphylococcal cassette chromosome(SCC) mec *** susceptibility patterns of isolates were assessed by disc diffusion method and minimum inhibitory concentrations were determined by ***: A total of 29 mRSA were isolated from 536 nasal swabs(5.4%) of health care workers and patients at a tertiary care hospital in Nepal. All isolates were susceptible to amikacin, gentamicin, vancomycin(minimal inhibitory concentrations mg/m L), tigecycline, tetracycline, nitrofurantoin, rifampicin, quinupristin-dalfopristin, and linezolid. Among the 29 mRSA isolates, resistance to erythromycin(72%), ciprofloxacin(75%), co-trimoxazole(62%), clindamycin(10%), and chloramphenicol(10%) was found, and fifteen isolates(51%)exhibited high-level mupirocin resistance(minimal inhibitory concentrations > 1 024 mg/m L).Fourteen isolates were found harboring the mup A gene and one isolate was found carrying the novel mup B gene. High prevalence(68%) of SCCmec I type was found, followed by SCCmec V(13%) and SCCmec III(3%) among all the mRSA ***: We found the emergence of SCCmec type I with high-level mupirocin resistance among mRSA in Nepal. Data also suggest that mRSA SCCmec type V strain has spread from the community to the hospital.

The critical nature of satellite network traffic provides a challenging environment to detect intrusions. The intrusion detection method presented aims to raise an alert whenever satellite network signals begin to exhibit anomalous patterns determined by Euclidian distance metric. In line with anomaly-based intrusion detection systems, the method presented relies heavily on building a model of＂normal＂ through the creation of a signal dictionary using windowing and k.m.ans clustering. The results of three signals fi＇om our case study are discussed to highlight the benefits and drawbacks of the method presented. Our preliminary results demonstrate that the clustering technique used has great potential for intrusion detection for non-periodic satellite network signals.

Spectral Crop Coefficient Approach for Estimating Daily Crop Water Use

While the amount of water used by a crop can be measured using lysimeters or eddy covariance systems, it is more common to estimate this quantity based on weather data and crop-related factors. Among these approaches, the standard crop coefficient method has gained widespread use. A limitation of the standard crop coefficient approach is that it applies to “standard conditions” that are invariant from field to field. In this article, we describe a method for estimating daily crop water use (CWU) that is specific to individual fields. This method, the “spectral crop coefficient” approach, utilizes a crop coefficient numerically equivalent to the crop ground cover observed in a field using remote sensing. This “spectral crop coefficient” ksp is multiplied by potential evapotranspiration determined from standard weather observations to estimate CWU. We present results from a study involving three farmers' fields in the Texas High Plains in which CWU estimated using the ksp approach is compared to observed values obtained from eddy covariance measurements. Statistical analysis of the results suggests that the ksp approach can produce reasonably accurate estimates of daily CWU under a variety of irrigation strategies from fully irrigated to dryland. These results suggest that the ksp?approach could be effectively used in applications such as operational irrigation scheduling, where its field-specific nature could minimize over-irrigation and support water conservation.

Numerical methods are used to evaluate variations of the electromagnetic fields generated by a head-sized birdcage coil as a function of load (“loading effect”). The loading effect was analyzed for the cases of a coil loaded with a conductive cylindrical sample, a dielectric cylindrical sample, and an anatomically precise head model. maxwell equations were solved by means of finite difference time domain (FDTD) method conducted at 12.8, 64, and 128 mHz. Simulation results indicate that at 12.8 mHz the conservative electric field (Ec) caused by the scalar electric potentials between the coil and the load or within the load was significantly higher than the magnetically-induced electric field (Ei) and was the major component of the total electric field (Etotal). The amplitudes of Ec and Etotal are seen to be lower within a sample than at a corresponding location in an empty coil, but approximately 65% higher in the space between coil and sample than at a corresponding location in an empty coil. This is due to polarization effects generating an additional scalar potential parallel to the original field. The increased electric field between coil and sample may cause increased power deposition at the surface of the sample and may affect the RF-induced currents in external leads used for physiological recording, i.e. ECG, during mRI scanning.

Assessment of multi-modality evaluations of obscure gastrointestinal bleeding

AImTo determine the frequency of bleeding source detection in patients with obscure gastrointestinal bleeding (OGIB) who underwent double balloon enteroscopy (DBE) after pre-procedure imaging [multiphase computed tomography enterography (mPCTE), video capsule endoscopy (VCE), or both] and assess the impact of imaging on DBE diagnostic *** cohort study using a prospectively maintained database of all adult patients presenting with OGIB who underwent DBE from September 1st, 2002 to June 30th, 2013 at a single tertiary *** hundred and ninety five patients (52% females; median age 68 years) underwent DBE for OGIB. AVCE and/or mPCTE performed within 1 year prior to DBE (in 441 patients) increased the diagnostic yield of DBE (67.1% with preceding imaging vs 59.5% without). Using DBE as the gold standard, VCE and mPCTE had a diagnostic yield of 72.7% and 32.5% respectively. There were no increased odds of finding a bleeding site at DBE compared to VCE (OR = 1.3, P = 0.150). There were increased odds of finding a bleeding site at DBE compared to mPCTE (OR = 5.9, P ment of OGIB, with a diagnostic yield that may be increased after obtaining a preceding VCE or mPCTE. However, inpatients with active ongoing bleeding may benefit from proceeding directly to antegrade DBE.
