BACKGROUND Cognitive dysfunction is the main manifestation of central *** cognitive impairments tend to be overlooked in patients with diabetes mellitus(DM),there is a growing body of evidence linking DM to cognitive *** is closely related to neurological abnormalities,while often disregarded in clinical *** in cerebral neurotransmitter levels are associated with a variety of neurological abnormalities and may be closely related to blood glucose control in patients with type 2 DM(T2DM).AIM To evaluate the concentrations of cerebral neurotransmitters in T2DM patients exhibiting different hemoglobin A1c(HbA1c)*** A total of 130 T2DM patients were enrolled at the Department of Endocrinology of Shanghai East *** participants were divided into four groups according to their HbA1c levels using the interquartile method,namely Q1(min(mALB),triglyceride,low-density lipoprotein cholesterol(LDL-C),homeostasis model assessment of insulin resistance(HOMA-IR),and beta cell function(HOMA-β),N-acetylaspartate/creatine(NAA/Cr),and NAA/choline(NAA/Cho).Spearman correlation analysis showed that mALB,LDL-C,HOMA-IR and NAA/Cr in the left brainstem area were positively correlated with the level of HbA1c(P<0.05),whereas HOMA-βwas negatively correlated with the HbA1c level(P<0.05).Ordered multiple logistic regression analysis showed that NAA/Cho[Odds ratio(OR):1.608,95%confidence interval(95%CI):1.004-2.578,P<0.05],LDL-C(OR:1.627,95%CI:1.119-2.370,P<0.05),and HOMA-IR(OR:1.107,95%CI:1.031-

For advanced aero-engine design and research,modeling and simulation in a digital environment is indispensable,especially for engines of complicated conﬁgurations,such as var-iable cycle engines(VCE)and adaptive cycle engines(ACE).Also,in the research of future smart engines,reliable real-time digital twins are ***,the 2 dominant methods that used in solving the simulation models,Newton-Raphson(N-R)method and volume-based method,are not fully qualiﬁed for the study requirements,because neither of them reaches the satisfactory balance of convergence rate and calculating efﬁ*** this study,by deeply analyzing the mathematical principle of these 2 methods,a novel modeling and solving method for aero-engine simulation,which integrates the advantages of both N-R and volume-based methods,is *** has distributed architecture and local quadratic convergence *** a novel modeling method for variable area bypass injectors(VABI)is put *** facilitate simulation of various conﬁgurations of *** modeling cases,including a high bypass-ratio(BPR)turbofan and an ACE,illustrate that the novel technique decreases the iterations by about two-thirds comparing with volume-based method,while the success rate of convergence remains over 99%.This proves its superiority in both convergence and calculating efﬁciency over the conventional *** technique can be used in advanced gas turbine en-gine design and control strategy optimization,and study of digital twins.

BACKGROUND Women have a 3%lifetime chance of developing an inguinal hernia,which is not as common in *** to its cosmetic benefits,single-incision laparoscopic transabdominal preperitoneal(SIL-TAPP)inguinal hernia repair is becoming in-creasingly popular in the management of inguinal hernia in ***,there are no studies comparing the safety and applicability of SIL-TAPP repair with conventional laparoscopic transabdominal preperitoneal(CL-TAPP)inguinal hernia repair for the treatment of inguinal hernia in *** To compare the outcomes of SIL-TAPP and CL-TAPP repair in adult female patients with inguinal hernia and to estimate the safety and applicability of SIL-TAPP repair in adult female inguinal hernia *** We retrospectively compared the clinical information and follow-up data of fe-male inguinal hernia patients who underwent SIL-TAPP inguinal hernia repair and those who underwent CL-TAPP inguinal hernia repair at the Affiliated Hos-pital of Nantong University from February 2018 to December 2020 and assessed the long-term and short-term outcomes of both *** This study included 123 patients,with 71 undergoing SIL-TAPP repair and 52 un-dergoing CL-TAPP *** two cohorts of patients and inguinal hernia charac-teristics were similar,with no statistically meaningful *** rate of intraoperative inferior epigastric vessel injury was lower in patients in the SIL-TAPP cohort(0,0%)than in patients in the CL-TAPP cohort(4,7.7%)and was significantly different(P<0.05).In addition,the median[interquartile range(IQR)]total hospitalization costs were significantly lower in patients in the SIL-TAPP cohort[$3287(3218-3325)]than in patients in the CL-TAPP cohort[$3511(3491-3599)].Postoperatively,the occurrence rate of trocar site hernia was lower in the SIL-TAPP cohort(0,0%)than in the CL-TAPP cohort(4,7.7%),and the median(IQR)cosmetic score was significantly higher in the SIL-TAPP cohort[10(10-10)]than in the CL-TAPP cohort[9(9

Hematospermia is a rare condition, but often causes frustration. The reason for this frustration is that theexact incidence of hematospermia remains unknown and its cause is difficult to determine.^1 Many reports in the past decades have focused on the etiology of hematospermia.^2 Hematospermia is treated initially by administration of antibiotics, coagulants, and sex steroid hormones. However, some cases may prove resistant to this therapy and the condition may continue. For some intractable and agnogenic cases, the therapeutic strategy mentioned above is not very effective. Recently, we treated 63 such nonresponsive patients by direct continuous antibiotic irrigation into the bilateral seminal vesicles through puncture under transrectal ultrasonography （TRUS） guidance.

Effect of confining pressure on rock breaking by high-pressure waterjet-assisted TBM

High-pressure waterjet-assisted tunnel boring machine(WTBM)is an efficient method for improving the tunneling performance of a tunnel boring machine(TBM)and reducing the wear of its disc cutters in hard rock with high *** pressure directly affects the efficiency of rock breaking and the configuration of the disc *** this study,we evaluated the effect of confining pressure on WTBM rock breaking by developing a self-designed and manufactured experimental system,including confining pressure loading,TBM disc-cutter penetration,and high-pressure *** macro fracture,acoustic emission(AE),peak normal force drop,and specific energy(SE)were analyzed for four different confining pressures(10,20,30,and 35 MPa).The results showed that the cutting depth of the waterjet increased linearly as the waterjet pressure increased and decreased with the gradual increase in the nozzle moving *** expansion and development of cracks formed rock debris,and the size of the rock fragments decreased with an increase in confining *** the waterjet pressure was 280 MPa,the nozzle moving velocity was 800 mm/min and the kerf space was 75 mm,which indicated that the confining pressure,which was 23.16 MPa,minimized the cutting SE under this ***,regardless of the confining pressure,the maximum normal force of WTBM was less than that of a TBM,whereas the SE of WTBM was less than that of complete TBM cutting mode(CTCM).The average force drop and average drop rate of SE were approximately 25%,and 80%,*** results of this study can inspire the design and mechanism of a TBM assisted by a high-pressure waterjet.
