Microsurgical treatment is well established as the preferred strategy for definitive obliteration of middle cerebral artery (MCA) aneurysms. However, increasing reports on the feasibility and efficacy of endovascular treatment of MCA aneurysms in large case series suggest coiling as a viable alternative to microsurgery. This review provides a critical overview of the current literature regarding MCA aneurysm treatment, with the objective to clarify the available evidence of efficacy with microsurgical compared to endovascular treatment.

Introduction Internal carotid artery termination(ICAT)and proximal A1 aneurysms can be challenging for open surgical clipping or endovascular *** with flow diversion covering the middle cerebral artery(MCA),an end vessel supplying a terminal circulation,has not been *** A prospective,Institutional Review Board-approved database was analysed for patients with pipeline embolisation device(PED)placement from the anterior cerebral artery(ACA)to the ICA during cerebral aneurysm *** Nine cases were identified,including five proximal A1,three posterior communicating artery and one ICAT aneurysm *** aneurysm size was 8.3 mm(range 3-17),with 67%saccular and 78%*** indication for treatment was significant dome irregularity(44%),recurrence or enlargement(33%),underlying collagen vascular disorder(11%)and traumatic pseudoaneurysm(11%).Preservation of the ipsilateral ACA(with PED placed in A1)was performed when the anterior communicating artery(67%)or contralateral A1(33%)were absent on *** coiling was done in four cases(44%).There was one major stroke leading to mortality(11%)and one minor stroke(11%).Clinical follow-up was 27 months on ***-up digital subtraction angiography(average interval 15 months)showed complete aneurysm obliteration(88%)or dome occlusion with entry remnant(12%).The jailed MCA showed minimal or mild delay(primarily anterograde flow)in 75%of cases and significant delay(reliance primarily on ACA and external carotid artery collaterals)in 25%.Conclusions Covering the MCA with a flow diverting stent should be reserved for select rare *** attention to blood pressure augmentation during the periprocedural period is necessary to minimise potential ischaemic compromise.

纤维肌痛是一种风湿病,该疾病往往表现出生理上的疼痛和心理上的不适。以往研究显示,一些积极的未来认知(如,乐观)与纤维肌痛的剧烈程度存在显著相关,未来取向作为其中的一个方面,可以描述患者在压力情境下的适应力。拥有积极未来取向的病人相信他们的情况在未来会好转,并感受到更低程度的疼痛。这两者之间的关系可能受到病人应对压力的方式影响。以往研究显示,自我同情是一种重要的应对机制,它是在压力时期指向自己的内在过程,包含三个正向维度(自我友善、普遍人性感和正念)和三个负向维度(孤立、自我批判和过度认同)。本研究以287名纤维肌痛患者为被试,考察了未来取向、纤维肌疼痛强度和自我同情之间的关系,并使用bootstrapping方法研究自我同情及其各维度在未来取向和纤维肌痛之间的中介作用。使用未来取向量表(Future Orientation Scale, FOS)测量未来取向;自我同情量表(Self-Compassion Scale, SCS)测量自我同情水平;纤维肌疼痛影响问卷修订版(Revised Fibromyalgia Impact Questionaire, FIQ-R)的第三部分测量被试近一周纤维肌疼痛症状的严重程度。实验结果表明:(1)未来取向与纤维肌痛之间存在显著的负相关;与自我同情的三个正向维度之间存在显著正相关;与自我同情的三个负向维度之间存在显著负相关。(2)自我同情的六个维度中只有孤立这一负向维度具有显著的间接效应,即在未来取向和纤维肌痛之间起中介作用。(3)将自我同情纳入模型之后,未来取向对纤维肌痛的预测不再显著,说明孤立是未来取向和纤维肌痛的完全中介。本研究说明,未来取向能显著预测纤维肌痛患者的疼痛程度,并且这种关系受自我同情中的一个特定维度,孤立的完全中介作用,即未来取向积极的患者会更少的思考与他人连结的缺失,进而体验到更轻的疼痛。

We describe nNPipe for the automated analysis of morphologically diverse catalyst materials. Automated imaging routines anddirect-electron detectors have enabled the collection of large data stacks over a wide range of sample positions at high temporalresolution. Simultaneously, traditional image analysis approaches are slow and hence unsuitable for large data stacks andconsequently, researchers have progressively turned towards machine learning and deep learning approaches. Previous studiesoften detail work on morphologically uniform material systems with clearly discernible features, limited workable image sizes andtraining data that may be biased due to manual labelling. The nNPipe data-processing method consists of two standaloneconvolutional neural networks that were exclusively trained on multislice image simulations and enables fast analysis of2048 × 2048 pixel images. Inference performance compared between idealised and real industrial catalytic samples and insightsderived from subsequent data analysis are placed into the context of an automated imaging scenario.

Plant thioglucosidases are the only known S-glycosidases in the large superfamily of glycosidases. These enzymes evolved more recently and are distributed mainly in Brassicales. Thioglucosidase research has focused mainly on the cruciferous crops due to their economic importance and cancer preventive benefits. In this study, we cloned a novel myrosinase gene, CpTGG1, from Carica papaya Linnaeus. and showed that it was expressed in the aboveground tissues in planta. The recombinant CpTGG1 expressed in Pichia pastoris catalyzed the hydrolysis of both sinigrin and glucotropaeolin （the only thioglucoside present in papaya）, showing that CpTGG1 was indeed a functional myrosinase gene. Sequence alignment analysis indicated that CpTGG1 contained all the motifs conserved in functional myrosinases from crucifers, except for two aglycon-binding motifs, suggesting substrate priority varia- tion of the non-cruciferous myrosinases. Using sinigrin as substrate, the apparent Km and Vmax values of recombinant CpTGG1 were 2.82 mM and 59.9 μmoI min-1 mg protein-1, respectively. The KcatlKm value was 23 s-1 mM-1, O-β-glucosidase activity towards a variety of substrates were tested, CpTGG1 displayed substrate-dependent and ascorbic acid-independent O-β-glucosidase activity towards 2-nitrophenyl-β- D-glucopyranoside and 4-nitrophenyl-β-D-glucopyranoside, but was inactive towards glucovanillin and n-octyl-β-D-glucopyranoside. Phylogenetic analysis indicated CpTGG1 belongs to the MYR II subfamily of myrosinases.
