Beta-blocker therapy in elderly patients with renal dysfunction and heart failure

OBJECTIVE To assess the role of beta-blockers(BB)in patients with chronic kidney disease(CKD)aged≥75 *** AND RESULTS From January 2008 to July 2014,we included 390 consecutive patients≥75 years of age with ejection fraction≤35%and glomerular filtration rate(GFR)≤60 m L/min per 1.73 m^*** analyzed the relationship between treatment with BB and mortality or cardiovascular *** mean age of our population was 82.6±4.1 *** ejection fraction was 27.9%±6.5%.GFR was 60-45 m L/min per 1.73 m^2 in 50.3%of patients,45-30 m L/min per 1.73 m^2 in 37.4%,andzation for heart failure).BB use was significantly associated with a reduced risk of death(HR=0.51,95%CI:0.35-0.74;P<0.001).Patients receiving BB consistently showed a reduced risk of death across the different stages of CKD:stage IIIa(GFR=30-45 m L/min per 1.73 m^2;HR=0.47,95%CI:0.26-0.86,P<0.0001),stage IIIb(GFR 30-45 m L/min per 1.73 m^2;HR=0.55,95%CI:0.26-1.06,P=0.007),and stages IV and V(GFR<30 m L/min per 1.73 m~2;HR=0.29,95%CI:0.11-0.76;P=0.047).CONCLUSIONS The use of BB in elderly patients with HFr EF and renal impairment was associated with a better *** of BB should be encouraged when possible.

Reconstruction of debris-flow activity in a temperate mountain forest catchment of central Mexico

The use of dendrochronology to study and date geomorphic processes in volcanic environments is still incipient, even more so on the volcanic slopes covered by temperate forests in central Mexico. Mass movements, such as debris flows, often impact forest stands where they cause damage to individual trees, thereby generating growth disturbances(GD) in the tree-ring records. The identification and dating of GD enables reconstruction of the age of trees colonizing bare surfaces after major events, but also allows the assessment of the frequency or spatial distribution of past geomorphic process activity. Here we used increment cores from 65 Pinus leiophylla, Abies religiosa, and Alnus jorullensis trees growing in the Axal gorge, on the southern slopes of La Malinche volcano, to unravel past debris-flow activity both temporally and spatially. Based on the combination of GD records, a weighted tree response index(Wit), field evidence and hydrometeorological records, we reconstructed 23 debris flows since ***, almost two-thirds of the reconstructed years with debris-flow activity in Axal gorge match with events recorded in Axaltzintle gorge located on the NE slopes of La Malinche. These findings suggest a regional triggering mechanism, most likely related to the occurrence of hurricanes. This research could be useful for disaster risk management of the La Malinche National Park.

AIM:To test a strategy for endoscopic submucosal dissection(ESD) training in animal models designed to overcome the initial learning ***:ESD was attempted in ex vivo and in vivo pig *** ESD procedures were attempted in the esophagus(n=9) or the stomach(n=21).The ex vivo model was used until initial competence was *** the in vivo model,several ESD procedures were performed in up to 3 *** following variables were analyzed:specimen size,complete and en bloc resection rate,time for circumferential incision,time for submucosal dissection,total ESD duration,and ***:Complete resection was achieved in 28 cases(en bloc 27);2 could not be completed(one perforation,one technical diff iculty).The mean ± SD time for circumferential incision was 36.2±16.8 min(range:8-87 min),and the mean±SD time for submucosal dissection was 45.1±35.7 min(range:9-196 min).The mean±SD size of the resected specimens was 45.2±17.8 *** mean±SD total resection time was signif icantly increased for the gastric cases performed in the f irst half of the study(n=13) than in the second half(n=8)(98.9±62.4 min vs 61.7±17.6 min,P=0.04),although the specimen size did not ***:Training in animal models could help endoscopists overcome the learning curve before starting ESD in humans.

Deposition of the H2A.z histone variant by the SWR1 complex （SWRI-C） in regulatory regions of specific loci modulates transcription. Characterization of mutations in Arabidopsis thaliana homologs of yeast SWRI-C has revealed a role for H2A.z exchange in a variety of developmental processes. Nevertheless, the exact composition of plant SWRI-C and how it is recruited to target genes remains to be established. Here we show that SWC4, the Arabidopsis homolog of yeast SANT domain protein Swc4/Eaf2, is a DNA-binding protein that interacts with SWR1-C subunits. We demonstrate that the swc4-1 knockout mutant is embryo- lethal, while SWC4 RNAi knockdown lines display pleiotropic phenotypic alterations in vegetative and repro- ductive traits, including acceleration of flowering time, indicating that SWC4 controls post-embryonic processes. Transcriptomic analyses and genome-wide profiling of H2A.z indicate that SWC4 represses tran- scription of a number of genes, including the floral integrator FT and key transcription factors, mainly by modulating H2A.z deposition. Interestingly, SWC4 silencing does not affect H2A.z deposition at the FLC locus nor expression of this gene, a master regulator of flowering previously shown to be controlled by SWR1-C. Importantly, we find that SWC4 recognizes specific AT-rich DNA elements in the chromatin regions of target genes and that SWC4 silencing impairs SWRI-C binding at FT. Collectively, our data suggest that SWC4 regulates plant growth and development by aiding SWR1-C recruitment and modulating H2A.z deposition.

Transportation Safety Requirements in the Use of Pushchairs on Buses

The safe use of buses by passengers travelling with pushchairs for young children is a mobility problem that has not yet been satisfactorily resolved. The lack of national and international standards in this area in the past led to the development of a research project developed jointly by the Universitat Politecnica Valencia and the Universidad Politecnica Madrid （Spain）. This paper shows a comparative study to evaluate the dynamic behaviour in case of low-g accidents reproducing frontal impacts resembling real traffic events （deceleration 2g, AV 20 km/h） of a new pushchair restraint system against the use of back-restraint by pushchairs as establishes R 107- 06 series of amendments. In the dynamic trials, three pushchair restraint prototypes and a typical wheelchair back-restraint system combined with two worst-case up-to-date pushchair models in misuse and correct use configurations were tested. The results demonstrated the need for preventing children injuries as a consequence of low-g accidents. The new design meets and improves the latest revision of regulation R 107 regarding the use of restraint systems for the transport of wheelchair and pushchair passengers and represents an advance for pushchairs travelling on buses.

With defined culture protocol, human embryonic stem cells （hESCs） are able to generate cardiomyocytes in vitro, therefore providing a great model for human heart development, and holding great potential for car- diac disease therapies. In this study, we successfully generated a highly pure population of human cardio- myocytes （hCMs）（〉95% cTnT＋） from hESC line, which enabled us to identify and characterize an hCM-specific signature, at both the gene expression and DNA meth- ylation levels. Gene functional association network and gene-disease network analyses of these hCM-enriched genes provide new insights into the mechanisms of hCM transcriptional regulation, and stand as an informative and rich resource for investigating cardiac gene func- tions and disease mechanisms. Moreover, we show that cardiac-structural genes and cardiac-transcription fac- tors have distinct epigenetic mechanisms to regulate their gene expression, providing a better understandingof how the epigenetic machinery coordinates to regulate gene expression in different cell types.

Determinants of percutaneous coronary intervention success in repeat chronic total occlusion procedures following an initial failed attempt

AIM To investigate the rates and determinants of success of repeat percutaneous coronary intervention(PCI) following an initial failed attempt at recanalising the chronic total occlusions(CTO) *** In 445 consecutive first attempt CTO-PCI procedures in our institution,procedural failure occurred in 149(33.5%). Sixty-four re-PCI procedures were performed in 58 patients(39%) all had a single CTO. Procedural and outcome data in the re-PCI population was entered into the institutional database. A retrospective analysis of clinical,angiographic and procedural data was performed. RESULTS Procedural success was achieved in 41(64%) procedures. Univariate analysis of clinical and angiographic characteristics showed that re-PCI success was associated with intravascular ultrasound(IVUS) guidance(19.5% vs 0%,P = 0.042),while failure was associated with severecalcification(30.4% vs 9.7%,P = 0.047) and a JCTO score > 3(56.5% vs 17.1% P = 0.003). Following multiple regression analysis the degree of lesion complexity(J-CTO score > 3),IVUS use,involvement of an experienced CTO operator and LAD CTO location were significant predictors of successful re-PCI. Overall the complication rate was low,with the only MACCE two periprocedural MI's neither of which required intervention. CONCLUSION Re-PCI substantially increases the overall success rate of CTO revascularization. Predictors of re-PCI success included the use of IVUS,the involvement of an experienced CTO operator in the repeat attempt and the location of the CTO.

Terahertz Modulation of Hexagonal-Shaped Metamaterials Employing znO Quantum Dots

The utilization of UV excitation to verify the terahertz (THz) wave modulation of hexagonal-shaped metamaterial (MM) arrays coated with synthesized photoluminescent, down-shifting znO quantum dots (QDs) of two different radius sizes, namely, 3.00 nm (pH 10) and 2.12 nm (pH 12), respectively is reported. In order to characterize the behavior of the MM before and after deployment of the znO QDs, THz time domain spectroscopy in transmission mode was employed. Upon exposure to UV excitation, the collected amplitude modulation values were 9.21% for the pH 12 and 4.55% for the pH 10 znO QDs, respectively. It is anticipated that the ability to actively tune the performance of otherwise passive structures will promote the proliferation of THz signal modulation devices in the near future.

Outlining key inflammation-associated parameters during early phase of an experimental gram-negative sepsis model in rhesus macaques(Macaca mulatta)

The aim of this study was to identify inflammation-associated markers during the early phase of sepsis in rhesus macaque. Four rhesus macaques were given an intravenous dose of 1010 CFU/kg of E. coli. Blood samples were collected before, or 30 minutes, 2, 4, 6 and 8 hours after E. coli infusion. Physiological parameters, bacteremia, endotoxemia, C-reactive protein(CRP), procalcitonin(PCT), and plasma cytokines/chemokines were determined for each animal. Bacteremia was present in all animals from 30 minutes to 3 hours after E. coli infusion whereas endotoxin was detected during the full-time course. CRP and PCT levels remained at detectable levels during the whole experimental window suggesting an ongoing inflammatory process. Signature cytokines and chemokines such as TNF-α, MIP-1α, and MIP-1β peaked about 2 hours after E. coli infusion and decreased thereafter. Plasma IL-6, IL-12 p40, IFN-γ, and IL-1 Ra, as well as I-TAC, MIG, IP-10 and MCP-1, remained at detectable levels after 4 hours of E. coli infusion. This nonhuman primate model could be useful for the assessment of new therapeutics aiming to suppress key inflammatory markers throughout sepsis early phases.

Breast cancer is the number one cause of can- cer deaths among Hispanic women in the United States, and in Mexico, it recently became the primary cause of cancer deaths. This malign- nancy represents a poorly understood and un- derstudied disease in Hispanic women. The ELLA Binational Breast Cancer Study was es- tablished in 2006 as a multi-center study to as- sess patterns of breast tumor markers, clinical characteristics, and their risk factors in women of Mexican descent. We describe the design and implementation of the ELLA Study and provide a risk factor comparison between women in the U.S. and those in Mexico based on a sample of 765 patients (364 in the U.S. and 401 in Mexico). Compared to women in Mexico, U.S. women had significantly (p < 0.05) lower parity (3.2 vs. 3.9 mean live births) and breastfeeding rates (57.5% vs. 80.5%), higher use of oral contraceptives (60.7% vs. 50.1%) and hormone replacement therapy (23.3% vs. 7.6%), and higher family history of breast cancer (15.7% vs. 9.0%). Re- sults show that differences in breast cancer risk factor patterns exist between Mexico and U.S. women. We provide lessons learned from the conduct of our study. Binational studies are an important step in understanding disease pat- terns and etiology for women in both countries.
