Caregiver Burden in the Management of Frail Elderly Patients with Diabetes in Internal Medicine

Aim: Advanced age and fragility often lead to dependence, making caregiver assistance necessary for performing the activities of daily living. Patients with diabetes are characterized by an elevated presence of comorbidities and subsequent frailty—a relevant consideration when making decisions regarding treatment goals. Health professionals may overlook the caregiver experience when assessing the circumstances surrounding frail elderly patients with diabetes, but this factor can have a large impact on relevant caregiver and patient outcomes. Methods: An observational, retrospective study was carried out to assess the impact of improved management in diabetes on caregiver burden. We assessed patients’ performance of activities of daily living using the Barthel scale. Our primary outcome was caregiver burden, as assessed using the Zarit scale. Results: At baseline, 81.4% of the caregivers reported severe overburden (>55 points on the Zarit scale). At three months, the proportion of caregivers reporting this level of burden had dropped to 76.7% and remained stable at six months (77.1%). In contrast, 8.5% of the caregivers reported moderate overburden (47 to 55 points) at baseline, compared to 13.3% at three months and 12.5% at six. The proportion of caregivers reporting no overburden (Conclusions: Caregiver burden remained persistently high, decreasing only slightly after six months.

An in vitro gas production （GP） technique was used to investigate the effects of combining different doses of Salix babylonica extract （SB） with exogenous fibrolytic enzymes （EZ） based on xylanase （X） and cellulase （C）, or their mixture （XC; 1：1 v/v） on in vitro fermentation characteristics of a total mixed ration of corn silage and concentrate mixture （50：50, w/w） as substrate. Four levels of SB （0, 0.6, 1.2 and 1.8 ml g-1 dry matter （DM）） and four supplemental styles of EZ （1 μl g-1 DM; control （no enzymes）, X, C and XC （1：1, v/v） were used in a 4×4 factorial arrangement. In vitro GP （ml g-1 DM） were recorded at 2, 4, 6, 8, 10, 12, 24, 36, 48 and 72 h of incubation. After 72 h, the incubation process was stopped and supernatant pH was determined, and then filtered to determine dry matter degradability （DMD）. Fermentation parameters, such as the 24 h gas yield （GY24）, in vitro organic matter digestibility （OMD）, metabolizable energy （ME）, short chain fatty acid concentrations （SCFA）, and microbial crude protein production （MCP） were also estimated. Results indicated that there was a SBxEZ interaction （P〈0.0001） for the asymptotic gas production （b）, the rate of gas production （c）, GP from 6 to 72 h, GP2 （P=0.0095）, and GP4 （P=0.02）. The SB and different combination of enzymes supplementation influenced （P〈0.001） in vitro GP parameters after 12 h of incubation; the highest doses of SB （i.e., 1.8 ml g-1 DM）, in the absence of any EZ, quadratically increased （P〈0.05） the initial delay before GP begins （l） and GP at different incubation times, with lowering b （quadratic effect, P〈0.0001 ） and c （quadratic effect, P〈0.0001 ; linear effect, P=0.0018）. The GP was the lowest （P〈0.05） when the highest SB level was combined with cellulose. There were SBxEZ interactions （P〈0.001） for OMD, ME, the partitioning factor at 72 h of incubation （PF72）, GY24, SCFA, MCP （P=0.0143）, and pH （P=0.0008）. Floods affect more people globally than any other type of natural hazard. Great potential exists for new technologies to support flood disaster risk reduction. In addition to existing expert-based data collection and analysis, direct input from communities and citizens across the globe may also be used to monitor, validate, and reduce flood risk. New technologies have already been proven to effectively aid in humanitarian response and recovery. However, while ex-ante technologies are increasingly utilized to collect information on exposure, efforts directed towards assessing and monitoring hazards and vulnerability remain limited. Hazard model validation and social vulnerability assessment deserve particular attention. New technologies offer great potential for engaging people and facilitating the coproduction of knowledge.

AIM:To evaluates the effectiveness and safety of the first generation,NS3/4A protease inhibitors(PIs) in clinical practice against chronic C virus,especially in patients with advanced fibrosis. METHODS:Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis C genotype 1,treatment-na?ve(TN) or treatment-experienced(TE),who underwent triple therapy with the first generation NS3/4A protease inhibitors,boceprevir(BOC) and telaprevir(TVR),in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow ***:One thousand and fifty seven patients were included,405(38%) were treated with BOC and 652(62%) with TVR. Of this total,30%(n = 319) were TN and the remaining were TE:28%(n = 298) relapsers,12%(n = 123) partial responders(PR),25%(n = 260) null-responders(NR) and for 5%(n = 57) with prior response unknown. The rate of sustained virologic response(SVR) by intention-to-treatment(ITT) was greater in those treated with TVR(65%) than in those treated with BOC(52%)(P lysis by groups,the TN patients treated with TVR by ITT showed a higher SVR(P = 0.005). However,by m ITT there were no significant differences between BOC and TVR. In the multivariate analysis by m ITT,the significant SVR factors were relapsers,Il28 B CC and non-F4; the type of treatment(BOC or TVR) was not significant. Assessment of Present and Future Radioactive Contamination at Global Scale

As a result of more than two thousand nuclear tests performed since 1945, two war actions and few accidents in nuclear reactor, it does already exist a radioactive contamination at global scale. This contamination has been accumulated mainly in marine sediments, because sea is about 80% of planet surface, and solid fission products released by nuclear explosions are transported by wind first and then by rain to liquid and solid portions on earth, while gaseous fission products are diffused in the atmosphere. In this way, heavy, high yielding fission products, such as 137Cs, get marine sediments, where they are found mixed with heavy radioactive natural elements such as U, Th and their also radioactive decay products. But since alkaline are so abundant on earth, it is also found natural radioactive isotope 4~K, 0.0118% of isotopes forming K element, with half life 1.28 ~ l09, in the time range of planet age and elements origin. So, one easy way to assess both the importance and evaluation of radioactive contamination at global scale should be to establish a RCF （radioactive contamination factor） as a percentage of 137Cs contaminant radioactivity in marine sediments, compared to 4~K natural radioactivity present from the very beginning of earth, both expressed as Bq per gram of sediment. In the frame of these rather general considerations, a research project has started in Mexico whose varied purposes are： to characterize sea regions in both vast littorals in the country, by its natural 4~K radioactivity present in sea salts, as well as to determine if 137Cs traces produced by recent radioactive contamination have reached their coasts. Also, to assess the 137Cs traces already present in marine sediments as a result of more than two thousand nuclear test explosions performed till now, rather than contamination produced by four main accidents in nuclear reactors during the last 60 years. Processor for Measuring Radio Network Design Quality

In this paper we present the design and prototyping of an arithmetic processor based on reconfigurable technology, whose purpose is to determine in a parallel manner the quality of the solution in a radio network design optimization problem. This problem consists in the search for an optimal set of locations in which to place radio antennas in order to obtain the maximum possible coverage, for a given terrain and antenna characteristics. The original computational contribution of this work is to use programmable logic devices to avoid the high cost of computing the evolutionary algorithms required to tackle this optimization problem. This is achieved by means of reconfigurable processors working in parallel. On the basis of the results obtained from the prototype, it may be considered a parallel architecture capable of achieving a great acceleration in the calculations.

Objective To determine whether topical applications of thiosulfinate-enriched Allium sativum extract(TASE)can accelerate acute cutaneous wound healing(WH)in a murine *** Keratinocyte viability and in vitro wound closure were assessed in keratinocyte *** of topical TASE(0.5μg/ml of allicin in 97%ethanol)on acute cutaneous WH were determined in a murine model of acute cutaneous *** mice were alternately assigned to the vehicle-and TASE-treated groups(n=6 per group).Expression levels of mRNA for keratinocyte differentiation marker-related proteins(filaggrin,loricrin and involucrin)and lipid synthetic enzymes(elongation of very long chain fatty acids protein 4(ElOVl4),fatty acid synthase(FA2H),3-hydroxy-3-methyl-glutaryl-coenzyme A reductase(HMGCoA),and serine palmitoyltransferase(SPT))were assessed using real-time quantitative polymerase chain reaction on day 3 and 8 after wounding,while transepidermal water loss(TEWl)rates were measured in wounded *** TASE accelerated WH both in vivo(40%vs.22%reduction in wound area,Pl applications of TASE upregulated the expression levels of epidermal mRNA for ElOVl4,HMGCoA,SPT,filaggrin,loricrin and involucrin(Pls)on day 3 after ***,TASE significantly lowered TEWl rates in comparison with vehicle alone on day 8(33.06±2.09 g/(m^2·h)vs.24.60±2.04 g/(m^2·h),Plusions Topical applications of TASE stimulated keratinocyte proliferation and formation of epidermal permeability barrier function,leading to acceleration of acute cutaneous *** products containing TASE could be used to manage acute cutaneous WH.

Comparison of Electric load Forecasting between Using SOM and MlP Neural Network

Electric load forecasting has been a major area of research in the last decade since the production of accurate short-term forecasts for electricity loads has proven to be a key to success for many of the decision makers in the energy sector, from power generation to operation of the system. The objective of this research is to analyze the capacity of the MlP （multilayer perceptron neural network） versus SOM （self-organizing map neural network） for short-term load forecasting. The MlP is one of the most commonly used networks. It can be used for classification problems, model construction, series forecasting and discrete control. On the other hand, the SOM is a type of artificial neural network that is trained using unsupervised data to produce a low-dimensional, discretized representation of an input space of training samples in a cell map. Historical data of real global load demand were used for the research. Both neural models provide good prediction results, but the results obtained with the SOM maps are markedly better Also the main advantage of SOM maps is that they reach good results as a network unsupervised. It is much easier to train and interpret the results.

Solid Particle Erosion of AISI 304 SS Caused by Alumina Particles

This research work was carried out with the aim of continuing to expand knowledge on the behaviour of AISI 304 stainless steel against solid particle erosion. In this particular case, the steel was subjected to the impact of alumina particles, which are hard abrasives with irregular and angular shapes. Different characterization techniques were applied to gain a better understanding of alumina. For instance, particle size distribution was obtained using the Analysette 28 Image Sizer and the particle size was between 300 - 400 µm. SEM and EDS analysis were used to know the morphology and chemical composition of both the abrasive particles and AISI 304 stainless steel. Additionally, mechanical properties values such as the hardness and Young’s modulus of AISI 304 steel were attained using a Berkovich indenter (model TTX-NHT, CSM Instruments). On the other hand, two tests were carried out for each impact angle used, 30˚, 45˚, 60˚ and 90˚, with a particle velocity of 24 ± 2 m/s and an abrasive flow rate of 63 ± 0.5 g/min, employing a test rig based on ASTM G76-95 standard. SEM images using two detectors, Backscattered Electron Detector (BED) and low Electron Detector (lED), were employed to identify the wear mechanisms on the AISI 304 eroded surfaces at 30˚ and 90˚. Finally, the erosion rates of AISI 304 compared to those results reached by AISI 1018 steel and AISI 420 stainless steel tested under identical conditions in previous works.
