A meta-analysis of 1-year outcomes of transcatheter versus surgical aortic valve replacement in low-risk patients with severe aortic stenosis

Background Transcatheter aortic valve replacement(TAVR)for the treatment symptomatic severe aortic stenosis(AS)is indicated in patients with intermediate or higher surgical *** trials showed TAVR,and surgical aortic valve replacement(SAVR)perform similarly at 1-year for the composite outcomes of mortality,stroke and *** performed a comprehensive meta-analysis to compare individual outcomes at 1-year for TAVR compared to SAVR in low-risk *** Pub Med,Embase,and cochrane central were searched for all the randomized controlled trials(RcTs)that reported 1-year comparative outcomes of TAVR and surgical aortic valve replacement(SAVR).Our conclusions are based upon the random-effects model using Der Simonian-Laird *** Data from 4 trials and 2887 randomized patients showed that TAVR had lower rates of all-cause mortality,cardiovascular mortality,and atrial fibrillation compared to SAVR at 1-year follow-up(Pciated with a significantly higher risk of permanent pacemaker implantation and moderate-severe paravalvular leak(Pconclusions The latest randomised trial data demonstrates that in short-term,TAVR is safe and effective in reducing all-cause mortality or *** follow-up of RcTs is needed to determine the durability of clinical benefits in TAVR over SAVR in low-risk patients.

Post-endoscopic procedure satisfaction scores: can we improve?

AIM To organize post-procedure satisfaction data into a useful reference and analyze patient-centered parameters to find trends that influence patient *** A robust database of two cohorts of outpatients that underwent an endoscopic procedure at Georgetown University Hospital at two separate three-month intervals ranging from November 2012 to January 2013 and November 2015 to January 2016 was compiled. Time of year was identical to control for weather/seasonal issues that may have contributed to the patient experience. The variables recorded included age, sex, body mass index(BMI), type of procedure, indication for procedure, time of the procedure, length of the procedure, type of prep used, endoscopist, satisfactory score, and comments/reasons for score. For continuous variables, differences in averages were tested by two sample t-test, Wilcoxon rank sum test, and ANOVA as appropriate. For categorical variables, differences in proportions between two groups were tested by χ~2 test. correlation test and linear regression analyses were conducted to examine relationships between length of procedure and continuous predictors. A P value cate statistically significant *** The primary outcome of this study was to assess if telephone outreach after an endoscopic intervention was a satisfactory method of obtaining post-procedure satisfaction scores from patients at a tertiary care center. Acute exercise is associated with specific executive functions in college students with ADHD:A preliminary study

Purpose： The relationship between acute exercise and executive functions in college students with attention deficit hyperactivity disorder （ADHD） has not been clearly established. The purpose of this preliminary study was to examine the difference in cognitive performance between college students with and without ADHD and to explore the effects of acute exercise on multiple aspects of executive functions and on serum brain derived neurotrophic factor （BDNF）. Methods： college students （normal： n = 10; ADHD： n = 10） performed the Stroop Test, Trail Making Test, and Digit Span Test prior to and after an acute exercise intervention. Blood samples were obtained prior to the pre-test cognitive test performance and then again after exercise and prior to the post-test cognitive test performance. Results： Students with ADHD exhibited impaired executive functions, particularly on inhibition. Additionally, while acute exercise improved all aspects of executive functions in those without ADHD, acute exercise only improved inhibitory performance for those with ADHD. Further, BDNF was not influenced by acute exercise regardless of the subjects＇ ADHD status. conclusion： These results provide preliminary evidence for exercise as a potential adjunct treatment for benefitting inhibition in college students with ADHD.

Development of Maize-Tigernut Fortified Weaning Food Using Starter cultures

The effects of starter fermentation on the nutritional qualities of maize-tigernut fortified weaning foods were investigated. The dry-milled, malted maize grains fortified with dry-milled roasted tigernut flours (70:30) were subjected to 48 hrs of spontaneous and starter fermentation (singly and as combined starters). Nutritional, sensory characteristics and feeding trials of the weaning foods were evaluated. Four fermented weaning blends were formulated: FMT (spontaneously fermented maize-tigernut), SFMT1 (Lactobacillus plantarum F2c fermented maize-tigernut), SFMT2 (Lactobacillus plantarum U2A fermented maize-tigernut) and SFMT3 (combined starter-fermented maize-tigernut). SFMT2 had the highest crude protein, fat, fibre, ash and least carbohydrate content among the blends. The highest energy content (456.84 Kcal/100 g) was observed in blend SMFT2 which was also higher than that of both negative (Nutrend) and positive (conventional animal feed) controls. The least antinutrient and vitamin contents were recorded in SFMT2. Blend SFMT1 had the highest Vitamin B1 (0.67 mg/100 g), Vitamin A (472.60 ug/100 g), phosphorus (75.45 mg/100 g) and zinc (1.05 mg/100 g) contents while the highest calcium (17.17 mg/100 g) and iron (22.82 mg/100 g) were recorded in SFMT2. Sample SFMT2 was rated the highest in all of the sensory characteristics except colour and the highest overall acceptability (6.00) which was not different significantly from all other starter produce blends. Biological evaluation showed blend SFMT2 fed animals having the highest weight by 28 days (73.14 g), mean weight gain (5.46 g), mean feed intake (18.71 g) and mean protein efficiency ratio PER (3.65). However, all the PER values including that of controls (2.30) were higher than the value of 2.10 recommended by the Protein Advisory Group (PAG) for complementary foods. As anconversion of essential cofactor for lipid biosynthesisand antioxidant defense, reduced nicotinamideadenine dinucleotide phosphate (NADPH) isproduced via various pathways, including the oxidativepentose phosphate pathway (oxPPP) and themalic enzyme 1 (ME1)-catalyzed conversion of malateto pyruvate. Live-cell detection of NADPH productionroutes remains challenging. Here, we reporttracing hydrides into lipid droplets (THILD), achemical imaging strategy for the detection ofpathway-specific NADPH generation in live cells. Thisstrategy exploits deuterium (2H)-labeled glucose([2H]Glc) tracers that transfer deuterides to NADPHvia specific pathways. The NADP^(2)H, in turn, transfersdeuterides to lipids, resulting in accumulation of c-2Hbonds in lipid droplets, which can be visualized bybioorthogonal stimulated Raman scattering (SRS)microscopy. We used this concept to demonstratethe imaging of oxPPP-produced NADPH using theoxPPP-specific tracer, [3-2H]Glc. Furthermore, the“switch on” of NADPH production by ME1 in differentiatingadipocytes was imaged by [4-2H]Glc. Finally,comparison of [3-2H]Glc and [4-2H]Glc THILDimaging of adipocytes showed that hypoxia inducessuppression of ME1-mediated NADPH productionand oxPPP-produced NADPH becomes the mainsource.

Trends of seasonal forage yield changes of triticale in the southern Great Plains of the United States

Blood glucose changes surrounding initiation of tumor-necrosis factor inhibitors and conventional disease-modifying anti-rheumatic drugs in veterans with rheumatoid arthritis

AIM To determine the scope of acute hypoglycemic effects for certain anti-rheumatic medications in a large retrospective observational study. METHODS Patients enrolled in the Veterans Affairs Rheumatoid Arthritis (VARA) registry were selected who, during follow-up, initiated treatment with tumor necrosis factor inhibitors (TNFi's, including etanercept, adalimumab, infliximab, golimumab, or certolizumab), prednisone, or conventional disease-modifying anti-rheumatic drugs(DMARDs), and for whom proximate random blood glucose (RBG) measurements were available within a window 2-wk prior to, and 6 mo following, medication initiation. Similar data were obtained for patients with proximate values available for glycosylated hemoglobin A1c values within a window 2 mo preceding, and 12 mo following, medication initiation. RBG and A1c measurements were compared before and after initiation events using paired t-tests, and multivariate regression analysis was performed including established comorbidities and *** Two thousands one hundred and eleven patients contributed at least one proximate measurement surrounding the initiation of any examined medication. A significant decrease in RBG was noted surrounding 653 individual hydroxychloroquine-initiation events(-3.68 mg/dL, P = 0.04), while an increase was noted for RBG surrounding 665 prednisone-initiation events(+5.85 mg/d L, P cally significant decrease in A1c was noted for sulfasalazine initiation, as measured by 49 individual initiation events(-0.70%, P chloroquine(β =-5.78, P = 0.01) use as predictors of lower post-medicationinitiation RBG and A1c values, respectively. Analysis by drug class suggested prednisone (or glucocorticoids) as predictive of higher medication-initiation event RBG among all start events as compared to DMARDs, while this analysis did not
