Patterns,procedures,and indications for pediatric surgery in a Tanzanian Refugee Camp:a 20-year experience

Background There are 103 million displaced people worldwide,41%of whom are *** on the provision of surgery in humanitarian settings are *** scarcer is literature on pediatric surgery performed in humanitarian settings,particularly protracted humanitarian *** We reviewed patterns,procedures,and indications for pediatric surgery among children in Nyarugusu Refugee Camp using a 20-year retrospective *** A total of 1221 pediatric surgical procedures were performed over the study *** between the ages of 12 and 17 years were the most common age group undergoing surgery(n=991;81%).A quarter of the procedures were performed on local Tanzanian children seeking care in the camp(n=301;25%).The most common procedures performed were cesarean sections(n=858;70%),herniorrhaphies(n=197;16%),and exploratory laparotomies(n=55;5%).Refugees were more likely to undergo exploratory laparotomy(n=47;5%)than Tanzanian children(n=7;2%;p=0.032).The most common indications for exploratory laparotomy were acute abdomen(n=24;44%),intestinal obstruction(n=10;18%),and peritonitis(n=9;16%).Conclusions There is a significant volume of basic pediatric general surgery performed in the Nyarugusu *** are used by both refugees and local *** hope this research will inspire further advocacy and research on pediatric surgical services in humanitarian settings worldwide and illuminate the need for including pediatric refugee surgery within the growing global surgery movement.

Ultrafast 3D imaging is indispensable for visualizing complex and dynamic biological *** scanning-based techniques necessitate an inherent trade-off between acquisition speed and space-bandwidth product(SBP).Emerging single-shot 3D wide-field techniques offer a promising alternative but are bottlenecked by the synchronous readout constraints of conventional CMOS systems,thus restricting data throughput to maintain high SBP at limited frame *** address this,we introduce EventLFM,a straightforward and cost-effective system that overcomes these challenges by integrating an event camera with Fourier light field microscopy(LFM),a state-of-theart single-shot 3D wide-field imaging *** event camera operates on a novel asynchronous readout architecture,thereby bypassing the frame rate limitations inherent to conventional CMOS *** further develop a simple and robust event-driven LFM reconstruction algorithm that can reliably reconstruct 3D dynamics from the unique spatiotemporal measurements captured by *** results demonstrate that EventLFM can robustly reconstruct fast-moving and rapidly blinking 3D fluorescent samples at kHz frame ***,we highlight EventLFM’s capability for imaging of blinking neuronal signals in scattering mouse brain tissues and 3D tracking of GFP-labeled neurons in freely moving *** believe that the combined ultrafast speed and large 3D SBP offered by EventLFM may open up new possibilities across many biomedical applications.

We present detailed characterization of laser-driven fusion and neutron production(-10^(5)/second) using 8 mJ, 40 fs laser pulses on a thin(vistic intensity(~ 5 × 10^(18)W/cm^(2))and high repetition rate(1 kHz), the system produces deuterium±deuterium(D-D) fusion, allowing for consistent neutron generation. Evidence of D-D fusion neutron production is verified by a measurement suite with three independent detection systems: an EJ-309 organic scintillator with pulse-shape discrimination, a ~3He proportional counter and a set of 36 bubble detectors. Time-of-flight analysis of the scintillator data shows the energy of the produced neutrons to be consistent with 2.45 Mev. Particle-in-cell simulations using the WarpX code support significant neutron production from D-D fusion events in the laser±target interaction region. This high-repetition-rate laser-driven neutron source could provide a low-cost, on-demand test bed for radiation hardening and imaging applications.

A novel rheumatoid arthritis(RA)synovial fluid protein,Syntenin-1,and its receptor,Syndecan-1(SDC-1),are colocalized on RA synovial tissue endothelial cells and fibroblast-like synoviocytes(FLS).Syntenin-1 exacerbates the inflammatory landscape of endothelial cells and RA FLS by upregulating transcription of IRF1/5/7/9,IL-1β,IL-6,and CCL2 through SDC-1 ligation and HIF1α,or mTOR ***,Syntenin-1 orchestrates RA FLS and endothelial cell invasion via SDC-1 and/or mTOR *** Syntenin-1 reprogrammed endothelial cells,the dynamic expression of metabolic intermediates coincides with escalated glycolysis along with unchanged oxidative factors,AMPK,PGC-1α,citrate,and inactive oxidative ***,RA FLS rewired by Syntenin-1 displayed a modest glycolytic-ATP accompanied by a robust mitochondrial-ATP *** enriched mitochondrial-ATP detected in Syntenin-1 reprogrammed RA FLS was coupled with mitochondrial fusion and fission recapitulated by escalated Mitofusin-2 and DRP1 *** found that vEGFR1/2 and Notch1 networks are responsible for the crosstalk between Syntenin-1 rewired endothelial cells and RA FLS,which are also represented in RA *** to RA explants,morphological and transcriptome studies authenticated the importance of vEGFR1/2,Notch1,RAPTOR,and HIF1αpathways in Syntenin-1 arthritic mice and their obstruction in SDC-1 deficient ***,dysregulation of SDC-1,mTOR,and HIF1αnegated Syntenin-1 inflammatory phenotype in RA explants,while inhibition of HIF1αimpaired synovial angiogenic imprint amplified by *** conclusion,since the current therapies are ineffective on Syntenin-1 and SDC-1 expression in RA synovial tissue and blood,targeting this pathway and its interconnected metabolic intermediates may provide a novel therapeutic strategy.

Metal–phthalocyanines are a class of catalytically active materials promising in energy conversion and storage fields(e.g.,electrocatalysis).However,understanding and controlling the electrochemical properties in metal-phthalocyanine systems is ***,we elucidate the electrocatalytic origins of a series of cobalt-phthalocyanine molecular catalysts and finetune their electronic properties at the atomic level,both experimentally and *** interactions between the cobalt center and the local coordination environment are regulated by introducing either electron-donating or electron-withdrawing groups on the phthalocyanine ligand,and the spin-orbit splitting of cobalt is increased by~0.15 ev compared with the nonsubstituted ***,the aminated cobalt phthalocyanine-based electrocatalysts exhibit low free energies in the ratedetermining steps of the oxygen reduction(-1.68 ev)and oxygen evolution reactions(0.37 ev).This contributes to the high electrocatalytic activity(e.g.,a halfwave potential of 0.84 v and an overpotential of 0.30 v at 10 mAcm^(-2)),featuring a high selectivity of a four-electron pathway(i.e.,a negligible by-product of hydrogen peroxide).These catalysts also exhibit exceptional kinetic current density(Tafel slope of 100 mv dec^(-1))in oxygen reduction reactions,in addition to a superior power density(158 mWcm^(-2))and a high cycling stability(>1,300 cycles)in Zn-air batteries,outperforming the commercial Pt/C and/or RuO2counterparts.
