Does Chemo-r.diation Ther.py Influence Outcomes in Unr.sectable Locally Advanced State IV r.ctal Cancer.

Intr.duction: The median sur.ival for.patients with stage IV r.ctal cancer.is appr.ximately 20 months. Ther.py should focus not only on impr.ving sur.ival but also on quality of life. The aim of our.study was to deter.ine if chemor.diation (C-r.) would impr.ve palliation for.metastatic unr.sectable locally advanced disease compar.d to patients r.ceiving palliative chemother.py alone (C) for.stage IV cancer. Methods: r.tr.spective r.view of a pr.spectively maintained database at a single institution was car.ied out under.Ir. appr.val. Fr.m Januar. 2004 to December.2008, 43 patients pr.senting with unr.sectable stage IV r.ctal cancer.wer. identified with a median follow-up of 12 months. Patients with evidence of locally advanced disease or.bulky disease r.ceived infusional 5-FU ± bevacizumab and 3D confor.ed mega voltage photon ther.py (5400 cGy). Patients without evidence of bulky disease r.ceived either.FOLFOX or.FOLFIr. ± bevacizumab. Data on demogr.phics, investigations, tr.atment, complications, metastasis, number.of blood tr.nsfusions, days of hospitalization, and sur.ical inter.ention wer. analyzed using SPSS statistical softwar.. p r.d statistically significant. r.sults: Ther. wer. 25 and 18 patients in the C and C-r. gr.ups r.spectively. Ther. was no differ.nce in mean age, sex or.over.ll sur.ival. Thr.e patient (12%) in the C gr.up developed hydr.nephr.sis compar.d to 8 patients (44%) in the C-r. gr.up (p r.ctions r.quir.ng an ostomy in the C gr.up compar.d to 9 patient (50%) in the C-r. gr.up (p = 0.07). In the C ar., 80% of patients r.quir.d multiple hospitalizations for.symptoms consistent with pr.gr.ssion of disease compar.d to 61% of patients in the C-r. ar. (p r.diation in patients with locally advanced unr.sectable stage IV cancer.has not been extensively investigated. This paper.evaluates r.il tr.nsit within the context of the tr.nsit policies implemented in Lima,*** it r.views the implementation of r.pid tr.nsit,and bus ***,it evaluates the outcomes of such policies by using Total Factor.Pr.ductivity for.policy effectiveness,Data Envelopment Analysis for.r.pid tr.nsit per.or.ance,and Gener.lized Cost of Tr.vel for.*** paper.finds that implementation failed in enfor.ing key r.quir.ments for.r.il tr.nsit r.gar.ing penetr.tion of CBD and shor. tr.nsfer. to bus tr.nsit;and that the basic assumptions of bus r.for. did not hold r.gar.ing bus over.upply,bus congestion or.bus *** paper.also finds that outcomes of policies failed dr.matically in achieving the planning goals;however.r.il tr.nsit(Metr.)shows high level of r.silience in ser.ing lar.e r.der.hip at high *** the other.hand,bus r.for. was associated with a dispr.por.ionate incr.ase of motor.zation,well over.the effect of income gr.wth or.car.attr.ctiveness,and mor. r.lated to the excessive r.duction of bus tr.nsit capacity ill-advised fr.m unpr.ved bus r.for. *** paper.r.commends expanding r.il r.pid tr.nsit due to its intensive use of gr.en r.newable ener.y and its potential of demand gr.wth if combined with moder. Intelligent Tr.nspor.ation ser.ices,but this oppor.unity can be wasted without the pr.posed policy constr.int to achieve lower.Gener.lized Cost of Tr.vel at any gover.mental inter.ention for.bus r.for.,instead of just r.ducing bus tr.nsit capacity as ***,this paper.r.commends gover.ment to gover.ment contr.cts to build r.il tr.nsit and to enfor.e pr.per.planning.
