Brucella suis is responsible for swine brucellosis worldwide. Of the five different B. suis biovars （by.）, bv. 2 appears restricted to Europe where it is frequently isolated from wild boar and hares, can infect pigs and can cause human brucellosis. In this study, the differ- ential gene expression profile was characterized in spleens of Eurasian wild boar naturally infected with B. suis bv. 2. Of the 20,201 genes analyzed in the microarray, 633 and 1,373 were significantly （fold change 〉 1.8; P 〈 0.01） upregulated and downregulated, respectively, in infected wild boar. The analysis was focused on genes that were over represented after conditional test for biological process gene ontology. Upregulated genes suggested that B. suis bv. 2 infection induced cell maturation, migration and/or proliferation in infected animals. The genes downregulated in infected wild boar impaired the activity of several important cellular metabolic pathways such as metabolism, cytoskeleton organization and biogenesis, immune response and lysosomal function and vesicle-mediated transport. In addition, the response to stress, sperm fertility, muscle development and apoptosis seemed to be also impaired in infected animals. These results suggested that B. suis bv. 2 may use strategies similar to other smooth brucellae to facilitate intracellular multiplication and the development of chronic infections. To our knowledge, this is the first report of the analysis of gene expression profile in hosts infected with B. suis bv. 2, which is important to understand the molecular mechanisms at the host-pathogen interface in the main reservoir species with possible implications in the zoonotic cycle of the pathogen.

Acute-on-chronic liver failure:Pathogenesis,prognostic factors and management

Acute-on-chronic liver failure(ACLF) is increasingly recognized as a complex syndrome that is reversiblein many cases. It is characterized by an acute deterioration of liver function in the background of a pre-existing chronic liver disease often associated with a high short-term mortality rate. Organ failure(OF) is always associated, and plays a key role in determining the course, and the outcome of the disease. The definition of ACLF remains controversial due to its overall ambiguity, with several disparate criteria among various associations dedicated to the study of liver diseases. Although the precise pathogenesis needs to be clarified, it appears that an altered host response to injury might be a contributing factor caused by immune dysfunction, ultimately leading to a pro-inflammatory status, and eventually to OF. The PIRO concept(Predisposition, Insult, Response and Organ Failure) has been proposed to better approach the underlying mechanisms. It is accepted that ACLF is a different and specific form of liver failure, where a precipitating event is always involved, even though it cannot always be ascertained. According to several studies, infections and active alcoholism often trigger ACLF. Viral hepatitis, gastrointestinal haemorrhage, or drug induced liver injury, which can also provoke the syndrome. This review mainly focuses on the physiopathology and prognostic aspects. We believe these features are essential to further understanding and providing the rationale for improveddisease management strategies.

Usefulness of Impella support in different clinical settings in cardiogenic shock

BACKGROUND The Impella pump has emerged as a promising tool in patients with cardiogenic shock(CS).Despite its attractive properties,there are scarce data on the specific clinical setting and the potential role of Impella devices in CS patients from routine clinical practice.mETHODS This is an observational,retrospective,single center,cohort study.All consecutive patients with diagnosis of CS and undergoing support with Impella 2.5?,Impella CP?or Impella 5.0?from April 2015 to December 2020 were included.Baseline characteristics,management and outcomes were assessed according to CS severity,age and cause of CS.main outcome measured was in-hospital mortality.RESULTS A total of 50 patients were included(median age:59.3±10 years).The most common cause of CS was acute coronary syndrome(ACS)(68%),followed by decompensation of previous cardiomyopathy(22%).A total of 13 patients(26%)had profound CS.most patients(54%)improved pulmonary congestion at 48 h after Impella support.A total of 19 patients(38%)presented significant bleeding.In-hospital mortality was 42%.Among patients with profound CS(n=13),five patients were previously supported with venoarterial extracorporeal membrane oxygenation.A total of eight patients(61.5%)died during the admission,and no patient achieved ventricular recovery.Older patients(≥67 years,n=10)had more comorbidities and the highest mortality(70%).Among patients with ACS(n=34),35.3%of patients had profound CS;and in most cases(52.9%),Impella support was performed as a bridge to recovery.In contrast,only one patient from the decompensated cardiomyopathy group(n=11)presented with profound CS.In 90.9%of these cases,Impella support was used as a bridge to cardiac transplantation.There were no cases of death.CONCLUSIONS In this cohort of real-life CS patients,Impella devices were used in different settings,with different clinical profiles and management.Despite a significant rate of complications,mortality was acceptable and lower than those observed in other series.

Evaluation of the iridocorneal angle with accommodation using optical coherence tomography

The changes in the iridocorneal angle structure during accommodation are assessed by means of anterior segment optical coherence tomography.Thirteen right eyes were included in the study.The device used for the measurement was the Visante？omni system.The stimuli were set up at different vergences（0.0 D,-1.5 D,and-3.0 D）.The angle opening distance 500 and 750,the trabecular iris space area 500 and 750,and the scleral spur angle parameters were assessed at the nasal and temporal regions.The results in the iridotrabecular angle comparing the three accommodative states of the eye did not yield any statistically significant difference at nasal or temporal angle sections.In light of our results and in the conditions of our study,the structures of the iridocorneal angle are not significantly changed with accommodation.

The use of adaptive optics systems in ophthalmic research studies has increased in the last years thanks to the development of more accurate devices.Adaptive optics was first introduced in Astronomy for the correction of the effects produced by the turbulences of the Earth’s atmosphere when observing astronomical objects(1,2).Sophisticated deformable mirrors controlled by computers can correct in real time atmospheric disturbances,allowing for much finer details on the images of distance objects.Adaptive optics technology makes it possible to correct many of the aberrations with which the wavefront,i.e.,the geometric envelope of all the rays of light that emerged at the same time of the observed object,arrives.Adaptive optics allows for obtaining sharper images,and thus,with better spatial resolution.

AIm: To determine the clinical reasons for conversion to everolimus(EVL) and long-term outcomes in heart transplant(HT) recipients.mETHODS: A retrospective 12-mo study has been carried out in 14 Spanish centres to assess the efficacy and safety of conversion to EVL in maintenance HT recipients.RESULTS: Two hundred and twenty-two patients were included(mean age: 53 ± 10.5 years; mean time from HT: 8.1 ± 4.5 years). The most common reasons for conversion were nephrotoxicity(30%), chronic allograft vasculopathy(20%) and neoplasms(17%). The doses and mean levels of EVL at baseline(conversion to EVL) and after one year were 1.3 ± 0.3 and 1.2 ± 0.6 mg/d and 6.4 ± 3.4 and 5.6 ± 2.5 ng/mL, respectively. The percentage of patients receiving calcineurin inhibitors(CNIs) at baseline and on the final visit was 95% and 65%, respectively. The doses and mean levels of CNIs decreased between baseline and month 12 from 142.2 ± 51.6 to 98.0 ± 39.4 mg/d(P m 126.1 ± 50.9 to 89.2 ± 47.7 ng/mL(P m 2.9 ± 1.8 to 2.6 ± 1.9 mg/d and from 8.3 ± 4.0 to 6.5 ± 2.7 ng/mL(P = 0.011) for tacrolimus. In the subgroup of patients converted because of nephrotoxicity, creatinine clearance increased from 34.9 ± 10.1 to 40.4 ± 14.4 mL/min(P most frequent adverse events were oedemas(12%), infections(9%) and gastrointestinal problems(6%). EVL was suspended in 44 patients(20%). Since the database was closed at the end of the study, no further followup data is available.CONCLUSION: Conversion to EVL in maintenance HT recipients allowed minimisation or suspension of the CNIs, with improved kidney function in the patients with nephrotoxicity, after 12 mo.

AIm:To validate a multimodal[structural and functional magnetic resonance(mR)]approach as coincidence brain clusters are hypothesized to correlate with clinical severity of auditory hallucinations.mETHODS:Twenty-two patients meeting Diagnostic and Statistical manual of mental Disorders(fourth edition,DSm-Ⅳ)criteria for schizophrenia and experiencing persistent hallucinations together with 28 healthy controls were evaluated with structural and functional mR imaging with an auditory paradigm designed to replicate those emotions related to the patients’hallucinatory experiences.Coincidence maps were obtained by combining structural maps of gray matter reduction with emotional functional increased activation.Abnormal areas were correlated with the brief psychiatric rating scale(BPRS)and the psychotic symptom rating scale(PSYRATS)scales.RESULTS:The coincidence analysis showed areas with coexistence gray matter reductions and emotional activation in bilateral middle temporal and superior temporal gyri.Significant negative correlations between BPRS and PSYRATS scales were observed.BPRS scores were negatively correlated in the middle temporal gyrus(right)(t=6.86,P=0.001),while negative PSYRATS correlation affected regions in both the superior temporal gyrus(left)(t=7.85,P=0.001)and middle temporal gyrus(left)(t=4.97,P=0.002).CONCLUSION:Our data identify left superior and middle temporal gyri as relevant areas for the understanding of auditory hallucinations in schizophrenia.The use of multimodal approaches,sharing structural and functional information,may demonstrate areas specifically linked to the severity of auditory hallucinations.
