Short-and long-term outcomes after hemihepatectomy for perihilar cholangiocarcinoma:does left or right side matter?

Background:The only potentially curative option for patients with perihilar cholangiocarcinoma(PHC)is resection,typically an extrahepatic bile duct resection in combination with(extended)liver *** such as bile leakage and liver failure have been suggested to be more common after right-sided resections compared to left-sided resections,whilst superior oncological outcomes have been reported after right-sided ***,data on outcomes after right-sided or left-sided liver resections in PHC are ***,we aimed to investigate short-and long-term outcomes after left and right hemihepatectomy in patients with ***:in this retrospective study,patients undergoing major liver resection for suspected PHC in a tertiary center between 2000-2018 were *** who had undergone left-sided resections were compared to patients with right-sided resections in terms of complications(90-day mortality,overall and severe morbidity and specific complications).For long-term outcomes,only patients with pathologically proven PHC were included in the survival ***:A total of 178 patients undergoing hemihepatectomy for suspected PHC were analysed,including 76 left-sided and 102 right-sided *** 90-day mortality was 14%(24 out of 178),with no significant difference after left-sided resection(11%;8 out of 76)versus right-sided resection(16%;16 out of 102)(P=0.319).Severe morbidity(Clavein Dindo≥3)was also comparable in both groups:54%versus 61%(P=0.361).No differences in specific complications including bile leakage were observed,although liver failure appeared to occur more frequently after right hemihepatectomy(22%versus 11%,P=0.052).Five-year overall survival for pathologically proven PHC,excluding in-hospital mortality,did not differ;43.7%after left-sided resection *** 38.2%after right-sided resection(P=0.553).Conclusions:Both short-and long-term outcomes between patients undergoing left and right hemihepate

Background: Liver regeneration is crucial to restore the functional liver mass after liver resection. The aim of this study was to evaluate the early postoperative changes in remnant liver function, volume and liver stiffness after major liver resection and their correlation with postoperative outcomes. Methods: Patients undergoing major liver resection (≥3 segments) between February and November 2018 underwent both functional assessment using technetium-99m mebrofenin hepatobiliary scintigraphy (HBS) and CT-volumetry of the (future) remnant liver on preoperative day 1, the 5th postoperative day, and 4-6 weeks after resection. At the same time points, patients underwent transient elastography (TE) for the assessment of liver stiffness. Severe postoperative complications (Clavien-Dindo ≥ 3A) and mortality were correlated with the functional and volumetric increases of the remnant liver. Liver failure was graded according to the international Study Group of Liver Surgery (iSGLS) criteria. Results: A total of 18 patients were included of whom 10 (56%) had severe complications and one patient (5%) developed liver failure. Function and volume of the remnant liver had increased by the 5th postoperative day from 6.9 (5.4-10.9) to 9.6 (6.7-13.8) %/min/m2, P=0.004 and from 795.5 (538.3-1,037.5) to 1,080.0 (854.0-1,283.3) mL, Pively. After 4-6 weeks, remnant liver volume had further increased [from 1,080.0 (854.0-1,283.3) to 1,222.0 (1,016.0-1,380.5) mL, P=0.035], however, liver function did not show any significant, further increase [from 9.6 (6.7-13.8) to 10.9 (8.8-13.6) %/min/m2, P=0.177]. Liver elasticity of the future remnant liver (FRL) increased [from 10.8 (5.7-18.7) to 17.5 (12.4-22.6) kPa, P=0.018] and gradually recovered after 4-6 weeks to a median of 10.9 (5.7-18.8) kPa (T3 vs. T4, P=0.079). introduction: Vaccination plays a pivotal role in mitigating the repercussions of the COViD-19 pandemic. However, vaccination campaigns encounter obstacles, especially in developing countries like the Democratic Republic of the Congo (DRC). This study aimed at investigating the roles of vaccine hesitancy, refusal, and access barriers, while identifying individual-level factors associated with non-vaccination in Mbujimayi, DRC. Methods: A community-based cross-sectional survey was conducted in three health districts and included 1496 residents. Attitudes and behaviors related to seeking COViD-19 vaccination were assessed using a standardized questionnaire. Hierarchical logistic regression modeling was used to assess factors potentially affecting non-compliance with vaccination. Results: Among participants (median age = 33, iQR = 23.3, M/F sex ratio = 0.7), 60% displayed misconceptions about COViD-19 or its vaccine, while only 35.2% perceived COViD-19 as a significant health threat. Vaccination coverage was estimated at 49.1% (95% Ci: 47.5;52.6), with 71.8% of vaccinated individuals having received one dose. Among the unvaccinated individuals, 50.9% expressed no intention to be vaccinated in the future, citing hesitation (30.4%) or refusal (39.6%) mainly due to side effects or distrust of vaccines. Conversely, 32.7% of the unvaccinated persons expressed access barriers despite willingness to be vaccinated. Misconceptions about COViD-19 and its vaccines were the main drivers of vaccination non-compliance. Conclusion: These findings demonstrate significant vaccine non-compliance driven by hesitancy, refusal, and access barriers. Strategies to enhance vaccination coverage and pandemic preparedness should address misconceptions, sociodemographic barriers, and geographic disparities.

Fermented liquid feed for pigs： an ancient technique for the future

Fermented liquid feed is feed that has been mixed with water at a ratio ranging from 1：1.5 to 1：4. By mixing with water lactic acid bacteria and yeasts naturally occurring in the feed proliferate and produce lactic acid, acetic acid and ethano which reduces the pH of the mixture. This reduction in pH inhibits pathogenic organisms from developing in the feed. in addition, when this low pH mixture is fed, it reduces the pH in the stomach of pigs and prevents the proliferation of pathogens such as coliforms and Salmonella in the gastrointestinal tract. For piglets, the use of fermented liquid feed offers the possibility of simultaneously providing feed and water, which may facilitate an easier transition from sow＇s milk to solid feed. Secondly, offering properly produced fermented liquid feed may strengthen the role of the stomach as the first line of defense against possible pathogenic infections by lowering the pH in the gastrointestinal tract thereby helping to exclude enteropathogens. Finally, feeding fermented liquid feed to pigs has been shown to improve the performance of suckling pigs, weaner pigs and growing-finishing pigs. in this review, current knowledge about the use of fermented liquid feed in pig diets will be discussed. This will include a discussion of the desirable properties of fermented liquid feed and factors affecting fermentation. in addition, advantages and disadvantages of fermented liquid feed will be discussed including its effects on gastrointestinal health, intestinal pH and the types of bacteria found in the gastrointestinal tract as well as the effects of fermented liquid feeds on pig performance.

BACKGROUND Preoperative biliary drainage in patients with presumed resectable perihilar cholangiocarcinoma(PHC)is hypothesized to promote the occurrence of seeding *** metastases can occur at the surgical scars or at the site of postoperative drains,and in case of percutaneous biliary drainage,at the catheter *** prevent seeding metastases after resection,we routinely treated PHC patients with preoperative radiotherapy(RT)for over 25 years until January *** To investigate the incidence of seeding metastases following resection of *** All patients who underwent resection for pathology proven PHC between January 2000 and March 2019 were included in this retrospective *** 2000-January 2018,patients received preoperative RT(3×3.5 Gray).RT was omitted in patients treated after January *** A total of 171 patients underwent resection for PHC between January 2000 and March *** 171 patients undergoing resection,111 patients(65%)were treated with preoperative *** bile cytology showed no difference in the presence of viable tumor cells in bile of patients undergoing preoperative RT or ***,two patients(1.2%)with seeding metastases were identified,both in the laparotomy scar and both after preoperative RT(one patient with endoscopic and the other with percutaneous and endoscopic biliary drainage).CONCLUSiON The incidence of seeding metastases in patients with resected PHC in our series was low(1.2%).This low incidence and the inability of providing evidence that preoperative low-dose RT prevents seeding metastases,has led us to discontinue preoperative RT in patients with resectable PHC in our center.

Finite versus small strain discrete dislocation analysis of cantilever bending of single crystals

Plastic size effects in single crystals are investigated by using finite strain and small strain discrete dislocation plasticity to analyse the response of cantilever beam specimens. Crystals with both one and two active slip systems are analysed, as well as specimens with different beam aspect ratios. Over the range of specimen sizes analysed here,the bending stress versus applied tip displacement response has a strong hardening plastic component. This hardening rate increases with decreasing specimen size. The hardening rates are slightly lower when the finite strain discrete dislocation plasticity（DDP） formulation is employed as curving of the slip planes is accounted for in the finite strain *** relaxes the back-stresses in the dislocation pile-ups and thereby reduces the hardening rate. Our calculations show that in line with the pure bending case, the bending stress in cantilever bending displays a plastic size dependence. However, unlike pure bending, the bending flow strength of the larger aspect ratio cantilever beams is appreciably *** is attributed to the fact that for the same applied bending stress, longer beams have lower shear forces acting upon them and this results in a lower density of statistically stored dislocations.

Male reproductive suppression： not a social affair

in the animal kingdom there are countless strategies via which males optimize their reproductivesuccess when faced with male-male competition. These male strategies typically fall into two maincategories： pre- and post-copulatory competition. Within these 2 categories, a set of behaviors,referred to as reproductive suppression, is known to cause inhibition of reproductive physiologyand/or reproductive behavior in an otherwise fertile individual. What becomes evident when con-sidering examples of reproductive suppression is that these strategies conventionally encompassreproductive interference strategies that occur between members of a hierarchical social ***, mechanisms aimed at impairing a competitor＇s reproductive output are also present innon-social animals. Yet, current thinking emphasizes the importance of sociality as the primarydriving force of reproductive suppression. Therefore, the question arises as to whether there is anactual difference between reproductive suppression strategies in social animals and equivalentpre-copulatory competition strategies in non-social animals. in this perspective paper we explore abroad taxonomic range of species whose individuals do not repeatedly interact with the same indi-viduals in networks and yet, depress the fitness of rivals. Examples like alteration of male repro-ductive physiology, female mimicry, rival spermatophore destruction, and cementing the rival＇sgenital region in non-social animals, highlight that male pre-copulatory reproductive suppressionand male pre-copulatory competition overlap. Finally, we highlight that a distinction between malereproductive interference in animals with and without a social hierarchy might obscure importantsimilarities and does not help to elucidate why different proximate mechanisms evolved. We there-fore emphasize that male reproductive suppression need not be restricted to social animals.

Sepsis is a systemic inflammatory response representing the leading cause of death in critically ill patients,mostly due to multiple organ *** gastrointestinal tract plays a pivotal role in the pathogenesis of sepsisinduced multiple organ failure through intestinal barrier dysfunction,bacterial translocation and *** this review we address the role of the gastrointestinal tract,the mediators,cell types and transduction pathways involved,based on experimental data obtained from models of inflammation-induced ileus and (preliminary) clinical *** complex interplay within the gastrointestinal wall between mast cells,residential macrophages and glial cells on the one hand,and neurons and smooth muscle cells on the other hand,involves intracellular signaling pathways,Toll-like receptors and a plethora of neuroactive substances such as nitric oxide,prostaglandins,cytokines,chemokines,growth factors,tryptases and *** signaling between the different components in the gastrointestinal wall,the spinal cord and central nervous system impacts inflammation and its *** propose that novel therapeutic strategies should target inflammation on the one hand and gastrointestinal motility,gas-trointestinal sensitivity and even pain signaling on the other hand,for instance by impeding afferent neuronal signaling,by activation of the vagal anti-inflammatory pathway or by the use of pharmacological agents such as ghrelin and ghrelin agonists or drugs interfering with the endocannabinoid system.

This paper reviews the use of fiber-reinforced polymers （FRPs） in architectural and structural bridge design in the Netherlands. The challenges and opportunities of this relatively new material, both for the architect and the engineer, are discussed. An inventory of recent structural solutions in FRP is included, followed by a discussion on architectural FRP applications derived from the architectural practice of the author and of other pioneers.
