Does immunohistochemical staining have a clinical impact in early gastric cancer conducted endoscopic submucosal dissection?

AIM: To evaluate clinicopathologic parameters and the clinical significance related lymphovascular invasion (LVI) by immunohistochemical staining (IHCS) in endoscopic submucosal dissection (ESD). METHODS: Between May 2005 and May 2010, a total of 348 lesions from 321 patients (mean age 63 ± 10 years, men 74.6%) with early gastric cancer (EGC) who met indication criteria after ESD were analyzed retrospectively. The 348 lesions were divided into the absolute (n = 100, differentiated mucosal cancer without ulcer ≤ 20 mm) and expanded (n = 248) indica-tion groups after ESD. The 248 lesions were divided into four subgroups according to the expanded ESD indication. The presence of LVI was determined by factor Ⅷ-related antigen and D2-40 assessment. We compared LVI IHCS-negative group with LVI IHCSpositive in each group. RESULTS: LVI by hematoxylin-eosin staining (HES) and IHCS were all negative in the absolute group, while was observed in only the expanded groups. The positive rate of LVI by IHCS was higher than that of LVI by HES (n = 1, 0.4% vs n = 11, 4.4%, P = 0.044). LVI IHCS-positivity was observed when the cancer invaded to the mucosa 3 (M3) or submucosa 1 (SM1) levels, with a predominance of 63.6% in the subgroup that included only SM1 cancer (P < 0.01). In a univariate analysis, M3 or SM1 invasion by the tumor was significantly associated with a higher rate of LVI by IHCS, but no factor was significant in a multivariate analysis. There were no cases of tumor recurrence or metastasis during the median 26 mo follow-up. CONCLUSION: EGCs of the absolute group are immunohistochemically stable. The presence of LVI may be carefully examined by IHCS in an ESD expanded indication group with an invasion depth of M3 or greater.

Iodized oil uptake assessment with cone-beam CT in chemoembolization of small hepatocellular carcinomas

AIM:To evaluate the utility of assessing iodized oil uptake with cone-beam computed tomography(CT)in transarterial chemoembolization(TACE)for small he-patocellular carcinoma(HCC).METHODS:Cone-beam CT provided by a biplane flat-panel detector angiography suite was performed on eighteen patients(sixteen men and two women;41-76 years;mean age,58.9 years)directly after TACE for small HCC(26 nodules under 30 mm;mean diam-eter,11.9 mm;range,5-28 mm).The pre-procedural locations of the tumors were evaluated using tripha-sic multi-detector row helical computed tomography(MDCT).The tumor locations on MDCT and the iodized oil uptake by the tumors were analyzed on cone-beam CT and on spot image directly after the ***:All lesions on preprocedural MDCT were de-tected using iodized oil uptake in the lesions on cone-beam CT(sensitivity 100%,26/26).Spot image depictediodized oil uptake in 22 of the lesions(sensitivity 85%).The degree of iodized oil uptake was overestimated(9%,2/22)or underestimated(14%,3/22)on spot image in f ive nodules compared with that of cone-beam ***:Cone-beam CT is a useful and conve-nient tool for assessing the iodized oil uptake of small hepatic tumors(< 3 cm)directly after TACE.

SnO_2 nanowires and ZnO nanorods were grown on the surface of thin film by heat treatment of mental Sn and Zn under Ar gas flow and O_2 atmospheric *** sensitivity of the SnO_2 thin film device on which grown nanowires to CO gas(concentration of 5000μg/g)was 50% at the operating temperature of 200℃.In case of using Pt as catalysts,the sensitivity was enhanced and operating temperature was reduced(sensitivity of 70% at the operating temperature of 150℃). The sensitivity of the ZnO nanorods device using Cu as catalysts to NO_x gas was 90% at the operating temperature of 200℃.It was found that the sensitivity to CO and NO_x gases for the device on which grown nanostructures was much higher than those for general thin films devices.

BACKGROUND Retroperitoneal sarcoma(RPS) is a rare malignancy arising from mesenchymal cells that most commonly presents as an abdominal mass and is associated with poor *** several studies have assessed the survival benefits of wide excision,few have reported detailed methods for achieving wide excision in patients with *** To describe our experience with multidisciplinary surgical resection of RPS using intra-and extra-pelvic *** Multidisciplinary surgery is an anatomical approach that combines intra-and extra-peritoneal access within the same surgery to achieve complete RPS *** retrospective review of the records of patients who underwent multidisciplinary surgery for RPS analyzed surgical and survival *** Eight patients underwent 10 intra-and extra-pelvic surgical resections,and their median mass size was 12.75 cm(range,6-45.5 cm).Using an intrapelvic approach,laparoscopy-assisted surgery was performed in four cases and laparotomy surgery in *** an extrapelvic approach,ilioinguinal and posterior approaches were used in four cases each,and the prone position and midline skin incision were shared in *** patients’ RPS masses were removed completely,and four achieved R0 resection through intra-and extra-pelvic *** median estimated blood loss was 2000 m L(range,300-20000 m L) and the median hospitalization was 12.6 d(range,9-69 d).Reoperation was needed in two patients(one for wound necrosis and the other for bowel perforation and wound necrosis).The median overall survival rate and median progression-free survival were 64.6 and 13.7 mo,*** RPS is therapeutically challenging because of its location and high risk of ***,intra-and extra-pelvic surgical approaches can improve the macroscopic security of the surgical margin.

Early expressions of hypoxia-inducible factor 1alpha and vascular endothelial growth factor increase the neuronal plasticity of activated endogenous neural stem cells after focal cerebral ischemia

Endogenous neural stem cells become ＂activated＂ after neuronal injury, but the activation sequence and fate of endogenous neural stem cells in focal cerebral ischemia model are little known. We evaluated the relationships between neural stem cells and hypoxia-inducible factor-1α and vascular endothelial growth factor expression in a photothromobotic rat stroke model using immunohistochemistry and western blot analysis. We also evaluated the chronological changes of neural stem cells by 5-bromo-2′-deoxyuridine（BrdU） incorporation. Hypoxia-inducible factor-1α expression was initially increased from 1 hour after ischemic injury, followed by vascular endothelial growth factor expression. Hypoxia-inducible factor-1α immunoreactivity was detected in the ipsilateral cortical neurons of the infarct core and peri-infarct area. Vascular endothelial growth factor immunoreactivity was detected in bilateral cortex, but ipsilateral cortex staining intensity and numbers were greater than the contralateral cortex. Vascular endothelial growth factor immunoreactive cells were easily found along the peri-infarct area 12 hours after focal cerebral ischemia. The expression of nestin increased throughout the microvasculature in the ischemic core and the peri-infarct area in all experimental rats after 24 hours of ischemic injury. Nestin immunoreactivity increased in the subventricular zone during 12 hours to 3 days, and prominently increased in the ipsilateral cortex between 3–7 days. Nestin-labeled cells showed dual differentiation with microvessels near the infarct core and reactive astrocytes in the peri-infarct area. BrdU-labeled cells were increased gradually from day 1 in the ipsilateral subventricular zone and cortex, and numerous BrdU-labeled cells were observed in the peri-infarct area and non-lesioned cortex at 3 days. Comparative detection of syndecan-2 methylation in preoperative and postoperative stool DNA in patients with colorectal cancer

BACKGROUND Early detection of colorectal cancer(CRC)is essential to reduce cancer-related morbidity and *** DNA(sDNA)testing is an emerging method for early CRC ***-2(SDC2)methylation is a potential biomarker for the sDNA *** DNA methylation is an early epigenetic event during tumorigenesis and can occur in the normal colonic mucosa during aging,which can compromise the sDNA test *** In this prospective study,we enrolled 151 patients with CRC who underwent curative surgical resection between September 2016 and May *** stool samples were collected from 123 patients and postoperative samples were collected from 122 patients.A total of 104 samples were collected from both preoperative and postoperative *** promoter methylation of SDC2 in sDNA was assessed using linear target enrichment quantitative methylation-specific real-time polymerase chain *** parameters were analyzed using the results of SDC2 *** Detection rates of SDC2 methylation in the preoperative and postoperative stool samples were 88.6%and 19.7%,*** tumor size(3 cm,P=0.019)and advanced T stage(T3–T4,P=0.033)were positively associated with the detection rate of SDC2 methylation before *** sex was associated with false positives after surgery(P=0.030).Cycle threshold(CT)values were significantly decreased postoperatively compared with preoperative values(P<0.001).The postoperative negative conversion rate for preoperatively methylated SDC2 was 79.3%(73/92).CONCLUSION Our results suggested that the SDC2 methylation test for sDNA has acceptable sensitivity and ***,small size and early T stage tumors are associated with a low detection rate of SDC2 *** the cycle threshold values significantly decreased after surgery,SDC2 methylation test for sDNA might have a diagnostic value for CRC.
