背景：证据显示，与临床常规降脂比较，对稳定性冠状动脉疾病患者施行强化降脂降低低密度脂蛋白胆固醇（low—density lipoprotein cholesterol，LDL—C）可进一步获益。目的：比较2种降脂治疗策略对既往有心肌梗死（MyocardialInfarction，MI）病史患者心血管事件危险的影响。设计、地点及研究对象：IDEAL研究是一项前瞻性、随机、开放、盲法终点评价试验。试验于1999年3月至2005年3月在北欧190个心脏病急救单位和专家诊所进行，中位随访时间为4．8年。试验人选8888例有急性MI病史的患者，年龄≤80岁。干预：患者随机分配接受大剂量阿托伐他汀（80mg／d；n=4439）或常规剂量辛伐他汀（20mg／d；n=4449）治疗。主要终点：主要冠脉事件的发生定义为冠脉死亡、证实有非致命性急性MI或心脏骤停复苏。结果：治疗期间，辛伐他汀组平均LDL—C水平为104mg／dL（SE，0．3），阿托伐他汀组为81mg／dL（SE，0．3）。辛伐他汀组有463例（10．4％）患者发生了主要冠脉事件，阿托伐他汀组为411例（9．3％）（风险比[hazardratio，HR]，0．89；95％CI，0．78—1．01；P=0．07）。两组发生非致命性MI者分别为321例（7．2％）和267例（6．0％）（HR，0．83；95％CI，0．71—0．98；P=0．02）。但是另外两个一级终点组分的发生率两组末见显著差异。两组发生主要心血管事件者分别为608例和533例（HR，0．87；95％CI，0．77—0．98；P=0．02）。辛伐他汀组发生任何一种冠脉事件的患者有1059例，阿托伐他汀组898例（HR，0．84；95％CI，0．76—0．91；P〈0．001）。两组发生非心血管死亡者分别为156例（3．5％）和143例（3．2％）（HR，0．92；95％CI，0．73—1．15；P=0．47）。各种原因所致死亡：辛伐他汀组374例（8．4％），阿托伐他汀组366例（8．2％）（HR，0．98；95％CI，0．85—1．13；P=0．81）。阿托伐他汀组患者凶非严重性不良事件而停药的比例较高；因转氨酶升高分别导致43例（1．0％）和5例（0．1％）患者停药（P（0．001）。两组罕见严重肌病及横纹肌溶解。结论：研究表明，在既往有MI的患者，强化降低LDL—C并不能显著降低主要冠脉事件的一级终点，但是能降低其他复合二级终点及非致命性急性MI的危险。心血管死亡率或全因死亡率没有差异。强化降低LDL—C可使MI患者获益，而且不增加非心血管死亡率或其他严重不良反应。

Fatigue failures cost approximately 4% of the United States' gross domestic product(GDP). The design of highly fatigue-resistant materials is always in demand. Different from conventional strategies of alloy design, high-entropy alloys(HEAs) are defined as materials with five or more principal elements, which could be solid solutions. This locally-disordered structure is expected to lead to unique fatigue-resistant properties. In this review, the studies of the fatigue behavior of HEAs during the last five years are summarized. The four-point-bending high-cycle fatigue coupled with statistical modelling, and the fatigue-crack-growth behavior of HEAs, are reviewed. The effects of sample defects and nanotwins-deformation mechanisms on four-point-bending high-cycle fatigue of HEAs are discussed in detail. The influence of stress ratio and temperature on fatigue-crack-growth characteristics of HEAs is also discussed. HEAs could exhibit comparable or greater fatigue properties, relative to conventional materials. Finally,the possible future work regarding the fatigue behavior of HEAs is suggested.

The Effect of Taping on Foot Structure, Functional Foot Stability and Running Gait Patterns of the Foot

Running related foot injuries are associated with excessive foot movements due to malaligned foot structure and poor functional foot stability. Clinicians tape the foot to alleviate pain and prevent further injuries, whilst allowing the athlete to continue training. However, the effect of taping is not conclusive. The purpose of this study is to investigate if taping effectively improves foot structure, functional foot stability and reduces excessive foot movements during running. Twenty-two subjects had their foot structure identified as： fiat foot stable, fiat foot unstable and normal arched unstable according to the FPI （foot posture index） and the Modified Romberg＇s Test with the BESS （balance error scoring system） criteria. The subjects ran on an instrumented treadmill barefooted with their feet taped and untaped. Running kinetic and kinematic data were collected and analysed using a paired t-test and 3x2 ANOVA. Taping improved foot structure but not functional foot stability. During running, taping significantly reduced rearfoot eversion. Taping increased the loading rate in the fiat foot and normal arched unstable groups but reduced the loading rate for the flat foot stable group. Implication on the appropriate use of foot taping was discussed.

To elucidate and quantify which cell types contribute to the developing neointima formation causing restenosis after vessel graft,we established a novel mouse model by grafting a decellularized vessel to the carotid ***,the graft developed neointimal lesions after 2 weeks,resulting in lumen closure within 4 weeks. Endothelial cells(EC) and smooth muscle cells(SMC),monocytes and stem/progenitor cells were detected 2 weeks *** lesional cells from neointimal tissue explant cultured in stem cell medium expressed a panel of stem/progenitor markers including c-kit,Sca-1 and *** showed clonogenic and multilineage differentiation *** Sca-1+ cells were able to differentiate into ECs and SMC in *** application of VEGF in vivo to the adventitial side of grafted vessel,increased re-endothelialization and reduced neointimal *** result indicate that a population of stem/progenitor cells exists within developing neointima,which display the ability to differentiate into both EC and SMC and can contribute to restenosis,drugs or cytokines directing cell differentiation to ECs may be effective in preventing or retarding restenosis.

TIFA(TRAF-interacting protein with forkhead-associated domain) is a small FHA-containing human *** was first identified as a TRAF2 binding protein and later reported to also bind to TRAF6 and play a major role in the activation of the nuclear factor-κB(NF-κB).As the superior regulator,NF-κB transactivates many cytokines,chemokines,and inflammationassociated transcriptional factors in response to immune and inflammatory *** this regard,aberrant activation of NF-κB has been reported for many chronic inflammatory *** previous study demonstrated that Thr9 phosphorylation-dependent oligomerization of TIFA triggers the activation of NF-κB under TNF-α ***,we reported the crystal structures of TIFA and its complex with the N-terminal pThr9 peptide,which uncovered its unique features in the FHA structure with the pThr9 peptide and provided the structural mechanism for TIFA *** a breakthrough report demonstrated that such pThr9-dependent oligomerization of TIFA is also triggered by the bacteria derived metabolic intermediate monosaccharide heptose-1,7-bisphosphate(HBP),leading to activation of innate *** then further showed that TIFA mediates innate immune response through assembly of NLRP3 inflammasome via both Signal 1(priming) and Signal 2(activation) in dysfunctional *** addition,we further identified that Aurora A is an essential kinase for TIFA pThr9-dependent Aurora A-NF-κB survival signaling axis in acute myeloid leukemia(AML).These reports together not only decipher the detailed molecular mechanism within TIFA-mediated inflammation and innate immunity activation,but also suggest that TIFA is a potential therapeutic target in the treatment of immune disorders and cancers.

Genotype and phylogenetic characterization of hepatitis B virus among multi-ethnic cohort in Hawaii

AIM: Hepatitis B virus (HBV) genomes in carriers from Hawaii have not been evaluated previously. The aim of thepresent study was to evaluate the distribution of HBV genotypes and their clinical relevance in ***: Genotyping of HBV among 61 multi-ethnicc arriers in Hawaii was performed by genetic *** complete genomes and 61 core promoter/precore regions of HBV were sequenced ***: HBV genotype distribution among the 61 carderswas 23.0% for genotype A, 14.7% for genotype B and 62.3% for genotype C. Genotypes A, B and C were obtained from the carriers whose ethnicities were Filipino and Caucasian,Southeast Asian, and various Asian and Micronesian,respectively. All cases of genotype B were composed of recombinant strains with genotype C in the precore plus core region named genotype Ba. HBeAg was detected more frequently in genotype C than in genotype B (68.4% vs 33.3%, P<0.05) and basal core promoter (BCP) mutation (T1762/A1764) was more frequently found in genotype C than in genotype B. Twelve of the 38 genotype C strains possessed C at nucleotide (nt) position 1858 (C-1858).However there was no significant difference in clinical characteristics between C-1858 and T-1858 variants. Based on complete genome sequences, phylogenetic analysis revealed one patient of Micronesian ethnicity as having C1858 clustered with two isolates from Polynesia with *** addition, two strains from Asian ethnidties were clustered with known isolates in carriers from Southeast ***: Genotypes A, B and C are predominant types among multi-ethnic HBV carriers in Hawaii, and distribution of HBV genotypes is dependent on the ethnic background of the carriers in Hawaii.
