Effects of milrinone on inflammatory response-related gene expressions in cultured rat cardiomyocytes

Background To evaluate the feasibility and safety of a fast initiation of cooling to a target temperature of 35℃by means of transnasal cooling in patients with anterior circulation large vessel occlusion(LVO)undergoing endovascular thrombectomy(EVT).Methods Patients with an LVO onset of<24hour who had an indication for EVT were included in the *** cooling(RhinoChill)was initiated immediately after the patient was intubated for EVT and continued until an oesophageal target temperature of 35℃was *** was maintained with surface cooling for 6-hour postrecanalisation,followed by active rewarming(+0.2℃/hour).The primary outcome was defined as the time required to reach 35℃,while secondary outcomes comprised clinical,radiological and safety *** Twenty-two patients(median age,77 years)were included in the study(14 received additional thrombolysis,4 additional stenting of the proximal internal carotid artery).The median time intervals were 309min for last-seen-normal-to-groin,58min for door-to-cooling-initiation,65min for door-to-groin and 123min for *** target temperature of 35℃was reached within 30min(range 13–78min),corresponding to a cooling rate of 2.6℃/*** recanalisation,86%of the patients had a body temperature of≤35℃.The median National Institutes of Health Stroke Scale at admission was 15 and improved to 2 by day 7,and 68%of patients had a good outcome(modified Rankin Scale 0–2)at 3 *** complications included asymptomatic bradycardia(32%),pneumonia(18%)and asymptomatic haemorrhagic transformation(18%).Conclusion The combined application of hypothermia and thrombectomy was found to be feasible in sedated and ventilated *** events were comparable to those previously described for EVT in the absence of *** effect of this procedure will next be evaluated in the randomised COmbination of Targeted temperature management and Thrombectomy after acute Ischemic Stro

Neutron-antineutron oscillations in the deuteron studied with NN and ■N interactions based on chiral effective field theory

Neutron-antineutron(n−n)oscillations in the deuteron are ***,the deuteron lifetime is calculated in terms of the free-space n−n oscillation time τn−n based on NN and NN interactions derived within chiral effective field theory(EFT).This results in(2.6±0.1)×10^22τ2^n−n s,which is close to the value obtained by Dover and collaborators more than three decades ago,but disagrees with recent EFT calculations that were performed within the perturbative scheme proposed by Kaplan,Savage,and *** reasons for the difference are discussed.

Different effects of a CD14 gene polymorphism on disease outcome in patients with alcoholic liver disease and chronic hepatitis C infection

AIM： Clinical and experimental data suggest that gut-derived endotoxins are an important pathogenic factors for progression of chronic liver disease. Recently, a C-T （-159） polymorphism in the promoter region of the CD14 gene was detected and found to confer increased CD14 expression and to be associated with advanced alcoholic liver damage. Here, we investigated this polymorphism in patients with less advanced alcoholic liver disease （ALD） and chronic hepatitis C virus （HCV） infection. METHODS： CD14 genotyping was performed by PCR-RFLP analysis in （a） 121 HCV patients, （b） 62 patients with alcohol-associated cirrhosis （AIc-Ci）, （c） 118 individuals with heavy alcohol abuse without evidence of advanced liver damage （Alc-w/o Ci）, and （d） 247 healthy controls. Furthermore, serum levels of soluble CD14 （sCD14） and transaminases were determined. RESULTS： The TT genotype was significantly more frequent in Alc-Ci compared to Alc-w/o Ci or controls （40.3% vs 23.7% or 24.0%, respectively）. In Alc-w/o Ci, serum levels of transaminases did not differ significantly between patients with different CD14 genotypes. In HCV patients, TT-homozygotes had significantly higher sCD14 levels and sCD14 serum levels were significantly higher in patients with advanced fibrosis or cirrhosis. However, no association was found between CD14 genotypes and histological staging or grading. CONCLUSION： Considering serum transaminases as surrogate markers for alcoholic liver damage, the CD14 polymorphism seems to exhibit different effects during the course of ALD. Differences in genotype distribution between cirrhotic HCV patients and alcoholics and the known functional impact of this polymorphism on CD14 expression levels further indicate differences in the pathophysiological role of CD14 and CD14-mediated lipopolysaccharides signal transduction with regard to the stage as well as the type of the underlying liver disease.

Broadband mid-infrared(IR)supercontinuum laser sources are essential for spectroscopy in the molecular fingerprint ***,we report generation of octave-spanning and coherent mid-IR supercontinua in As_(2)S_(3)-silica nanospike hybrid waveguides pumped by a custom-built 2.8μm femtosecond fiber *** waveguides are formed by pressure-assisted melt-filling of molten As_(2)S_(3) into silica capillaries,allowing the dispersion and nonlinearity to be precisely *** coherent spectra spanning from 1.1μm to 4.8μm(30 dB level)are observed when the waveguide is designed so that 2.8μm lies in the anomalous dispersion ***,linearly tapered millimeter-scale As_(2)S_(3)-silica waveguides are fabricated and investigated for the first time,to the best of our knowledge,showing much broader supercontinua than uniform waveguides,with improved spectral *** waveguides are demonstrated to be long-term stable and water-resistant due to the shielding of the As_(2)S_(3) by the fused silica *** offer an alternative route to generating broadband mid-IR supercontinua,with applications in frequency metrology and molecular spectroscopy,especially in humid and aqueous environments.

Analysis of monocyte chemotactic protein-1 gene polymorphism in patients with spontaneous bacterial peritonitis

AIM:To investigate a genetic polymorphism of the monocyte chemotactic protein-1 (MCP-1 ) gene in patients with spontaneous bacterial peritonitis (SBP).METHODS:MCP-1 genotyping was performed in 23 patients with SBP and 83 cirrhotic control patients with non-infected ***:The frequency of carriers of the G-allele was lower in SBP patients but this difference did not reach statistical significance. However,in the subgroup of patients with alcoholic cirrhosis (n=80),carriers of the G-allele were significantly less frequent in SBP-patients (38.1%) than in cirrhotic controls (67.8%,P=0.021). CONCLUSION:In patients with alcoholic liver cirrhosis,the-2518 MCP-1 genotype AA is a risk factor for the development of SBP.
