Antithrombotic treatment in chronic heart failure and sinus rhythm: Systematic review

AIM: To assess the efficacy and safety of antithrombotic drugs(antiplatelet or anticoagulant drugs) compared to no antithrombotic treatment or placebo in patients with heart failure(HF) and sinus rhythm. METHODS: We searched Medline and Cochrane Library for randomized controlled trials evaluating antithrombotic treatment and no antithrombotic treatment in patients with HF and sinus rhythm. Risk ratio(RR) and 95%CIs were estimated performing meta-analysis with random effects method. RESULTS: Two studies met the inclusion criteria: Heart failure Long-term Antithrombotic Study and Warfarin/Aspirin Study in Heart failure, with 336 patients and mean follow-up 1.8-2.25 years. Stroke risk was not reduced by acetylsalicylic acid(RR = 1.18, 95%CI: 0.17-8.15), oral anticoagulation(RR = 0.30, 95%CI: 0.03-2.65) or overall antithrombotic drugs(RR = 0.52, 95%CI: 0.10-2.74). Acetylsalicylic acid showed a significant increased risk of worsening HF(RR = 1.78, 95%CI: 1.08-2.92), while oral anticoagulation had no impact in this outcome(RR = 1.03, 95%CI: 0.61-1.75). Overall antithrombotic drugs showed a significant risk increase of major bleeding(RR = 6.99, 95%CI: 0.89-54.64). CONCLUSION: Best available evidence does not support the routine use of antithrombotic drugs in patients with HF and sinus rhythm. These drugs, particularly oral anti-coagulation has the hazard of increase significantly major bleeding risk.

ENERGY ESTIMATES FOR DELAY DIFFUSION-REACTION EQUATIONS

In this paper we consider nonlinear delay diffusion-reaction equations with initial and Dirichlet boundary conditions. The behaviour and the stability of the solution of such initial boundary value problems （IBVPs） are studied using the energy method. Simple numerical methods are considered for the computation of numerical approximations to the solution of the nonlinear IBVPs. Using the discrete energy method we study the stability and convergence of the numerical approximations. Numerical experiments are carried out to illustrate our theoretical results.

In this paper the effect of integral memory terms in the behavior of diffusion phenomena is studied. The energy functional associated with different models is analyzed and stability inequalities are established. Approximation methods for the computation of the solution of the integro-differential equations are constructed. Numerical results are included.

A balanced adaptive time-stepping strategy is implemented in an implicit discontinuous Galerkin solver to guarantee the temporal accuracy of unsteady simulations.A proper relation between the spatial,temporal and iterative errors generated within one time step is *** an estimate of temporal and spatial error using an embedded RungeKutta scheme and a higher order spatial discretization,an adaptive time-stepping strategy is proposed based on the idea that the time step should be the maximum without obviously infuencing the total error of the *** designed adaptive time-stepping strategy is then tested in various types of problems including isentropic vortex convection,steady-state fow past a fat plate,Taylor-Green vortex and turbulent fow over a circular cylinder at Re=*** results indicate that the adaptive time-stepping strategy can maintain that the discretization error is dominated by the spatial error and relatively high efciency is obtained for unsteady and steady,well-resolved and under-resolved simulations.

Siderophore-Producing Rhizobacteria as a Promising Tool for Empowering Plants to Cope with Iron Limitation in Saline Soils:A Review

Iron(Fe) bioavailability to plants is reduced in saline soils;however, the exact mechanisms underlying this effect are not yet completely understood. Siderophore-expressing rhizobacteria may represent a promising alternative to chemical fertilizers by simultaneously tackling salt-stress effects and Fe limitation in saline soils. In addition to draught, plants growing in arid soils face two other major challenges: high salinity and Fe deficiency. Salinity attenuates growth, affects plant physiology, and causes nutrient imbalance,which is, in fact, one of the major consequences of saline stress. Iron is a micronutrient essential for plant development, and it is required by several metalloenzymes involved in photosynthesis and respiration. Iron deficiency is associated with chlorosis and low crop productivity. The role of microbial siderophores in Fe supply to plants and the effect of plant growth-promoting rhizobacteria(PGPR) on the mitigation of saline stress in crop culture are well documented. However, the dual effect of siderophore-producing PGPR, both on salt stress and Fe limitation, is still poorly explored. This review provides a critical overview of the combined effects of Fe limitation and soil salinization as challenges to modern agriculture and intends to summarize some indirect evidence that argues in favour of siderophore-producing PGPR as biofertilization agents in salinized soils. Recent developments and future perspectives on the use of PGPR are discussed as clues to sustainable agricultural practices in the context of present and future climate change scenarios.

Introduction In Brazil,approximately 5%are born with a congenital disorder,potentially fatal without *** study aims to evaluate the relationship between gastrointestinal congenital malformation(GICM)mortality,health indicators,and socioeconomic factors in *** GICM admissions(Q39-Q45)between 2012 and 2019 were collected using national *** demographics,socioeconomic factors,clinical management,outcomes,and the healthcare workforce density were also accounted *** Surgical Workforce density and the number of neonatal intensive care units in a region were extracted from national datasets and combined to create a clinical index termed'NeoSurg'.Socioeconomic variables were combined to create a socioeconomic index termed SocEcon'.Simple linear regression was used to investigate if the temporal changes of both indexes were *** correlation between mortality and the different indicators in Brazil was evaluated using Pearson's correlation *** Over 8 years,Brazil recorded 12804 GICM *** Southeast led with 6147 cases,followed by the Northeast(2660),South(1727),North(1427),and Midwest(843).The North and Northeast reported the highest mortality,lowest NeoSurg,and SocEcon Index ***,mortality rates declined across regions from 7.7%(2012)to 3.9%(2019),a 51.7%*** North and Midwest experienced the most substantial reductions,at 63%and 75%,*** significantly correlated with the indexes in nearly all regions(p<0.05).Conclusion Our study highlights the correlation between social determinants of health and GICM mortality in Brazil,using two novel indexes in the pediatric *** findings provide an opportunity to rethink and discuss new indicators that could enhance our understanding of our country and could lead to the development of necessary solutions to tackle existing challenges in Brazil and globally.

As jET is developing and testing operational scenarios for higher fusion performance,an increase in pulse disruptivity is being *** a deeper analysis,we find that several radiative phenomena play an active role in determining the outcome of the *** analysis is enabled by the use of real-time tomography based on the bolometer *** though plasma tomography is an inverse problem,we use machine learning to train a forward model that provides the radiation profile directly,based on a single matrix multiplication *** model is used to investigate radiative phenomena including sawtooth crashes,ELMs and MARFE,and their relationship to the radiated power in different regions of *** particular,we use realtime tomography to monitor the core region,and to throw an alarm whenever core radiation exceeds a certain *** results suggest that this measure alone can anticipate a significant fraction of disruptions in the jET baseline scenario.

Design and Optimization of a GaN GIT Based PFC Boost Converter

The advent of GaN power devices brings the possibility of pushing up frequencies of power electronics system beyond the capacity of conventional Si *** harvest the high frequency operation benefits of GaN technology,not only GaN devices should be used optimally by providing proper switching and conduction conditions to best accommodate parasitic elements inherent with the devices,other components should also be capable of facilitating high frequency *** this paper,PFC boost converter is used as a platform to harvest the benefits of GaN technology and,at the same time,to uncover the limitations in pushing power electronics systems up.A 600W PFC boost converter with a maximum switching frequency of 3MHz is designed and implemented,which reached an average efficiency of 98.5%.The results indicate that,when properly used,a GaN PFC boost converter could operate at a frequency that is beyond the reach of conventional Si *** well,with availability of GaN devices,high frequency operation of power electronics systems is no longer limited by the lack of switching devices but by practical issues *** frequency magnetic materials,high frequency controllers,etc.

Droplet-based bioprinting enables the fabrication of cell–hydrogel–microfibre composite tissue precursors

Composites offer the option of coupling the individual benefits of their constituents to achieve unique material properties,which can be of extra value in many tissue engineering *** combining hydrogelswith fibre-based scaffolds can create tissue constructs with enhanced biological and structural ***,developing efficient and scalable approaches to manufacture such composites is ***,we use a droplet-based bioprinting system called reactive jet impingement(RejI)to integrate a cell-laden hydrogel with a microfibrous *** system uses microvalves connected to different bioink reservoirs and directed to continuously jet bioink droplets at one another in mid-air,where the droplets react and form a hydrogel that lands on a microfibrous ***–hydrogel–fibre composites are produced by embedding human dermal fibroblasts at two different concentrations(5×10^(6) and 30×10^(6) cells/mL)in a collagen–alginate–fibrin hydrogel matrix and bioprinted onto a fibre-based *** results show that both types of cell–hydrogel–microfibre composite maintain high cell viability and promote cell–cell and cell–biomaterial *** lower fibroblast density triggers cell proliferation,whereas the higher fibroblast density facilitates faster cellular organisation and infiltration into the ***,the fibrous component of the composite is characterised by high swelling properties and the quick release of calcium *** data indicate that the created composite constructs offer an efficient way to create highly functional tissue precursors for laminar tissue engineering,particularly for wound healing and skin tissue engineering applications.

Enhancing production monitoring: A back allocation methodology to estimate well flow rates and assist well test scheduling

Production flow rates are crucial to make operational decisions,monitor,manage,and optimize oil and gas *** rates also have a financial importance to correctly allocate production to fiscal purposes required by regulatory agencies or to allocate production in fields owned by multiple *** its significance,usually only the total field production is measured in real time,which requires an alternative way to estimate wells'*** address these challenges,this work presents a back allocation methodology that leverages real-time instrumentation,simulations,algorithms,and mathe-matical programming modeling to enhance well monitoring and assist in well test *** methodology comprises four modules:simulation,classification,error calculation,and *** modules work together to characterize the flowline,wellbore,and reservoir,verify simulation outputs,minimize errors,and calculate flow rates while honoring the total platform flow *** well status generated through the classification module provides valuable information about the current condition of each well(*** the well is deviating from the latest well test parameters),aiding in decision-making for well testing scheduling and *** effectiveness of the methodology is demonstrated through its application to a representative offshore oil field with 14 producing wells and two years of daily production *** results highlight the robustness of the methodology in properly classifying the wells and obtaining flow rates that honor the total platform flow ***,the methodology supports well test scheduling and provides reliable indicators for well *** uti-lizing real-time data and advanced modeling techniques,this methodology enhances production monitoring and facilitates informed operational decision-making in the oil and gas industry.
