Tracheoesophageal Voice Prosthesis Outcomes:Success or Insucess?

Introduction: Total laryngectomy/pharingo-laryngectomy is a potentially aggressive surgery for advanced laryngeal/hypopharyngeal carcinomas, which results in important physical and functional changes that compromise some of the most vital functions, including speech communication. For these patients, tracheoesophageal speech is considered to be the gold standard for voice rehabilitation. Objectives: The purpose of the present study was to determinate the success rate of voice prosthesis rehabilitation, voice prosthesis lifetime and the rate of complications, and its related clinicopathologic factors. Material and Methods: Retrospective review of 92 patients who undergone tracheoesophageal puncture (TEP) performed between January 2007 and December 2012 at the Francisco Gentil Portuguese Institute of Oncology of Oporto. Age, primary disease, staging, the extent of surgical resection, radiotherapy treatment, timing of TEP, surgical and prosthesis-related complications were noted. The impact of these clinicpathological factors on functional outcome, complications of TEP and lifetime of prosthetic valves was assessed in univariate analysis. Vocal rehabilitation efficacy with voice prosthesis was assessed with the multidimensional Harrison-Robillard Shultz (HRS) Rating Scale. Lifetime of voice prosthesis and early and late complications were recorded. Results: A total of 83 patients met the study criteria, predominantly males (94%) with a mean age of 63.7 years. 77% of the patients underwent primary and 23% secondary TEP. 68.7% of patients achieved functional tracheoesophageal speech (HRS score ≥ 10), 67.2% had performed primary TEP and 73.7% had performed secondary TEP. The mean device lifetime was 9.8 months for voice prosthesis. Prosthesis-related complications occurred in 81% of the patients and the most common issues were prosthesis leakage (76%) and displacement (22%). Comparison of the Internal Structures of Bones by Microtomography

The use of computed microtomography （***） has revolutionized many areas of research, such as noninvasive, fast and high precision techniques, which allows immediate visualization of internal structures without any risk to the object of study. ~tCT is widely accepted for medical diagnostics, is also important for purposes of zoological research and paleontological. In this work, we used the μCT to investigate the internal structure of bones from mammalian and poultry. We studied the bones of rats and quail. Through microtomography images, we observed that the bones of the poultry have a bony structure in the form of a trellis that is not present in bones of mammals. These trellises bony is an evolutionary adaptation that allowed the bones of the birds to become longer and lighter maintaining its strength. It was also observed that the percentage of the trabecular area in poultry is almost half of that observed in mammals. The results obtained validate the use of μCT as a technique that allows the study of bone structures in small samples, enabling to explore the morphological differences between the bones of those species.

Modeling and Analysis of Ladder-Network Transmission Lines with Capacitive and Inductive Lumped Elements

This paper examines the properties of wave propagation in transmission lines with periodic LC and CL cells, taking into account ohmic losses in resistors connected in series to lumped capacitors and inductors. First order time differential equations are derived for current and charge, thus allowing analysis of transient regimes of the lines being excited by a pulse of arbitrary shape. In particular we examine the propagation characteristics of periodic lines in which identical unit cells are repeated periodically and also discuss the interpretation of positive and negative phase velocities associated with the LC and CL topologies. Loss effects on the propagation bandwidths of both lines are also discussed, and it is shown that in the left-handed transmission line (CL configuration) the phase advance of the crest of the transmitted signal with respect to the source signal is due to the intrinsic dispersive nature of the CL line which, in contrast to the LC line, is highly dispersive at low propagation factors.

Using Graphics Processing Units to Parallelize the FDK Algorithm for Tomographic Image Reconstruction

The paper presents the implementation of a parallel version of FDK （Felkamp, David e Kress） algorithm using graphics processing units. Discussion was briefly some elements the computed tomographic scan and FDK algorithm; and some ideas about GPUs （Graphics Processing Units） and its use in general purpose computing were presented. The paper shows a computational implementation of FDK algorithm and the process of parallelization of this implementation. Compare the parallel version of the algorithm with the sequential version, used speedup as a performance metric. To evaluate the performance of parallel version, two GPUs, GeForce 9400GT （16 cores） a low capacity GPU and Quadro 2000 （192 cores） a medium capacity GPU was reached speedup of 3.37.

Acmella oleracea is a plant species belonging to the Asteraceae family, native to the Amazon that has broad therapeutic and culinary use. Several secondary metabolites and your bioactive component spilanthol have been associated with its medicinal effects. This study aims to evaluate the leaf extract of A. oleracea for therapeutic purposes. When applied to both cell cultures HEp-2 neoplastic cells and L929 fibroblast cells, the increasing extract concentrations yielded a significant reduction in cell viability using ANOVA test with Significance of P ≤ 0.05 (*), Very Significant P ≤ 0.01 (***) and Extremely Significant P ≤ 0.001 (****), causing visible damage to cytoskeleton actin filaments. The cytotoxicity of the A. oleracea leaf extract may be associated with other components, including flavonoids. Thus, A. oleracea has a great cytotoxic activity on tumoral cells and can be considered for future antitumor therapy. However, because of its toxicity to non-tumoral cells, it is essential to evaluate the efficacy of each extract sub-fraction compound, alerting the population for the traditional use of the plant that can cause damage for the consumer.

Discogenic pain: Who cares?

Chronic low back pain has a huge impact on daily living and a negative economic and professional effect. It is a matter of debate and concern for all health professionals involved, particularly spine surgeons. Recent discoveries on the innervation and biochemical properties of the intervertebral disc clarify the role of this structure as a possible cause of chronic low back pain. However, multiple causes may be present in the same patient making the diagnosis a challenging process. Discogenic pain is defined as a chronic low back pain induced by a degenerative disc disease. There are no specific characteristics of discogenic pain, although it has a higher incidence in younger age, it is usually localized medially in the back, worsens with axial loading and improves with recumbence. In the last decades we have assisted the emergence of multiple treatment techniques. However, neither the conservative treatment nor the interventional management has strong evidence in treating discogenic pain. Randomized control clinical trials are sought to improve patient outcome. Meanwhile, we believe each patient should be approached on an individual base. Discogenic pain: we care.

伴随着全球资源枯竭、环境污染加重以及生态系统不断恶化的状态加剧,环境问题逐渐成为了限制经济发展的重要问题。自中国共产党的十八大以来,中国政府进一步强调了生态环境保护的重要性,积极探讨经济增长与环境保护之间的协调发展,提出“绿色发展”的理念,这无疑也推动了绿色技术在企业中的发展与应用,绿色技术创新能力正在成为一个国家和企业可持续发展的重要指标。随着中国政府提出“双碳”目标,如何补充研究促进“双碳”目标实现的路径成为当今学术界所关注的重要议题。尤其是在逆全球化愈演愈烈的背景下,绿色技术创新究竟给企业出口带来了怎样的影响值得深入研究。本文就是在这一研究背景下,将企业绿色技术创新纳入到企业出口分析的整体框架中,深入探讨企业绿色技术创新是否以及如何促进企业出口这一主要问题,为助力中国“双碳”目标的实现路径提供了一个微观研究视角。除此之外,绿色技术创新也是一项能够带动环境保护及经济发展的重要途径,对于政府部门而言,其环境政策将会直接对国家在企业绿色技术创新的能力提高方面产生较大的影响,还会促进环境污染问题的有效解决。所以在研究绿色技术创新对于出口企业的效应方面,具有一定的研究意义。但是对比西方发达国家而言,中国的绿色技术创新起步整体较晚,发展也较为迟缓,所以对于中国而言,大力发展绿色技术创新是推动外贸企业出口效应提升的重要途径之一。本文首先运用企业微观数据测算企业绿色技术创新水平和企业出口水平,然后就企业绿色技术创新对自身出口的影响进行实证分析,并分析具体作用机制。研究结果表明:企业绿色技术创新可以直接促进企业出口,这一效应十分显著,未来企业大力进行绿色技术创新将有助于企业在国际竞争中建立优势,这一结论在经过一系列稳健性检验后仍然成立;二是企业进行绿色技术创新可以通过降低企业成本和提高企业生产率两条机制来促进出口;三是企业绿色技术创新存在所有权异质性,与现有研究略有不同的是,绿色技术创新在国有制企业的出口促进效应更加明显。本文的研究为政府制定创新政策、出口政策和“双碳”目标的实现路径提供了一定的参考,为企业出口效应的研究提供了一个较为新颖的视角,探索了绿色技术创新对企业出口影响的具体作用机制,以期能为实现中国绿色发展和在国际竞争中建立优势提供一定启示。同时在新时期,通过绿色技术创新的研究与发展,能够带动国际合作模式的新局面形成,对于带动中国的绿色技术进步而言,具有重要的现实意义。
