中成药是中医药产业的重要组成部分,也是中医药现代化的重要标志。目前,中成药制剂批间质量稳定性问题成为制约中药产业高质量发展的关键因素之一,投料饮片质量不均一是影响该问题的直接因素。该文就提高中成药投料饮片质量的均一性问题,提出了均一化投料研究方案。分别从均一化对象及前处理、均一化投料操作规程、均一化评价指标的选择、均一化投料算法、均一化投料工艺等方面进行系统阐述,依据关键质控指标作为均一化目标,探讨均一化投料工艺及其质量分析,形成中成药均一化投料示范性工艺策略,以期为推进中成药制剂批间质量一致性研究提供科学依据。

Brain interstitial fluid drainage and extracellular space affected by inhalational isoflurane: in comparison with intravenous sedative dexmedetomidine and pentobarbital sodium

Brain interstitial fluid drainage and extracellular space are closely related to waste clearance from the brain. Different anesthetics may cause different changes of brain interstitial fluid drainage and extracellular space but these still remain unknown. Herein,effects of the inhalational isoflurane, intravenous sedative dexmedetomidine and pentobarbital sodium on deep brain matters’ interstitial fluid drainage and extracellular space and underlying mechanisms were investigated. When compared to intravenous anesthetic dexmedetomidine or pentobarbital sodium, inhalational isoflurane induced a restricted diffusion of extracellular space, a decreased extracellular space volume fraction, and an increased norepinephrine level in the caudate nucleus or thalamus with the slowdown of brain interstitial fluid drainage. A local administration of norepinephrine receptor antagonists, propranolol,atipamezole and prazosin into extracellular space increased diffusion of extracellular space and interstitial fluid drainage whilst norepinephrine decreased diffusion of extracellular space and interstitial fluid drainage. These findings suggested that restricted diffusion in brain extracellular space can cause slowdown of interstitial fluid drainage, which may contribute to the neurotoxicity following the waste accumulation in extracellular space under inhaled anesthesia per se.

Objective This study aimed to evaluate the associations of serum folate and/or vitamin B12 concentrations with obesity among Chinese children and *** A cross-sectional study was conducted including 3,079 Chinese children and adolescents,aged 6 to 17 years,from Jiangsu,*** indices,such as,children's body mass index(BMI),BMI z-scores,waist circumference,and waist-to-height ratio were *** linear regression and generalized additive models were used to investigate the associations of serum folate and vitamin B12 levels with anthropometric indices and odds of *** We observed that serum vitamin B12 concentrations were inversely associated with all anthropometric indices and the odds of general obesity[odds ratio(OR)=0.68;95%confidence interval(CI)=0.59,0.78]and abdominal obesity(OR=0.68;95%CI=0.60,0.77).When compared to participants with both serum vitamin levels in the two middle quartiles,those with both serum folate and vitamin B12 levels in the highest quartile were less prone to general(OR=0.31,95%CI=0.19,0.50)or abdominal obesity(OR=0.46,95%CI=0.31,0.67).Conversely,participants with vitamin B12 levels in the lowest quartile alongside folate levels in the highest quartile had higher odds of abdominal obesity(OR=2.06,95%CI=1.09,3.91).Conclusion Higher serum vitamin B12 concentrations,but not serum folate concentrations,were associated with lower odds of childhood *** and adolescents with high levels of vitamin B12 and folate were less likely to be obese.

AIM: To investigate the relationship between serum vitamin D3 levels and liver fibrosis or inflammation in treatment-naive Chinese patients with chronic hepatitis C(CHC).METHODS: From July 2010 to june 2011, we enrolled 122 CHC patients and 11 healthy controls from dingxicity, Gansu Province, china. The patients were infected with Hepatitis C virus(HCV) during blood cell retransfusion following plasma donation in 1992-1995, and had never received antiviral treatment. At present, all the patients except two underwent liver biopsy with ultrasound guidance. The Scheuer Scoring System was used to evaluate hepatic inflammation and the Metavir Scoring System was used to evaluate hepatic fibrosis. Twelve-hour overnight fasting blood samples were collected in the morning of the day of biopsy. Serum levels of alanine aminotransferase, aspartate aminotransferase, total bilirubin, direct bilirubin, cholineste rase, prothrom binactivity, albumin, γ-glutamyl transpeptidase, hemoglobin, calcium and phosphorus were determined. Serum HCV RNA levels were measured by real-time PCR. Serum levels of 25-hydroxyvitamin D3 [25(OH)D3] and 24,25-dihydroxyvitamin D3 [24,25(OH)2D3] were measured by high-performance liquid chromatography tandem mass spectrometry.R E S U LT S : Serumlevels of 2 5(OH) D3 but not 24,25(OH)2D3 were significantly lower in CHC patients than in control subjects. Serum 25(OH)D3 levels did not correlate with liver fibrosis, inflammation, patient age, or levels of alanine aminotransferase, aspartate aminotransferase, total bilirubin, direct bilirubin, prothrombin activity, cholinesterase or HCV RNA. However, serum 25(OH)D3 levels did correlate with serum 24,25(OH)2D3 levels. Objective This study is aimed to report the development,the reliability and validity of the Chinese Children Physical Activity Questionnaire(CCPAQ)which was designed for the assessment of physical activity pattern in young population in *** The CCPAQ was administered for two times in 119 children(mean age 13.1±2.4 years;boys 47%)to examine reliability by using intraclass correlation *** was determined in 106 participants by agreement with the CCPAQ measures and the objective method,the ActiGraph *** on physical activity patter n including time spe nt on d iff ere nt intensi ties and total physical activity,sedentary behavior as well as physical activity energy expenditure were used to assess the validity with Spearman's correlation coefficient and the Bland-Altman *** The reliability coefficient of the CCPAQ ranged from 0.63-0.93(Intra-class correlation coefficient).Spearman's correlation coefficient for validity of time spent on total physical activity and sedentary behavior were all 0.32(P1),and for physical activity energy expenditure was 0.58(P1).Time spent on moderate-to-vigorous physical activity and light physical activity showed a relatively low correlation with the accelerometer(rho=0.20,P=0.040;rho=0.19,P=0.054).Conclusion The CCPAQ appears to be a promising and feasible method to assess physical activity pattern in Chinese children.

目的分析老年嗜酸性肉芽肿性多血管炎(eosinophilic granulomatosis with polyangiitis,EGPA)患者的临床特征。方法回顾性分析2007年2月至2022年2月首都医科大学附属北京朝阳医院确诊的EGPA患者的临床资料,根据年龄分为老年组(年龄≥60岁)和非老年组(年龄101例EGPA患者,其中老年组42例(41.6%),非老年组59例(58.4%)。老年组与非老年组比较,合并高血压病(40.5%vs 16.9%,P=0.008)、血嗜酸性粒细胞>10%(90.5%vs 72.9%,P=0.029)及估算肾小球滤过率(estimated glomerular filtration rate,eGFR)降低[eGFR1)·(1.73 m^(2))^(-1)](61.9%vs 16.9%,P1)的患者比例更高,老年组红细胞沉降率(erythrocyte sedimentation rate,ESR)[20(10,56.25)mm/h vs 12(5,32.25)mm/h,P=0.028]及肺功能指标残气量/肺总量(residual volume/total lung capacity,RV/TLC)[(48.84±11.43)%vs(41.92±9.06)%,P=0.009]更高。老年组与非老年组相比,心脏受累(73.8%vs 32.2%,P1)、中枢神经系统受累(23.8%vs 5.1%,P=0.006)、周围神经系统病变(83.3%vs 64.4%,P=0.036)、肾脏受累(69.0%vs 47.5%,P=0.031)及五因子评分(five-factor score,FFS)≥2分(61.9%vs 15.3%,P1)的发生率均更高。随访中位时间41(19,59)个月,6例患者死亡,老年组病死率高于非老年组(13.9%vs 1.92%,P=0.032)。结论老年EGPA患者合并高血压病、血嗜酸性粒细胞>10%、eGFR降低、脏器受累及FFS≥2分的患者比例更高,ESR、RV/TLC及病死率更高。临床医生应加强对老年EGPA患者疾病特征的认识,以提高诊断和治疗水平。
