Effect of ulinastatin on the inflammatory response after video-assisted thoracic lobectomy in patients with lung cancer:a randomized controlled study

Background:The first-line treatment for lung cancer is surgical resection,and one-lung ventilation(OLV)is the most basic anesthetic management method in lung *** OLV,inflammatory cytokines are released in response to the lung tissue damage and promote local and contralateral lung damage through the systemic *** designed a randomized,prospective study to evaluate the effect of the urinary trypsin inhibitor(UTI)ulinastatin on the inflammatory response after video-assisted thoracic lobectomy in patients with lung ***:Adult patients aged 19 to 70 years,who were scheduled for video-assisted thoracic lobectomy surgery to treat lung cancer between May 2020 and August 2020,were enrolled in this randomized,prospective ***(300,000 units)mixed with 100 mL of normal saline in the ulinastatin group and 100 mL of normal saline in the control group was administered over 1 h after inducing ***:The baseline(T0)interferon-γ(IFN-γ)/interleukin-4(IL-4)ratio was not different between the groups(6941.3±2778.7vs.6954.3±2752.4 pg/mL,respectively;P>0.05).The IFN-γ/IL-4 ratio was significantly higher in ulinastatin group at 30 min after entering the recovery room than control group(20,148.2±5054.3vs.6674.0±2963.6,respectively;adjustedP<0.017).Conclusion:Administering UTI attenuated the anti-inflammatory response,in terms of INF-γexpression and the IFN-γ/IL-4 ratio,after video-assisted thoracic surgery in lung cancer *** registration:Clinical Research Information Service of Korea National Institute of Health(CRIS),KCT0005533.

Effects of an Urban Park and Residential Area on the Atmospheric CO_2 Concentration and Flux in Seoul,Korea

The CO2 concentrations and fluxes over an urban forest site （Namsan） and an urban residential region （Boramae） in Seoul, Korea, during the non-growing season （2 4 March 2011）, the growing season （10-12 June 2011）, and the late-growing season （22-24 September 2011） were analyzed. The CO2 concentrations of two sites showed nearly the same diurnal variation, with a maximum value occurring during the night and a minimum value occurring during daytime, as well as the same seasonal variation, with a maximum value during the non-growing season （early spring） and a minimum value during the growing season （summer）. The CO2 flux over the urban forest did not show any typical diurnal variation during the non-growing season, but did show diurnal variation with a small positive value during the night and a large negative value during daytime in the growing and late-growing seasons due to photosynthesis in the urban forest. The CO2 flux over the urban residential region showed a positive daily mean value for all periods, with large values during the non-growing season and small values during the growing season, and it also showed diurnal variation with two maxima at 0600-1000 LST and 1800-2400 LST, and two minima at 0300-0600 LST and 1100-1500 LST, and was strongly correlated with the use of liquefied natural gas for cooking and heating by surrounding houses.

Carbon Dioxide Concentration and Flux in an Urban Residential Area in Seoul,Korea

The carbon dioxide （CO2） concentrations and fluxes measured at a height of 17.5 m above the ground by a sonic anemometer and an open-path gas analyzer at an urban residential site in Seoul, Korea from February 2011 to January 2012 were analyzed. The annual mean CO2 concentration was found to be 750 mg m-3, with a maximum monthly mean concentration of 827 mg m-3 in January and a minimum value of 679 mg m-3 in August. Meanwhile, the annual mean CO2 flux was found to be 0.45 mg m-2 s-1, with a maximum monthly mean flux of 0.91 mg m-2 s-1 in January and a minimum value of 0.19 mg m-2 s-1 in June. The hourly mean CO2 concentration was found to show a significant diurnal variation; a maximum at 0700-0900 LST and a minimum at 1400-1600 LST, with a large diurnal range in winter and a small one in summer, mainly caused by diurnal changes in mixing height, CO2 flux, and surface complexity. The hourly mean CO2 flux was also found to show a significant diurnal variation, but it showed two maxima at 0700-0900 LST and 2100-2400 LST, and two minima at 1100-1500 LST and 0300-0500 LST, mainly caused by a diurnal pattern in CO2 emissions and sinks from road traffic, domestic heating and cooking by liquefied natural gas use, and the different horizontal distribution of CO2 sources and sinks near the site. Differential advection with respect to wind direction was also found to be a cause of diurnal variations in both the CO2 concentration and flux.

ELK3-ID4 axis governs the metastatic features of triple negative breast cancer

Purpose:Cancer cell metastasis is a multistep process,and the mechanism underlying extravasation remains ***3 is a transcription factor that plays a crucial role in regulating various cellular processes,including cancer *** on the finding that ELK3 promotes the metastasis of triple-negative breast cancer(TNBC),we investigated whether ELK3 regulates the extravasation of TNBC by forming the ELK3-ID4 ***4 functions as a transcriptional regulator that interacts with other transcription factors,inhibiting their activity and subsequently influencing various biological processes associated with cell differentiation,survival,growth,and ***:We assessed the correlation between the expression of ELK3 and that of ID4 in TNBCs using bioinformatics analyses,QRT-PCR,western blot analysis,luciferase reporter assays,and chromatin ***,adhesion,invasion,and lung metastasis assays were employed to determine whether the ELK3-ID4 axis regulates the metastatic features of ***:We found that ELK3 binds directly to a binding motif close to the ID4 promoter to repress promoter *** expression of E-cadherin in TNBC was regulated by the ELK3-ID4 *** vitro and in vivo analyses showed that inhibiting ID4 expression in ELK3-knockdown MDA-MB-231(ELK3KD)cells restored the ability to extravasate and ***:The results indicate that the ELK3 regulates ID4 promoter activity,and that the ELK3-ID4 axis regulates the metastatic characteristics of TNBC ***,the data suggest that the ELK3-ID4 axis regulates metastasis of TNBCs by modulating expression of E-cadherin.

BACKGROUND Polymyxin B hemoperfusion(PMX-HP)has been used as a treatment for intraabdominal septic shock by absorbing and removing endotoxins of gram-negative *** To investigate the clinical efficacy of PMX-HP in patients with gram-negative septic shock who underwent abdominal *** From January 2012 to December 2018,patients who had septic shock secondary to peritonitis were *** were classified into PMX-HP treated and control groups based on postopreative intervention using *** clinical outcomes were compared using 1:1 propensity score matching methods to balance the overall distribution between the two *** After propensity score matching,40 patients were analyzed(20 patients in the PMX group and 20 patients in the control group).The scores of total Sequential Organ Failure Assessment(SOFA)score,renal SOFA and coagulation SOFA were significantly improved in the PMX group but not in the control group.(from 11.2±5.8 to 4.7±3.5 in PMX group vs 10.0±4.0 to 8.7±7.3 in control group,P=0.047 from 2.6±1.0 to 0.7±1.0 in PMX group vs 2.6±1.5 to 2.8±1.6 in control group,P=0.000,from 1.6±1.5 to 1.3±1.3 in PMX group vs 1.2±1.2 to 2.8±1.8 in control group,P=0.014,respectively).Further,the length of intensive care unit(ICU)stay was significantly shorter in PMX ***,no statistically significant difference was found in ICU mortality(50%in PMX group vs 50%in control group).CONCLUSION PMX-HP is a feasible adjunct treatment for peritonitis in ICU patients with peritonitis for improved organ impairment and to stabilize *** would be helpful to enhance clinical outcomes especially in patients with complete elimination of the source of gram-negative bacilli infection by surgical procedure accompanied with conventional treatment of sepsis.

Metachronous bile duct cancer nine years after resection of gallbladder cancer

We report a rare case of a 74-year-old man with metachronous gallbladder cancer and bile duct cancer who underwent curative resection twice, with the operations nine years apart. At the age of 65 years, the patient underwent a cholecystectomy and resection of the liver bed for gallbladder cancer. This was a well-differentiated adenocarcinoma, with negative resection margins (T2N0M0, stageB). Nine years later,during a follow-up examination, abdominal computed tomography and MRCP showed an enhanced 1.7 cm mass in the hilum that extended to the second branch of the right intrahepatic bile duct. We diagnosed thislesion as a perihilar bile duct cancer, Bismuth type.

Complete remission of advanced hepatocellular carcinoma by sorafenib: A case report

Hepatocellular carcinoma (HCC) is the fifth most common malignant disease worldwide, and curative treatment remains difficult because the majority of cases are diagnosed in the advanced stage. Sorafenib is the only known effective systemic treatment, but patients rarely achieve complete remission (CR). A 66-year-old man with a history of alcoholic liver cirrhosis with a diagnosis of advanced HCC, was initially treated with transarterial chemoembolization on four occasions. However, the disease progressed with portal vein thrombosis. Therefore, sorafenib was started, and 4 mo later, the patient achieved CR. The treatment was continued for 12 mo, and CR was maintained up to 4 mo after sorafenib discontinuation.

Canopy Net Photosynthesis Rate of a Mongolian Oak (<i>Quercus mongolica</i>) Forest Estimated by Field Experimental Data

The canopy net photosynthesis rate of Mongolian oak (Quercus mongolica) tree species that are dominant in East Asia and Korea is estimated with empirical models derived from field experimental data obtained from the Nam-San site in Seoul, Korea for the growing period from early May to late October in 2010. The empirical models include the attenuation function of photosynthetic photon flux density (PPFD) (r2 = 0.98-0.99, p r2 = 0.99, p < 0.001) derived from the measured data at several levels within the canopy. The incident PPFD at each level within canopy significantly varies diurnally and seasonally due to the seasonal variation of the total plant area index (TPAI = leaf area index + wood silhouette area index) and the light shielding effect of light path-length through the canopy in association with the variation of solar elevation angle. Consequently, a remarkable seasonal variation of the total canopy net photosynthesis rate of Q. mongolica forest stand is found for its growing period. The PPFD exceeding 1000 μmol m-2·s-1 is found to cause the decrease of net photosynthesis rate due to the thermal stress in the early (May) and late (September) growing period. During the whole growing season, the estimated total canopy net photosynthesis rate is found to be about 3.3 kg CO2 m-2.

Impact of statin intensity on adverse cardiac and cerebrovascular events in older adult patients with myocardial infarction

BACKGROUND There is insufficient evidence regarding the effect of high-intensity statin therapy in older *** study aimed to investigate the effects of high-intensity statin treatment on the clinical outcomes in older adults with myocardial infarc-tion(MI).METHODS Consecutive patients with MI aged at least 75 years were analyzed *** primary endpoint was major adverse cardiac and cerebrovascular events(MACCE),defined as a composite of all-cause death,MI,rehospitalization due to un-stable angina,repeat revascularization,and ischemic *** high-intensity group was compared to the low-to-moderate in-tensity group in the propensity score-matched *** Average age of total 546 patients was 81 *** them,84%of patients underwent percutaneous coronary *** unadjusted seven-year MACCE rate differed by statin intensity(high-intensity statin group:38%,moderate-intensity statin group:42%,low-intensity statin group:56%,and no-statin group:61%,P=0.004).However,among these groups,many baseline characteristics were significantly *** the 74 propensity score-matched pairs,which lacked any significant differences in all baseline characteristics,the high-intensity group had a significantly lower rate of MACCE than the low-to-moderate intensity group(37%vs.53%,P=0.047).Follow-up low-density lipoprotein cholesterol levels were significantly lower in the high-intensity group than that in the low-to-moderate intensity group(69.4±16.0 mg/dL vs.77.9±25.9 mg/dL,P=0.026).CONCLUSIONS In older adult patients with MI,the use of high-intensity statin caused significantly less occurrence of MACCE in comparison to that in low-to-moderate intensity for up to seven years of follow-up.
