Substantial increase of heat-induced labor and economic loss in China under rapid economic and environmental temperature growth

Exposure to heat stress causes diminished work performance and leads to economic *** the last two decades,China has experienced both rapid economic development and environmental warming;however,the spatiotemporal variations of association between environmental heat and labor loss have been rarely investigated behind this double ***,we evaluated the variation in China's heat-induced labor productivity loss and related economic cost from 2001 to 2019 and investigated the effect of economic development and regional *** found that decline in labor productivity because of heat stress increased significantly(p<0.05 for heavy-intensity work),and the corresponding economic loss also exhibited a drastic increase by 6-to *** relative economic loss showed a slight but nonsignificant increase with an average value of 0.54%of the annual total *** the sub-regional and sectoral level,adverse effects were more prominent in the southeast region,and the secondary industry sectors,such as construction and manufacturing,contributed to larger proportions of economic *** transformation of industrial structure made economic sectors more vulnerable to heat exposure,and increasingly preventive effects of air-conditioning use were noted on economic damage due to heat-induced productivity decline(36.0%relative economic benefits in 2019 compared to 9.7%in 2001).Our findings could provide a deep insight into heat burden on occupational health and heat adaptation strategies regionally under climate change,especially in developing areas with higher temperature and humidity environment.

Background:There is no consensus on whether triplet regimen is better than doublet regimen in the first-line treatment of advanced gastric cancer(AGC).We aimed to compare the efficacy and safety of oxaliplatin plus capecitabine(XELOX)and epirubicin,oxaliplatin,plus capecitabine(EOX)regimens in treating ***:This phase III trial enrolled previously untreated patients with AGC whowere randomly assigned to receive the XELOXor *** primary endpoint was non-inferiority in progression-free survival(PFS)for XELOX as compared with EOX on an intention-to-treat ***:Between April 10,2015 andAugust 20,2020,448AGCpatientswere randomized to receive XELOX(n=222)or EOX(n=226).The median PFS(mPFS)was 5.0 months(95%confidence interval[CI]=4.5-6.0 months)in the XELOX arm and 5.5 months(95%CI=5.0-6.0 months)in the EOX arm(hazard ratio[HR]=0.989,95%CI=0.812-1.203;P_(non-inferiority)=0.003).There was no significant difference inmedian overall survival(mOS)(12.0 vs.12.0months,P=0.384)or objective response rate(37.4%vs.45.1%,P=0.291)between the two *** patients with poorly differentiated adenocarcinoma and liver metastasis,the EOX arm had a significantly longer mOS(P=0.021)and a trend of longer mPFS(P=0.073)than the XELOX *** rate of grade 3/4 adverse events(AEs)was 42.2%(90/213)in the XELOX arm and 72.5%(156/215)in the EOX arm(P=0.001).The global health-related quality of life(QoL)score was significantly higher in the XELOX arm than in the EOX arm during ***:This non-inferiority trial demonstrated that the doublet regimen was as effective as the triplet regimen and had a better safety profile and QoL as a first-line treatment for AGC ***,the triplet regimen might have a survival advantage in patients with poorly differentiated adenocarcinoma and liver metastasis.

Raman amplification of Stokes pulse in ultra-small silicon-on-insulator optical waveguide

The stimulated Raman amplification of picosecond Stokes pulse is numerically investigated in ultra-small silicon-on- insulator optical waveguide. Numerical results show that we obtain the gain of up to 30-dB for weak Stokes pulse in the co- propagation configuration for 10 mm Jength waveguide using high intensity pump optical pulse. The peak gain, pulse width, rise time, and fall time of Stokes pulse will experience the variation course of decaying then increasing with increas- ing waveguide length. The time delay of output Stokes pulse is controlled by adjusting the initial time delay of both pump and Stokes pulses.

Background:To analyze the expression of cluster of differentiation 80(CD80)/cluster of differentiation 163(CD163)in colorectal cancer(CRC)patients and their correlation with the syndrome of traditional Chinese medicine,pathology and ***:(1)The correlation between the pathological characteristics of 232 postoperative CRC patients and the deficiency and excess syndromes of traditional Chinese medicine was analyzed using the chi-square test,Spearman’s correlation,and Cox regression methods.(2)Immunohistochemistry and quantitative real-time PCR were used to detect the expression of CD80 and CD163 in cancer and paracancer tissues of CRC patients.(3)The relationships between the changes of CD80 and CD163 and the prognosis of CRC patients with deficiency *** survival were analyzed using cardinality and Cox regression proportional-hazards model ***:(1)The degree of differentiation and tumor node metastasis stage of CRC were statistically different between patients with deficiency and excess syndromes(P<0.05);carcinoembryonic antigen and carbohydrate antigen 19-9 were highly expressed in the excess syndrome group,and both were significantly correlated with the distribution of traditional Chinese medicine syndromes(P<0.001);deficiency and excess syndromes,and carbohydrate antigen 19-9 were all independent factors affecting the postoperative survival of CRC patients.(2)The distribution of post-operative survival in CRC patients was significantly correlated with the distribution of the disease type(P<0.001).(3)The expression levels of CD163 protein and message RNA were significantly higher in CRC cancer tissues than in paraneoplastic tissues(P<0.001);whereas the expression of CD80 was significantly higher in paraneoplastic tissues than in cancer tissues(P<0.001).(4)The expression levels of CD80/CD163 were significantly different in different parts of the tissues of patients with deficiency and excess syndromes(P<0.001).(5)CRC patients w

Early Kinetics of Procalcitonin in Predicting Surgical Outcomes in Type A Aortic Dissection Patients

Background： In cardiac surgery, elevation of procalcitonin （PCT） could be observed postoperatively in the absence of any evidence of infection and also seems to be a prognostic marker. PCT levels measured in patients undergoing Type A aortic dissection （TAAD） were used to determine prognostic values for complications and surgical outcomes. Methods： Measurements of PCT, C-reactive protein （CRP）, and leukocyte count were observed in TAAD surgery patients （n = 251 ; average age： 49.02 ± 12.83 years; 78.5% male） at presurgery （TO） and 24 h （T1）, 48 h （T2）, and 7 days （T3） postsurgery. PCT clearance （PCTc） oll days 2 and 7 was calculated：（PCTd - PCTd,±,, d,,v）/PCTdx 100%. Endotracheal intubation duration, length of stay （LOS） in the Intensive Care Unit （1CU）/hospital, and complications were recorded± Results： PCT peaked 24 h postsurgery （median 2.73 ng/ml） before decreasing, Correlation existed between PCT levels at T1 and duration of cardiopulmonary bypass （P = 0.001, r = 0.278）. Serum PCT concentrations were significantly higher in nonsurvivor and multiple organ dysfunction syndrome groups on all postoperative days. PCT levels at T1 correlated with length of time of ventilation support and ICU/hospital LOS. Comparing PCT values of survivors versus nonsurvivors, a PCT cutoff level of 5.86 ng/ml at T2 had high sensitivity （70.6%） and specificity （74.3%） in predicting in-hospital death. PCTc-day 2 and 7 were significantly higher in survivor compared with nonsurvivor patients （38% vs. 8%, P = 0.012, 83% vs. -39%, P 〈 0.001）. A PCTc-day 7 cutoff point of 48.7% predicted survival with high sensitivity （77.8%） and specificity （81.8%）. Conclusions： PCT level and PCTc after TAAD surgery might serve as early prognostic markers to predict postoperative outcome. PCT measurement may help identify high-risk patients.
