Gasless Laparoscopic Surgery plus Abdominal Wall Lifting for Giant Hiatal Hernia——Our Single-center Experience

Giant hiatal hernia（GHH） comprises 5% of hiatal hernia and is associated with significant *** traditional operative procedure,no matter transthoracic or transabdomen repair of giant hiatal hernia,is characteristic of more invasion and more *** laparoscopic repair as a minimally invasive surgery is accepted,a part of patients can not tolerate pneumoperitoneum because of combination with cardiopulmonary diseases or severe posterior mediastinal and neck emphesema during *** aim of this article was to analyze our experience in gasless laparoscopic repair with abdominal wall lifting to treat the giant hiatal *** performed a retrospective review of patients undergoing gasless laparoscopic repair of GHH with abdominal wall lifting from 2012 to 2015 at our *** GHH was defined as greater than one-third of the stomach in the *** laparoscopic repair of GHH with abdominal wall lifting was attempted in 27 *** age was 67 *** results showed that there were no conversions to open surgery and no intraoperative *** mean duration of operation was 100 min（range：90–130 min）.One-side pleura was injured in 4 cases（14.8%）.The mean postoperative length of stay was 4 days（range：3–7 days）.Median follow-up was 26 months（range：6–38 months）.Transient dysphagia for solid food occurred in three patients（11.1%）,and this symptom disappeared within three *** was one patient with recurrent hiatal hernia who was reoperated *** patients still complained of heartburn three months after *** reoperation nor endoscopic treatment due to signs of postoperative esophageal stenosis was required in any ***,satisfactory outcome was reported in 88.9% *** was concluded that the gasless laparoscopic approach with abdominal wall lifting to the repair of GHH is feasible,safe,and effective for the patients who cannot tolerate the pneumoperitoneum.

Background： Obstructive sleep apnea is strongly associated with obesity, particularly abdominal obesity common in centrally obese males. Previous studies have demonstrated that intra-abdominal pressure （IAP） is increased in morbid obesity, and tracheal traction forces may influence pharyngeal airway collapsibility. This study aimed to investigate that whether IAP plays a role in the mechanism of upper airway （UA） collapsibility via IAP-related caudal tracheal traction. Methods： An abdominal wall lifting （AWL） system and graded CO2 pneumoperitoneum pressure was applied to four supine, anesthetized Guizhou miniature pigs and its effects on tracheal displacement （TD） and airflow dynamics of UA were studied. Individual run data in 3 min obtained before and after AWL and obtained before and after graded pneumoperitoneum pressure were analyzed. Differences between baseline and AWL/graded pneumoperitoneum pressure data of each pig were examined using a Student＇s t-test or analysis of variance. Results： Application of AWL resulted in decreased IAP and significant caudal TD. The average displacement amplitude was 0.44 mm （P 〈 0.001 ）. There were three subjects showed increased tidal volume （TV）（P 〈 0.01 ） and peak inspiratory airflow （P 〈 0.01 ）; however, the change of flow limitation inspiratory UA resistance （Rua） was not significant. Experimental increased lAP by pneumoperitoneum resulted in significant cranial TD. The average displacement amplitude was 1.07 mm （P 〈 0.001） when lAP was 25 cmH2O compared to baseline. There were three subjects showed reduced Rua while the TV increased （P 〈 0.01 ）. There was one subject had decreased TV and elevated Rua （P 〈 0.001）. Conclusions： Decreased IAP significantly increased caudal TD, and elevated lAP significantly increased cranial TD. However, the mechanism of UA collapsibility appears primarily mediated by changes in lung volume rather than tracheal traction effect. TV plays an independent role in the mechanism of UA collapsibil

Features and variations of a radial artery approach in southern Chinese populations and their clinical significance in percutaneous coronary intervention

Background More and more percutaneous coronary intervention were done from radial artery approach. But the great limitation of radial artery approach and main failure cause of transradial coronary intervention is smaller size and more variations of a radial artery approach. The aim of the study is to explore the features and variations of a radial artery approach in southern Chinese populations and their clinical significance in percutaneous coronary intervention. Methods A total of 1400 patients who underwent scheduled first time transradial coronary angiography between July 2007 and September 2010 were enrolled. Radial arteriography was performed in all patients to detect the anatomical variations of this vessel. All patients＇ radial and ulnar artery inner diameters were measured using a computer assisted quantification method. A detailed patient history was recorded. Multivariate Logistic regression analysis was performed to evaluate the predictive value of variables （including age, gender, ethnicity, height, weight, body mass index, smoking, diabetes, hypertension and dyslipidemia） in arterial tortuosities and variations of this vessel. Results In southern Chinese populations, there were no significant differences in the diameters of the forearm arteries： the mean radial artery inner diameter was （3.04±0.43） mm in ethnic Han Chinese and （3.05±0.42） mm in ethnic Zhuang Chinese, P 〉0.05）, the mean ulnar artery inner diameter was （3.03±0.38） mm in Han Chinese and （3.05±0.36） mm in Zhuang Chinese, P 〉0.05）. It was estimated that the inner diameter of the radial artery was not smaller than a 6F Cordis sheath in 86.1% of male patients and in 57.0% of female patients, and not smaller than a 7F Cordis sheath in 59.3% of male patients and 24.9% of female patients. Background： Decoy receptor 3 （DcR3） is a protein with anti-apoptotic effect that belongs to the tumor necrosis factor receptor superfamily. DcR3 is highly expressed in a variety of malignant tumors including cholangiocarcinoma and its expression was found to be related to the clinical stage, the invasion, and tile metastasis of the tumor. This in vitro study aimed to investigate the effect ofdownregulated expression of DcR3 on cell viability, cell apoptosis, and cell cycle in cholangiocarcinoma cell line TFK-1. Methods： Three different cell lines were cultured： human cholangiocarcinoma TFK-I, human biliary epithelial carcinoma HuCCT-1, and human cholangiocarcinoma RBE. The cholangiocarcinoma cell line with the highest expression of DcR3 was selected for further investigation. The expression of DcR3 was silenced/knocked down by transfection with DcR3-siRNA in the selected cell line. Various biological phenotype parameters such as cell viability, apoptosis, and cell cycle were observed. Results： The mRNA and protein levels of DcR3 were measured in the three cell lines, and TFK-1 was selected. After the treatment with DcR3-siRNA for 48 h, DcR3 mRNA and protein expression in the treatment group were 38.45% （P 〈 0.01 ） and 48.03% （P 〈 0.05） of that of＇the control, respectively. It was found that the cell viability decreased to 61.87% of the control group （P 〈 0.01 ） after the downregulation of DcR3 in cholangiocarcinoma cell line TFK- 1 by transfection with DcR3-siRNA, while tile percentage ofapoptotic cells was 2.98 times as compared with the control group （P 〈 0.05）. Compared with the control group the ratio of G0/G1 increased, and the ratio of G2/M dccreased in the treatment group. However, the differences were not statistically significant. Conclusions： The effect of DcR3 on the growth and apoptosis ofcholangiocarcinoma has been demonstrated. DcR3 is not only a predictive marker for malignant tumor but it is also likely to be a potential target for cancer gene therap

Observation of acoustic hybrid topological phases induced by the p-d orbital interactions

In this paper, cotton fabric for soft functional finishing of soft finished cotton fabrics and fabric processing carried out without the function of the size parameters and key performance indicators to take the test to compare softening properties of fabric before and after the experiment showed that: After a soft finishing fabric to reduce its permeability, and the higher the concentration of softening agent, then the greater the decline in its permeability;drape properties, wear resistance, crease flexibility, the apparent thickness has been enhanced, and the higher the concentration of softening agent, then the performance gains are obvious;with regard to fabric style, when there is relatively high concentration of softening agent, it can clearly see that the fabric-style indicators improved. Therefore, soft finishing of cotton fabrics considering the performance improvement is beneficial.

In spinless systems,growing attention has recently been attracted to synthetic gauge fields,which redefine the fundamental crystal symmetries by utilizing the projective algebraic ***,synthetic gauge fields have been commonly investigated in single orbital systems,and explorations on hybrid orbitals remain elusive in ***,we report the experimental observation of hybrid topological phases induced by hybrid synthetic gauge flux,which is formed by the interaction between p and d orbitals embedded in acoustic *** breaking the translation symmetries of Lxand/or Ly,we unambiguously demonstrate the first-order M?bius edge states and higher-order corner *** work establishes a platform for seeking exotic topological phases induced by hybrid-orbitals interactions and initializing the framework of hybrid-orbitalsrelated topological *** applications can be anticipated in scenarios such as high-quality sensing and robust wave trapping due to the stepwise energy distribution of the hybrid topological phase.
