Perinatal transmission in infants of mothers with chronic hepatitis B in California

To evaluate maternal hepatitis B virus (HBV) DNA as risk for perinatal HBV infection among infants of HBV-infected women in California. METHODSRetrospective analysis among infants born to hepatitis B surface antigen (HBsAg)-positive mothers who received post vaccination serologic testing (PVST) between 2005 and 2011 in California. Demographic information was collected from the California Department of Public Health Perinatal Hepatitis B Program databaseand matched to birth certificate records. HBV DNA level and hepatitis B e antigen (HBeAg) status were obtained from three large commercial laboratories in California and provider records if available and matched to mother infant pairs. Univariate analysis compared infected and uninfected infants. Multivariate analysis was restricted to infected infants and controls with complete maternal HBV DNA results using a predefined high HBV DNA level of > 2 × 107 IU/mL, a 5:1 ratio of cases to controls and a two-sided confidence level of 95%. RESULTSA total of 17687 infants were born to HBsAg positive mothers in California between Jan 1 2005 and Dec 31, 2011. Among 11473 infants with PVST, only 125 (1.1%) were found to be HBV infected. Among these infected infants, lapses in Advisory Committee on Immunization Practices recommended post exposure prophylaxis (PEP) occurred in only 9 infants. However, PEP errors were not significantly different between infected and uninfected infants. Among the 347 uninfected and infected infants who had maternal HBeAg and HBV DNA level, case-control analysis found HBeAg positivity (70.4% vs 28.9%, OR = 46.76, 95%CI: 6.05-361.32, P 7 IU/mL (92.6% vs 18.5%, OR = 54.5, 95%CI: 12.22-247.55, P 7 IU/mL was the only significant independent predictor of perinatal HBV infection. Dietary supplements represent an increasingly common source of drug-induced liver injury. Hydroxycut is a popular weight loss supplement which has previously been linked to hepatotoxicity, although the individual chemical components underlying liver injury remain poorly understood. We report two cases of acute hepatitis in the setting of Hydroxycut exposure and describe possible mechanisms of liver injury. We also comprehensively review and summarize the existing literature on commonly used weight loss supplements, and their individual components which have demon-strated potential for liver toxicity. An increased effort to screen for and educate patients and physicians about supplement-associated hepatotoxicity is warranted.

This study focuses on the migration of middle school students to the interior of China after the Japanese invaded in 1937. It argues that the Guomindang （GMD） central government was generally successful in handling the 500,000 displaced students, making substantial efforts to monitor, register, educate, and provide training for them, as well as establishing government-run ＂national middle schools＂ during the war. Meanwhile, the GMD also exerted a strong influence on course curriculum, instructing educators how to implement the Three People＇s Principles and other party doctrines in classrooms. These processes expanded the state＇s hand in secondary education and allowed the GMD to include refugee students and schools in its wartime narrative of progress, praising the students＇ patriotic participation in defying the Japanese occupiers and their contribution to ＂national reconstruction＂（jianguo）. However, there were still many challenges. Refugee students, teachers, and principals forcibly converted Buddhist temples into schools and clashed with local monks, farmers, villagers, and even the GMD military. With schools merging and moving inland, relocation also provided opportunities for unscrupulous administrators and teachers to exploit the situation for themselves, as government reports reveal many cases of corruption in the wartime schools.

The song systems of oscine passerines(songbirds)are complex and *** songs are used for both mate attraction and territory defense and are therefore important signals for survival and reproduction,comprehensive knowledge of within and among species song structure and distribution is informative for understanding the evolution of song repertoires and vocal *** this study,we explored variation in the song structure of the Ovenbird(Seiurus aurocapilla),a widespread warbler(Family Parulidae)found in North American *** analyzed recordings from the 2021 breeding season to assess song type variation at a local(n=158 birds;Sault ***,ON)and breeding range scale(n=512 birds;eBird).We characterized the local song types and tested whether Ovenbirds share song types with their neighbors more often than expected by *** then characterized song types of Ovenbirds across the breeding range to determine whether any geographic pattern of song clustering exists(i.e.,macro-dialects).We found 10 distinct song types and some evidence for song type clustering at the local study site(i.e.,song neighborhoods).We found 7 of those 10 song types throughout the breeding range and identified an additional 24 types that were not recorded in our local *** found no evidence for song dialects across the Ovenbird breeding *** study contributes to our understanding of Ovenbird song while simultaneously adding to our understanding of geographic structuring of warbler *** work contributes to delineating a more comprehensive understanding of factors affecting dialect development for this diverse group of songbirds.

Hepatocellular carcinoma (HCC) is the fourth most common cause of cancer worldwide and as such represents a significant global health burden. The clinical societies dedicated to the study of liver diseases all recommend routine surveillance for those at risk. These societies include the Asian-Pacific, European, and American associations for the study of liver disease. The aim of surveillance is to prolong survival, which is to diagnose HCC when the patient remains eligible for potentially curative interventions including resection and transplantation. In a detailed review published recently, Kanwal and Singal discuss the evidence for HCC surveillance, evaluate current surveillance methods and point out new imaging and serological initiatives to improve on current method (1). Their review covers some familiar topics and point to new endeavors in improving the effectiveness of HCC surveillance. In regards to their review article, several important points need to be emphasized as follows.

Hepatocellular carcinoma(HCC) is the fifth leading cause of cancer mortality in the United States and the second leading cause of cancer mortality worldwide. Sorafenib is the only food and drug administration(FDA) approved as first line systemic treatment in HCC. Regorafenib and nivolumab are the only FDA approved second line treatment after progression on sorafenib. We will discuss all potential first and second line options in HCC. In addition, we also will explore sequencing treatment options in HCC, and examine biomarkers that can potentially predict benefits from treatments such as immune checkpoint inhibitor. This minireview summarizes potential treatments in HCC based on clinical trials that have been published in manuscript or abstract format from 1994-2018.
