Association between axial length and toric intraocular lens rotation according to an online toric back-calculator

AIM: To assess the relationship between axial length(Al) and intraocular lens(IOl) rotation among eyes receiving a toric IOl and subsequently entered into an online toric back-calculator ***: Retrospective analysis of data collected online via ***, a freely available online toric back-calculator where surgeons enter pre-and postoperative information to help manage residual postoperative *** records were deemed valid with entry of Al and IOl orientation between January 2017 and March *** was determined by a difference of ≥5° between pre-operative intended IOl orientation and actual post-operative IOl *** and magnitude of rotation are presented with means and associated standard deviation(SD).linear regression models of this association are ***: Records of 6752 eyes were included in the analysis, of which 74.8% were determined to have a rotated *** magnitude of rotation increased with each millimeter(mm) increase in Al with a mean rotation of 13.3°(SD: 12.8°) for eyes with Al 20-20.9 mm and a maximum mean rotation of 30.6°(SD: 30.3°) among eyes with Al 29-29.9 *** linear modeling demonstrated a significant association(Plysis from an online database indicates that toric IOls inserted into eyes with longer Al are more likely to rotate and to rotate more degrees from the target *** findings from this study are clinically relevant for surgeons implanting toric IOls.

Gender as an effect modifler in the relationship between hypertension and reticular pseudodrusen in patients with early or intermediate age-related macular degeneration

AIM: To determine whether the prevalence of treated hypertension is higher among males or females with early/intermediate(e/i) age-related macular degeneration(AMD) with and without bilateral reticular pseudodrusen(RPD).METHODS: Retrospective review of the records of patients with e/iAMD who were recruited into the University of Colorado AMD registry between July 2014 and November *** were classified using the Beckman Initiative criteria and presence/absence of *** were categorized into three groups: 1) e/iAMD with RPD;2) e/iAMD without RPD;3) control patients who did not have *** logistic regression analysis was used for adjusted analysis with odds ratios(OR) and confidence intervals(CI).RESUlTS: There were 260 patients with e/iAMD of which 101 had bilateral RPD and 159 had no RPD, and 221 ***, 62% of patients were female and the three groups did not differ by *** stratified by gender, the female e/iAMD/RPD group had a higher prevalence of hypertension, 64.1% vs 45.2% for controls, OR=2.2(95%CI: 1.2-4.0).The frequency of hypertension in the e/iAMD/no RPD group was 54.1% and did not significantly differ from the control *** males, prevalence rates of treated hypertension did not *** is a significant interaction of hypertension and gender for the e/iAMD/RPD group such that women with e/iAMD who had RPD were significantly more likely to have hypertension(P=0.042).This relationship was not significant in the e/iAMD/no RPD group(P=0.269).CONClUSION: Among females treated hypertension is significantly higher among e/iAMD/RPD patients, whereas for males there is no significant association.

Iris manipulation during phacoemulsification:intraoperative and postoperative complications

AIM:To quantify intraoperative and postoperative complications in complex phacoemulsification cataract extraction(phacoemulsification)with iris manipulation compared to non-complex and complex phacoemulsification without iris ***:All phacoemulsification cases at the University of Colorado between January 1,2014,and June 30,2017 were *** criteria for the primary outcome of intraoperative complications were planned combination surgery and eyes with less than 28 d *** criteria for the secondary outcomes of postoperative complications were unplanned additional surgery,and chronic steroid eye drop use prior to *** including sex,race/ethnicity,surgery length,visual acuity,intraoperative and postoperative complications,and intraocular pressures(IOP)were collected and analyzed utilizing general linear and logistic regression ***:The medical records of 5772 eyes were reviewed(500 complex without iris manipulation,367 with iris manipulation).The number of any intraoperative complication in the complex with iris manipulation and complex without iris manipulation groups was 15(4.1%)and 26(5.2%),respectively,compared to 41(0.8%)in the non-complex *** inflammation was found in 135(2.8%)non-complex cases,20(4.1%)complex cases without iris manipulation,and 20(5.6%)complex cases with iris *** adjusted odds ratio of postoperative inflammation in phacoemulsification with iris manipulation compared to non-complex was 2.3(95%CI:1.3-4.0,P=0.005).The rate of IOP spikes>10 mm Hg was significantly greater in cases with iris manipulation(P=0.001).CONClUSION:Complex cases have more intraoperative ***,only complex cases with iris manipulation led to increase rates of postoperative inflammation and IOP spikes>10 mm Hg.

Are there sex-based disparities in cataract surgery?

AIM:To investigate sex-based differences in the occurrence of intra-operative and post-operative complications and associated visual outcomes following cataract ***:This was a retrospective study of patients who had phacoemulsification cataract surgery at the University of Colorado School of *** collected included the patient’s health history,ocular comorbidities,operative and post-operative complications,and the post-operative best corrected visual acuity(BCVA).The data were analyzed using univariate and multivariable logistic regression with generalized estimating equations to account for the correlation of some patients having two eyes included in the ***:A total of 11977 eyes from 7253 patients were included in the *** comorbidities differed by sex,with males having significantly higher percentages of traumatic cataracts(males 0.7%vs females 0.1%),prior ocular surgery(6.7%vs 5.5%),and mature cataracts(2.8%vs 1.9%).Conversely,females had significantly higher rates of pseudoexfoliation(2.0%vs 3.2%).In unadjusted analysis,males had higher rates of posterior capsular rupture(0.8%vs 0.4%)and vitreous loss(1.0%vs 0.6%),but this difference was not significant after adjustment for *** had a significantly increased risk of post-operative retinal detachment,but in multivariable analysis this was no longer *** were significantly less likely to undergo post-operative neodymium-doped yttrium aluminum garnet(Nd:YAG)laser capsulotomy for posterior capsule opacification(OR=0.8,95%CI=0.7-0.9,P=0.0005).The BCVA was slightly worse for males pre-operatively;but post-operatively,both sexes exhibited similar visual acuity of Snellen equivalent 20/***:The study finds that in a cohort of patients presenting for cataract surgery,sex differences exist in pre-operative comorbidities and surgical characteristics that contribute to higher rates of some complications for ***,observed surgical complicatio

Refractive outcomes among glaucoma patients undergoing phacoemulsification cataract extraction with and without Kahook Dual Blade goniotomy

Background:Glaucoma patients undergoing phacoemulsification alone have a higher rate of refractive surprise compared to patients without *** risk is further increased with combined filtering ***,there are few and conflicting reports on the effect of combined phacoemulsification and micro-invasive glaucoma surgery(MIGS).Here,we look at refractive outcomes of glaucoma patients undergoing phacoemulsification with and without Kahook Dual Blade(KDB)***:Retrospective chart review of 385 glaucomatous eyes of 281 patients,which underwent either phacoemulsification alone(n=309)or phacoemulsification with KDB goniotomy(n=76,phaco-KDB)at the University of *** main outcome was refractive surprise defined as the difference in target and postoperative refraction spherical equivalent greater than±0.5 Diopter(D).Results:Refractive surprise greater than±0.5 D occurred in 26.3%of eyes in the phaco-KDB group and 36.2%in the phacoemulsification group(p=0.11).Refractive surprise greater than±1.0 D occurred in 6.6%for the phacoKDB group and 9.7%for the phacoemulsification group(p=0.08).There was no significant difference in risk of refractive surprise when pre-operative IOP,axial length,keratometry or performance of KDB goniotomy were assessed in univariate ***:There was no difference between refractive outcomes of glaucomatous patients undergoing phacoemulsification with or without KDB goniotomy.

Comparison of intraocular pressure readings with Perkins,Tonopen,iCare 200,and iCare Home to manometry in cadaveric eyes

AIM:To compare intraocular pressure(IOP)readings obtained with Perkins tonometry,i Care Home,i Care 200,and Tonopen to IOP readings obtained with the manometer of a perfusion system to assess the accuracy and reproducibility of each method of tonometry at set ***:The IOP of human cadaveric eyes(n=2)was measured using a manometer inserted into the eye through the optic *** measurements were obtained using a Perkins tonometer,i Care Home,i Care 200,and *** measurements were compared to set point IOP measurements of a manometer to determine accuracy and reproducibility of each ***:Mean IOP readings obtained with the Perkins tonometer compared to manometer readings demonstrated a difference of-1.0±5.0 mm Hg(P=0.45),indicating a lower reading on average than manometery although not *** IOP difference between iCare 200 and manometer was 5.3±2.2 mm Hg(Ple the Tonopen measurements demonstrated a 4.0 mm Hg standard *** comparison,iCare 200 and iCare Home demonstrated 2.2 and 2.4 mm Hg standard deviation,***:Applanation tonometry produces more accurate IOP readings than rebound tonometry or Tonopen,however it demonstrates greater variability than the other forms of *** tonometry is more reproducible but tends to over-estimate IOP.

The effect of gas phase polydimethylsiloxane surface treatment of metallic aluminum particles： Surface characterization and flow behavior

Aluminum particles were exposed to gaseous polydimethylsiloxane （PDMS） to produce a hydropho- bic surface coating for enhanced flow and fluidity. Surface retention of the intact PDMS was confirmed through infrared and X-ray photoelectron spectroscopy. Transmission electron microscopy was used to image cross-sections of the treated particles and energy dispersive spectroscopy element maps demon- strated the presence of a surface layer consisting of silicon and oxygen. Density measurements provided evidence for improvements in the Hausner ratio and Carr index of the PDMS-treated aluminum, indicating a reduction in inter-particulate cohesion through increased bulk density. Stability, compressibility, shear, aeration, and permeability of the particles were assessed by powder rheometer. The compressibility was reduced by approximately 32% following surface treatment, revealing a reduction in void space, while Mohr＇s circle analysis and shear testing determined that the extrapolated cohesion value was reduced by approximately 53% and the flow factor at 6 kPa was doubled. Aeration testing showed that the air velocity required to obtain a fluidized bed was on the order of 0.35 mm/s for the treated powder, whereas the raw powder could not be uniformly fluidized. PDMS may be a viable option for the large-scale treatment of aluminum powder for flow applications.

Perspectives:Gene expression in fisheries management

Functional genes and gene expression have been connected to physiological traits linked to effective production and broodstock selection in aquaculture, selective implications of commercial fish harvest, and adaptive changes reflected in non-commercial fish populations subject to human disturbance and climate change. Gene mapping using single nucleotide polymorphisms （SNPs） to identify functional genes, gene expression （analogue microarrays and real-time PCR）, and digital sequencing technologies looking at RNA transcripts present new concepts and opportunities in support of effective and sustainable fisheries. Genomic tools have been rapidly growing in aquaculture research addressing aspects of fish health, toxicology, and early development. Genomic technologies linking effects in functional genes involved in growth, maturation and life history development have been tied to selection resulting from harvest practices. Incorporating new and ever-increasing knowledge of fish gehomes is opening a different perspective on local adaptation that will prove invaluable in wild fish conservation and management. Conservation of fish stocks is rapidly incorporating research on critical adaptive responses directed at the effects of human dis- turbance and climate change through gene expression studies. Genomic studies of fish populations can be generally grouped into three broad categories： l） evolutionary genomics and biodiversity; 2） adaptive physiological responses to a changing environment; and 3） adaptive behavioral genomics and life history diversity. We review current genomic research in fisheries focusing on those that use microarrays to explore differences in gene expression among phenotypes and within or across populations, information that is critically important to the conservation of fish and their relationship to humans [Current Zoology 56 （1）： 157-174, 2010].

Frailty is independently associated with increased hospitalisation days in patients on the liver transplant waitlist

AIMTo investigate the impact of physical frailty on risk of hospitalisation in cirrhotic patients on the liver transplant *** listed for liver transplantation at a single centre underwent frailty assessments using the Fried Frailty Index, consisting of grip strength, gait speed, exhaustion, weight loss, and physical activity. Clinical and biochemical data including MElD score as collected at the time of assessment. The primary outcome was number of hospitalised days per year; secondary outcomes included incidence of infection. Univariable and multivariable analysis was performed using negative binomial regression to associate baseline parameters including frailty with clinical outcomes and estimated incidence rate ratios (IRR).RESUlTSOf 587 cirrhotics, 64% were male, median age (interquartile range) was 60 (53-64) years and MElD score was 15 (12-18). Median Fried Frailty Index was 2 (1-3); 31.6% were classified as frail (fried frailty ≥ 3). During 12 mo of follow-up, 43% required at least 1 hospitalisation; 38% of which involved major infection. 107/184 (58%) frail and 142/399 (36%) non-frail patients were hospitalised at least once (P le analysis, Fried Frailty Index was associated with total hospitalisation days per year (IRR = 1.51, 95%CI: 1.28-1.77; P ≤ 0.001), which remained significant on multivariable analysis after adjustment for MElD, albumin, and gender (IRR for frailty of 1.21, 95%CI: 1.02-1.44; P = 0.03). Incidence of infection was not influenced by *** cirrhotics on the liver transplant waitlist, physical frailty is a significant predictor of hospitalisation and total hospitalised days per year, independent of liver disease severity.
