Addition of c.psaic.n to Loc.l Anesthetic. for Spinal Anesthesia in Rats Shortens Motor Defic.ts and Prolongs Anti-Noc.c.ption

Bac.ground and Objec.ives: Sensory-selec.ive anesthesia, greater or longer-lasting anti-noc.c.ption than motor or autonomic.defic.ts, is often c.inic.lly desirable but traditional loc.l anesthetic. rarely have suc. selec.ive ac.ions. Addition of c.psaic.n to tertiary amine loc.l anesthetic. has rec.ntly been reported to affec. a preferential prolongation of noc.c.ptive over motor bloc. in rat sc.atic.nerve. We hypothesized that this c.mbination when used intrathec.lly will also prolong noc.c.ptive bloc.. Methods: Under sevoflurane inhalation anesthesia, rats were injec.ed intrathec.lly either with loc.l anesthetic. (bupivac.ine, lidoc.ine, and artic.ine) alone or simultaneously with c.psaic.n. Motor bloc. was evaluated by the c.ntrac.ile func.ion of foot musc.es, from proximal to distal. Anti-noc.c.ption was assessed by reduc.ions in noc.fensive withdrawal and voc.lization induc.d by pinc.ing the skin fold over the lateral metatarsus. Durations and degrees of defic.ts were assessed, along with c.mplete rec.very times and c.mpared between loc.l anesthetic. alone and in c.mbination with c.psaic.n. Results: Addition of c.psaic.n to any of the loc.l anesthetic. shortened motor defic.ts. Bupivac.ine, lidoc.ine and artic.ine motor bloc.s were reduc.d upon c.mbination with c.psaic.n to 0.32, 0.32 ans 0.43 of the duration from the respec.ive loc.l anesthetic.alone. Duration of anti-noc.c.ptive ac.ion was inc.eased by c.psaic.n only for artic.ine. The ratios of bloc. noc.c.ptive to sensory bloc. durations were 3.5, 5.1 and 3.3 for the respec.ive loc.l anesthetic.. c.nc.usions: Intrathec.l injec.ion of c.psaic.n c.mbined with loc.l anesthetic. produc.d a preferentially longer anti-noc.c.ptive defic.t. These c.mbinations have potential c.inic.l applic.tions, inc.uding peri-operative spinal anesthesia and pain management.

In vivo measurement of NADH fluoresc.nc. lifetime in skeletal musc.e via -ber-c.upled time-c.rrelated single photon c.unting

Despite substantial progress in the treatment of c.stration-resistant prostate c.nc.r (c.Pc., inc.uding radiation therapy and immunotherapy alone or in c.mbination, the response to treatment remains poor due to the hypoxic.and immunosuppressive nature of the tumor mic.oenvironment. Herein, we exploited the bioreac.ivity of novel polymer–lipid manganese dioxide nanopartic.es (PLMDs) to remodel the tumor immune mic.oenvironment (TIME) by inc.easing the loc.l oxygen levels and extrac.llular pH and enhanc.ng radiation-induc.d immunogenic.c.ll death. This study demonstrated that PLMD treatment sensitized hypoxic.human and murine c.Pc.c.lls to radiation, signific.ntly inc.easing radiation-induc.d DNA double-strand breaks and ultimately c.ll death, whic. enhanc.d the sec.etion of damage-assoc.ated molec.lar patterns, attributable to the induc.ion of autophagy and endoplasmic.retic.lum stress. Reoxygenation via PLMDs also polarized hypoxic.murine RAW264.7 mac.ophages toward the M1 phenotype, enhanc.ng tumor nec.osis fac.or alpha release, and thus reduc.ng the viability of murine c.Pc.TRAMP-c. c.lls. In a syngeneic.TRAMP-c. tumor model, intravenous injec.ion of PLMDs suppressed, while radiation alone enhanc.d rec.uitment of regulatory T c.lls and myeloid-derived suppressor c.lls. Pretreatment with PLMDs followed by radiation down-regulated programmed death-ligand 1 and promoted the infiltration of antitumor c.8+ T c.lls and M1 mac.ophages to tumor sites. Taken together, TIME modulation by PLMDs plus radiation profoundly delayed tumor growth and prolonged median survival c.mpared with radiation alone. These results suggest that PLMDs plus radiation is a promising treatment modality for improving therapeutic.effic.c. in radioresistant and immunosuppressive solid tumors.
