Bilateral versus unilateral cryptorchidism in nonobstructive azoospermia:testicular sperm extraction outcomes

Cryptorchidism is one of the most frequent causes of nonobstructive azoospermia(NOA)in adulthood.Although it is well known that spermatogenesis is more impaired in bilateral than in unilateral cryptorchidism,previous studies have only described small cohorts or inhomogeneous population.Consequently,we analyzed a cohort of 225 men with only a history of cryptorchidism as sole etiopathogenetic factor for NOA,and compared testicular sperm extraction(TESE)outcomes between men with bilateral versus unilateral cryptorchidism.Our results show no difference in follicle-stimulating hormone(FSH)levels and testicular volumes between men with a history of bilateral cryptorchidism compared to unilateral cryptorchidism(median:21.3 IU I^-1 vs 19.3 IU I^-1,P=0.306;and 7.2 ml vs 7.9 ml,P=0.543,respectively).In addition,sperm retrieval rates were similar(66.2%vs 60.0%,P=0.353).Using multivariate analysis,we have found that only a low inhibin B level(above the assay's detection limit)was positively associated with successful sperm retrieval(P<0.05).Regarding intracytoplasmic sperm injection outcomes,we found that cumulative pregnancy rate and live birth rate per cycle were not statistically different between the two groups(17.4%vs 27.8%,P=0.070;and 16.1%vs 26.4%,P=0.067,respectively).Unexpectedly,there was no significant difference in hormonal profiles(FSH,luteinizing hormone[LH],testosterone,and inhibin B levels)and TESE outcomes between unilateral versus bilateral cryptorchidism.This suggests that a history of unilateral cryptorchidism could reflect a bilateral testicular impairment.Interestingly,inhibin B level might be a predictor of successful TESE.

Dear Editor, The cytosolic protein Tau is naturally present in human neurons, where it has a pivotal role in controlling microtubule stability. Hyperphosphorylation of Tau （observed during neurodegenerative diseases, such as Alzheimer＇s disease） impairs the protein＇s ability to bind microtubules. This results in microtubule disassembly and the formation of Tau aggregates, Tau protein is also widely expressed in peripheral tissues. In the male reproductive system, screening for Tau has focused solely on the rodent and bovine testis. In the present study, we used immunofluorescence and immunoenzymatic techniques （with a Tau-specific antibody） to investigate the presence of Tau protein in human ejaculated sperm and testicular tissue.

Populism as a Crisis of Democratic Legitimacy:With Some References to France and the United States

Populism combines the following characteristics:a rejection of economic policies linked to globalization;a condemnation of political and economic elites perceived as allied,directly or indirectly,to globalization policies;a nationalist conception of the national interest;a mistrust of immigration,seen as a threat to identity;as for populism’s supporters,a rather popular sociological profile,and an electoral profile that tends toward the“hard right”(“droite dure”),without ruling out bridges to the“hard left”(“gauche dure”).Having said this,I propose to defend three theses:populism is not the solution to the problems of the countries where it is developing,whether in France,the United States or elsewhere;it would be dangerous,however,to ignore the crises of which populism is the product;it is essential to take the measure of the problems that favor its emergence and to try to provide answers to them.

Pulsed lavage in joint arthroplasty: A systematic review and metaanalysis

BACKGROUND Knee and hip osteoarthritis affects millions of people around the world and is expected to rise even more in frequency as the population ages.Joint arthroplasty is the surgical management of choice in these articulations.Heterotopic ossi-fication and radiolucent lines formation are two frequent problems faced in hip and knee replacements respectively.Some studies show that the usage of pulsed lavage may prevent their formation.AIM To compare pulsed lavage to standard lavage in joint arthroplasty.METHODS PubMed,Cochrane,and Google Scholar(page 1-20)were searched till December 2023.Only comparative studies were included.The clinical outcomes evaluated were the heterotopic ossification formation in hip replacements,radiolucent lines formation,and functional knee scores in knee replacements.RESULTS Four studies met the inclusion criteria and were included in this meta-analysis.Pulsed lavage was shown to reduce the formation of radiolucent lines(P=0.001).However,no difference was seen in the remaining outcomes CONCLUSION Pulsed lavage reduced the formation of radiolucent lines in knee replacements.No difference was seen in the remaining outcomes.Furthermore,the clinical significance of these radiolucent lines is poorly understood.Better conducted randomized controlled studies and cost-effectivity studies are needed to reinforce these findings.

Microplastics and microbiota:Unraveling the hidden environmental challenge

This editorial explores the intricate relationship between microplastics(MPs)and gut microbiota,emphasizing the complexity and environmental health implications.The gut microbiota,a crucial component of gastrointestinal health,is examined in the context of potential microbial degradation of MPs.Furthermore,dysbiosis induced by MPs emerges as a consensus,disrupting the balance of gut microbiota and decreasing diversity.The mechanisms triggering dysbiosis,including physical interactions and chemical composition,are under investigation.Ongoing research addresses the consequences of MPs on immune function,nutrient metabolism,and overall host health.The bidirectional relationship between MPs and gut microbiota has significant implications for environmental and human health.Despite uncertainties,MPs negatively impact gut microbiota and health.Further research is essential to unravel the complex interactions and assess the long-term consequences of MPs on both environmental and human well-being.

考虑到非晶合金具有独特的无序微观结构，其表现出极为复杂的弛豫动力学特性.动态弛豫行为在非晶合金中普遍存在，深刻理解其机制对于解构非晶合金固有的非均匀微观结构至关重要.少量元素的添加是调控非晶合金微观结构有效手段之一.本研究以玻璃形成能力良好的Cu46Zr47-x Hfx Al7 (x=0, 8)非晶合金为模型合金，运用动态力学分析方法研究复模量随频率和温度的变化，进而探究Hf元素取代Zr元素对动态弛豫的影响.在动态力学频率谱中，借助准点缺陷理论阐明非晶合金在玻璃转变温度Tg以下处于等构型状态，准点缺陷浓度保持恒定；而在Tg以上，准点缺陷浓度随温度的上升呈线性增加. Hf元素的添加会降低玻璃体系等构型状态下的准点缺陷浓度，并抑制原子迁移能力.通过获取连续和离散的弛豫时间分布，直观展示了非晶合金的动力学非均匀性特征.在动态力学温度谱中，Hf元素的引入增加了玻璃体系的混合焓，抑制了β弛豫强度.进一步从内耗角度表明，Hf元素的引入不仅提高了与β弛豫相关的基本结构单元的激活难度，还增加了整个β弛豫过程的激活能.

NRF2 signaling cascade in amyotrophic lateral sclerosis:bridging the gap between promise and reality

Amyotrophic lateral sclerosis is a very disabling disease due to the degeneration of motor neurons.Symptoms include muscle weakness and atrophy,spasticity,and progressive paralysis.Currently,there is no treatment to reverse damage to motor neurons and cure amyotrophic lateral sclerosis.The only two treatments actually approved,riluzole and edaravone,have shown mitigated beneficial effects.The difficulty to find a cure lies in the complexity and multifaceted pattern of amyotrophic lateral sclerosis pathogenesis.Among mechanisms,abnormal RNA metabolism,nucleocytoplasmic transport defects,accumulation of unfolded protein,and mitochondrial dysfunction would in fine induce oxidative damage and vice versa.A potent therapeutic strategy will be to find molecules that break this vicious circle.Sharpening the nuclear factor erythroid-2 related factor 2 signaling may fulfill this objective since nuclear factor erythroid-2 related factor 2 has a multitarget profile controlling antioxidant defense,mitochondrial functioning,and inflammation.We here discuss the interest of developing nuclear factor erythroid-2 related factor 2-based therapy in regard to the pathophysiological mechanisms and we provide a general overview of the attempted clinical assays in amyotrophic lateral sclerosis.

It’s not all in your feet: Improving penalty kick performance with humanavatar interaction and machine learning

Penalty kicks are increasingly decisive in major international football competitions.Yet,over 30%of shootout kicks are missed.The outcome of the kick often relies on the ability of the penalty taker to exploit anticipatory movements of the goalkeeper to redirect the kick toward the open side of the goal.Unfortunately,this ability is difficult to train using classical methods.We used an augmented reality simulator displaying an holographic goalkeeper to test and train penalty kick performance with 13 young elite players.Machine learning algorithms were used to optimize the learning rate by maintaining an optimal level of training difficulty.Ten training sessions of 20 kicks reduced the redirection threshold by 120 ms,which constituted a 28%reduction with respect to the baseline threshold.Importantly,redirection threshold reduction was observed for all trained players,and all things being equal,it corresponded to an estimated 35%improvement of the success rate.

A convolutional neural network to detect possible hidden data in spatial domain images

Hiding secret data in digital multimedia has been essential to protect the data.Nevertheless,attackers with a steganalysis technique may break them.Existing steganalysis methods have good results with conventional Machine Learning(ML)techniques;however,the introduction of Convolutional Neural Network(CNN),a deep learning paradigm,achieved better performance over the previously proposed ML-based techniques.Though the existing CNN-based approaches yield good results,they present performance issues in classification accuracy and stability in the network training phase.This research proposes a new method with a CNN architecture to improve the hidden data detection accuracy and the training phase stability in spatial domain images.The proposed method comprises three phases:pre-processing,feature extraction,and classification.Firstly,in the pre-processing phase,we use spatial rich model filters to enhance the noise within images altered by data hiding;secondly,in the feature extraction phase,we use two-dimensional depthwise separable convolutions to improve the signal-to-noise and regular convolutions to model local features;and finally,in the classification,we use multi-scale average pooling for local features aggregation and representability enhancement regardless of the input size variation,followed by three fully connected layers to form the final feature maps that we transform into class probabilities using the softmax function.The results identify an improvement in the accuracy of the considered recent scheme ranging between 4.6 and 10.2%with reduced training time up to 30.81%.
